摘要
目的探讨化疗期肺癌患者自我感受负担与疾病感知、乐观倾向及创伤后成长的相关性。方法采用疾病感知问卷简化版、生活定向测验修订版、创伤后成长评定量表及癌症患者自我感受负担量表,对109例化疗期肺癌患者进行调查。结果67%化疗期肺癌患者自我感受负担为中重度,自我感受负担总分及各维度得分与创伤后成长总分及各维度得分、乐观倾向得分呈负相关,与疾病感知得分呈正相关(P<0.01);进一步回归分析显示,创伤后成长总分、乐观倾向及疾病感知分别可以共同解释经济/家庭负担、心理/情感负担及总体负担的48.1%、49.2%及63.1%的变异;乐观倾向及疾病感知可以共同解释照护负担51.5%的变异,与个人力量维度一起解释治疗负担56.4%的变异。结论化疗期肺癌患者自我感受负担与其疾病感知、乐观倾向及创伤后成长密切相关,临床医护人员应高度重视并提供相应的干预措施。
Objective To investigate the correlations between self-perceived burden and disease perception,optimism or post-traumatic growth in patients with lung cancer during chemotherapy.Methods A total of 109 patients with lung cancer during chemotherapy were investigated by using Brief Illness Perception Questionnaire(BIPQ),Revised Life Orientation Test(LOT-R),Posttraumatic Growth Inventory(PTGI)and Self-Perceived Burden Scale for Cancer Patients(SPBS-CP).Results Among the patients,67%suffered moderate or severe self-perceived burden.The total and each dimension scores of SPBS-CP were negatively correlated with the total and each dimension scores of PTGI,and the score of LOT-R;and positively correlated with the score of BIPQ(P<0.01).Further regression analysis showed that the total score of PTGI,optimism and disease perception could explain 48.1%,49.2%and 63.1%variation of economic/family burden,psychological/emotional burden and total burden,respectively;optimism and disease perception could explain 51.5%variation of care burden,and 56.4%variation of treatment burden together with individual strength dimension.Conclusion It is closely correlated between the self-perceived burden and post-traumatic growth,disease perception and optimistic tendency of lung cancer patients during chemotherapy.Clinicians and nurses should attach great importance to it and provide corresponding interventions.
