摘要
目的观察口面部运动疗法、呼吸训练、口服巴氯芬对脑卒中后双侧咀嚼肌痉挛伴周期性膈肌痉挛致吞咽障碍的康复疗效。方法对2022年7月22日—2022年11月20日在济南医院(山东大学康复医院)康复医学科进行康复治疗的1例脑卒中后双侧咀嚼肌痉挛伴周期性膈肌痉挛致吞咽障碍的患者,经家属及本人知情同意,进行口面部运动疗法、呼吸训练4个月,口服巴氯芬治疗1个月。比较治疗前后的下颌运动功能、功能性经口进食量表分级,打嗝发作天数、间隔天数及发作频率对吞咽功能的影响。结果治疗后患者张口范围明显增大,打嗝对吞咽功能的影响明显缓解,拔除鼻饲管、经口进食。结论口面部运动疗法可明显改善脑卒中后因肌张力异常增高导致的下颌运动功能异常,呼吸训练、口服巴氯芬可有效缓解打嗝对吞咽功能的影响。
Objective To observe the rehabilitation effect of oral and facial exercise therapy,respiratory training,and oral baclofen on swallowing disorders caused by bilateral masticatory muscle spasms and periodic diaphragmatic muscle spasms after stroke.Methods A patient with swallowing disorders caused by bilateral masticatory muscle spasms and periodic diaphragmatic muscle spasms after stroke,who underwent rehabilitation treatment in the Rehabilitation Medicine Department of Jinan Hospital(Jinan Rehabilitation Hospital)from July 22,2022 to November 20,2022,underwent oral and facial exercise therapy and respiratory training for four months with the informed consent of the family and the individual,and oral Baclofen treatment for one month.Compare the mandibular motor function and functional oral intake scale grading before and after treatment,and the effects of the number of days,interval days,and frequency of hiccups on swallowing function.Results After treatment,the patient's mouth opening range significantly increased,and the impact of burping on swallowing function was significantly alleviated.The nasal feeding tube was removed and oral feeding was performed.Conclusion Oral and facial motor therapy can significantly improve mandibular motor dysfunction caused by abnormal increase in muscle tone after stroke,and respiratory training and oral administration of baclofen can effectively alleviate the impact of hiccups on swallowing function.
