摘要
目的:探讨创伤性或术后膝关节僵硬与非僵硬患者肌肉功能和就诊时间的差异。方法:选取31名膝关节创伤后或术后患者。根据其僵硬情况分为僵硬组和非僵硬组。记录一般资料和关节活动度,进行双侧等速肌力评估。结果:①创伤性膝关节损伤患者患侧肌力和耐力较健侧降低,且差异有统计学意义(P<0.05);②僵硬患者较非僵硬患者首诊时间更早,且差异有统计学意义(P<0.05);③僵硬患者健侧耐力较非僵硬患者耐力更好,且差异有统计学意义(P<0.05),健侧伸肌群耐力与膝关节僵硬的程度呈负相关,且有统计学意义(P<0.05)。结论:创伤性膝关节炎(包括非僵硬)患者的肌肉功能训练是一个非常重要的康复内容。越早开始功能锻炼,康复效果越好,即使不存在日常生活能力受限,如非僵硬患者,也要进行专业的评估、康复指导及训练。神经肌肉系统再教育及患侧负重也是一个康复重点。
Objective:To investigate the difference of muscle function and duration of trauma between patients with posttraumatic and/or postsurgical knee stiffness and without stiffness.Method:According to the limitation of range of motion,thirty-one patients with posttraumatic knee were enrolled and divided into two groups,including 13 patients with stiffness and 18 without.The general information,range of motion and isokinetic muscle force test were recorded.Result:The strength and endurance of injured knee were significantly lower than the other side(P<0.05).The duration of trauma were shorter and endurance of unaffected side were significantly higher in patients with stiffness(P<0.05).There was a significant negative correlation between the endurance of unaffected extensors and the anchylosis extent of knee(P<0.05).Conclusions:Muscle strength and endurance training were important in posttraumatic or postsurgical knee rehabilitation procedure.Rehabilitation procedure should be started as soon as possible after trauma,even a patient without limitation of daily activities like those without stiffness.Central nervous system reeducation training and weight bearing should be included in rehabilitation procedure.
