摘要
目的:研究3D打印个体化导板在胸椎和颈椎手术中置钉应用的准确性和安全性。方法:对19例需要行胸椎、颈椎椎弓根螺钉植入手术的患者,术前接受薄层CT扫描,将数据输入软件后,建立三维仿真模型,制作钉道导板,用激光快速成型机打印导板,术中借助导板辅助椎弓根螺钉置钉。术后CT扫描评价螺钉位置,记录置钉的准确性及有无相关并发症。结果:通过导板植入的64枚椎弓根螺钉,62枚完全在椎弓根内,2枚穿破椎弓根外壁,椎弓根穿破率为3.2%,植钉准确率为96.8%。无一例出现螺钉植入有关的神经、血管损伤等并发症。结论:3D打印个体化导板可以辅助胸椎、颈椎椎弓根螺钉的植入,同时置钉准确性高、安全,能明显缩短手术时间。
Objective:To study the accuracy and safety of 3 D printed individualized guide plate in the application of screw placement in thoracic and cervical surgery.Methods:19 patients undergoing thoracic and cervical pedicle screw implantation receivedpreoperative thin-section CT scan before surgery.After data were input into Mimic and Geomagic Studio 2013,a 3 D simulation model was established to design the best internal fixation screw channel and make the screw channel guide plate.The guide plate was printed by laser rapid prototyping machine,which was used to assist screw placement for internal fixation during surgery.The screw position was evaluated by CT scan after surgery,and the accuracy of screw placement and related complications were recorded.Results:Of the 64 pedicle screws implanted through the guide plate,62 were completely in the pedicle,and 2 penetrated the outer wall of the pedicle with a penetrationrate of 3.2%,and the accuracy of screw placement was 96.8%.Nonerve and vascular injury related to screw implantation was found.Conclusion:The individualized guide plate designed and printed by 3 d printing technology can assist in the implantation of thoracic and cervical pedicle screws with high accuracy and safety,and it can significantly shorten the operation time.
