摘要
目的探讨球囊压迫术(PBC)与显微血管减压术(MVD)治疗三叉神经痛(TN)患者的效果。方法选择2021年12月—2023年1月就诊于该院的60例TN患者为研究对象,按随机数字表法将其分为MVD组与PBC组,各30例。PBC组在全麻下行PBC治疗,MVD组在全麻下行MVD治疗。对比两组疼痛缓解情况、住院时间、手术时间、氧化应激反应、并发症发生情况。结果两组疼痛缓解总有效率对比,差异无统计学意义(P>0.05)。MVD组手术时间、住院时间均长于PBC组,组间差异有统计学意义(P<0.05)。治疗后,两组谷胱甘肽过氧化物酶(GSH-Px)、超氧化物歧化酶(SOD)水平低于治疗前,丙二醛(MDA)水平均高于治疗前,MVD组GSH-Px、SOD水平均较PBC组低,MDA水平较PBC组高,差异有统计学意义(P<0.05)。MVD组并发症总发生率13.33%较PBC组的36.67%低,差异有统计学意义(P<0.05)。结论MVD与PBC均可有效缓解TN患者疼痛,MVD并发症发生率低,PBC应激刺激低且手术时间短、恢复快、费用低,临床可根据患者实际情况选择术式。
Objective To investigate the efficacy of Percutaneous balloon compression(PBC)and microvascular decompression(MVD)in the treatment of trigeminal neuralgia(TN)patients.Methods 60 TN patients who visited the hospital from December 2021 to January 2023 were selected as the study objects and randomly divided into a MVD group and a PBC group,with 30 patients in each group.The PBC group received PBC treatment under general anesthesia,while the MVD group received MVD treatment under general anesthesia.Compare the pain relief,hospital stay,surgical time,oxidative stress response,and complications between the two groups.Results There was no statistically significant difference in the total effective rate of pain relief between the two groups(P>0.05).The surgical time and hospital stay in the MVD group were longer than those in the PBC group,and the differences between the groups were statistically significant(P<0.05).After treatment,the level of glutathione peroxidase(GSH-Px)and superoxide dismutase(SOD)in both groups were lower than before treatment,and the level of malondialdehyde(MDA)were higher than before treatment,while the level of GSH-Px and SOD in the MVD group were lower than those in the PBC group,and the MDA level was higher than that in the PBC group,and the differences were statistically significant(P<0.05).The total incidence of complications in the MVD group 13.33%was lower than 36.67%in the PBC group,and the difference was statistically significant(P<0.05).Conclusion Both MVD and PBC can effectively alleviate pain in TN patients.The former has a low incidence of complications,while the latter has low surgical stress stimulation,short surgical time,fast recovery,and low cost.The clinical selection of surgical methods can be based on the actual situation of the patient.
