静态渐进性牵伸技术结合肌肉能量技术对创伤后肘关节僵硬患者康复效果的影响

The Effect of Static Progressive Stretching Technology Combined with Muscle Energy Technology on the Rehabilitation of Patients with Elbow Joint Stiffness after Trauma

摘要 目的探讨静态渐进性牵伸技术(SPS)结合肌肉能量技术(MET)在创伤后肘关节僵硬患者中的应用效果.方法选择2018年3月—2022年9月无锡市第八人民医院收治的70例创伤后肘关节僵硬患者为研究对象,按随机数字表法将其分为对照组和观察组,各35例.对照组在常规康复治疗的基础上采用MET治疗,观察组在对照组基础上采用SPS治疗,持续治疗4周.对比两组训练前后的关节疼痛度、关节活动度、关节功能及上肢功能.结果训练前,两组关节疼痛度、关节活动度、关节功能及上肢功能比较,组间差异无统计学意义(P>0.05).训练4周后,观察组前臂内旋的疼痛视觉模拟评分为(1.21±0.55)分,前臂外旋的疼痛视觉模拟评分为(1.19±0.48)分,上肢功能评定表评分为(32.44±3.27)分,均低于对照组的(2.74±0.91)分、(2.69±1.65)分、(49.52±3.48)分,组间差异有统计学意义(P<0.05);肘关节主动活动范围大于对照组,Mayo肘关节功能评分高于对照组,组间差异有统计学意义(P<0.05).结论SPS结合MET在创伤后关节僵硬患者中应用效果较好,能够减轻关节疼痛,提高关节活动度,改善关节及上肢功能. Objective To explore the application effect of static progressive stretching technique(SPS)combined with muscle energy technique(MET)in patients with post traumatic elbow stiffness.Methods 70 patients with post-traumatic elbow stiffness admitted to Wuxi Eighth People's Hospital from March 2018 to September 2022 were selected as the research objects.According to the random number table method,they were divided into a control group and an observation group,with 35 cases in each group.The control group was treated with MET on the basis of routine rehabilitation treatment,and the observation group was treated with SPS on the basis of the control group,and the treatment lasted for 4 weeks.The joint pain degree,joint range of motion,joint function and upper limb function were compared between the two groups before and after training.Results Before training,there was no significant difference in joint pain degree,joint range of motion,joint function and upper limb function between the two groups(P>0.05).After 4 weeks of training,the pain Visual Analogue Scale score of forearm internal rotation pain in the observation group was(1.21±0.55)points,the pain Visual Analogue Scale score of forearm external rotation pain was(1.19±0.48)points,and the upper limb function rating scale score was(32.44±3.27)points,the scores were lower than(2.74±0.91)points,(2.69±1.65)points,(49.52±3.48)points of the control group,the differences between the two groups were statistically significant(P<0.05);the active range of motion of the elbow joint in the observation group was greater than that in the control group,and the Mayo elbow function score was higher than that in the control group,and the differences between the two groups were statistically significant(P<0.05).Conclusion The application of SPS combined with MET in patients with post traumatic joint stiffness has a good effect,reducing joint pain,improving joint range of motion,and improving joint and upper limb function.
