摘要
目的分析闭合复位克氏针内固定在肱骨髁上骨折患儿中的应用效果。方法选取该院2020年5月-2023年5月收治的74例肱骨髁上骨折患儿为研究对象,按随机数字表法将其分为对照组与观察组,各37例。对照组采用切开复位克氏针内固定,观察组采用闭合复位克氏针内固定,两组均术后随访3个月。对比两组患者的围术期指标、肘关节活动度、并发症发生情况。结果两组骨折愈合时间、并发症发生率相比,组间差异无统计学意义(P>0.05)。观察组术中出血量为(18.71±2.46)mL,少于对照组的(33.59±4.36)mL,手术时间为(75.31±4.70)min、住院时间为(5.18±0.63)d,均短于对照组的(93.46±6.44)min、(8.41±1.59)d,组间差异有统计学意义(P<0.05)。术后,两组肘关节屈伸、旋前、旋后活动度均大于术前,且观察组大于对照组,差异有统计学意义(P<0.05)。结论闭合复位克氏针内固定具有创伤小、术后恢复快优势,可改善肱骨髁上骨折患儿肘关节活动度,且并发症较少。
Objective To analyze the effect of closed reduction and Kirschner wire internal fixation in children with supracondylar fractures of the humerus.Methods 74 children with supracondylar fracture of humerus admitted to our hospital from May 2020 to May 2023 were selected as the study objects,and were divided into a control group and an observation group according to random number table method,with 37 cases in each group.The control group was treated with open reduction Kirschner wire internal fixation,and the observation group was treated with closed reduction Kirschner wire internal fixation.Both groups were followed up for 3 months.The perioperative indexes,elbow motion and complications were compared between the two groups.Results There were no significant differences in fracture healing time and complication rate between the two groups(P>0.05).The intraoperative blood loss in the observation group was(18.71±2.46)mL,less than(33.59±4.36)mL in the control group,the operation time was(75.31±4.70)min,and the hospital stay was(5.18±0.63)d,both shorter than(93.46±6.44)min and(8.41±1.59)d in the control group,the differences between groups were statistically significant(P<0.05).After operation,elbow flexion and extension,pronation and supination motion in both groups were higher than before operation,and observation group was higher than control group,the difference was statistically significant(P<0.05).Conclusion Closed reduction and Kirschner wire internal fixation have the advantages of minimal trauma and fast postoperative recovery,which can improve elbow joint mobility in children with supracondylar humeral fractures and have fewer complications.
