摘要
目的评价3种治疗晚期胃癌化疗方案的成本-效果,为临床提供参考。方法采用回顾分析方法,把151例患者分为3组,A组51例患者采用XELOX治疗方案(奥沙利铂+卡培他滨)、B组49例患者采用XP方案(顺铂+卡培他滨)、C组51例患者采用TX方案(紫杉醇+卡培他滨)治疗。治疗以21天为1个疗程,2个疗程后根据成本与疗效进行药物经济学分析。结果3组化疗方案的有效率分别为A组43.1%、B组38.8%、C组47.1%;3组的不良反应发生率分别为A组202.0%、B组226.5%、C组319.6%;3组的C/E值分别为A组109.4、B组106.3、C组112.8,B组最小,敏感度分析结果差异无统计学意义。结论从成本效果分析B组的化疗方案最佳,实施最有意义,但3种方案各有特点,临床应用时根据实际情况,实施具体的给药方法。
Objective To evaluate the cost-effectiveness of three chemotherapy regimens for the treatment of advanced gastric cancer,and to provide clinical references.Methods By retrospective analysis method,151 patients were divided into 3 groups.51 patients in group A were treated with XELOX treatment plan(oxaliplatin+capecitabine),and 49 patients in group B were treated with XP plan(cisplatin+capecitabine).51 patients in group C were treated with TX regimen(paclitaxel+capecitabine).The treatment took 21 days as a course of treatment.After 2 courses,pharmacoeconomic analysis was carried out according to the cost and efficacy.Results The effective rates of the three groups of chemotherapy regimens were 43.1%in group A,38.8%in group B,and 47.1%in group C.The adverse reaction rates of the three groups were 202.0%in group A,226.5%in group B,and 319.6%in group C.The C/E values of group A were 109.4 in group A,106.3 in group B,and 112.8%in group C.Group B was the smallest.There was no statistically significant difference in sensitivity analysis results.Conclusion According to the cost-effectiveness analysis,the chemotherapy regimen of group B was the best and the implementation was the most meaningful.However,the three regimens had their own characteristics.In clinical application,specific methods of medication should be implemented according to actual conditions.
