RNA双扩增检测技术在手足口病肠道病毒核酸检测中的应用 被引量:2

Application of RNA Dual Amplification Technology in the Detection of Enterovirus Nucleic Acid of Hand-Foot-Mouth Disease

摘要 目的研究新型RNA双扩增检测技术(dual amplification technology,DAT)在手足口病(hand-foot-mouth disease,HFMD)肠道病毒感染中的诊断价值。方法选取HFMD临床确诊病例、疑似病例共202例,应用DAT及聚合酶链反应(polymerase chain reaction,PCR)-荧光探针法对患儿的咽拭子标本进行肠道病毒的核酸检测,对于两种方法检测结果不一致的样本,进行基因测序验证。结果与PCR-荧光探针法相比,DAT阳性检出率更高,阳性符合率、阴性符合率、总符合率都在85%以上,一致性Kappa值0.75以上;20例不一致的样本,DAT和基因测序的检测结果一致性高于PCR-荧光探针法(P<0.05)。结论DAT可同时完成3种肠道病毒RNA检测,灵敏度高,特异性强,为临床HFMD的预防、控制、诊断和治疗提供了合理化建议和理论参考依据。 Objective To investigate the diagnostic value of RNA dual amplification technology(DAT)in the enterovirus infection of hand-foot-mouth disease(HFMD).Methods A total of 202 patients with clinically confirmed HFMD or suspected HFMD were enrolled,the nasopharyngeal swab samples of patients were detected by DAT and polymerase chain reaction(PCR)-fluorescence probing assay.The samples with different result in both assays were further confirmed by gene sequencing assay.Results Compared with PCR-fluorescence probing assay,the positive rate of DAT were higher.The positive coincidence rate,negative coincidence rate and total coincidence rate were all above 85%,and the consistent Kappa value was above 0.75.In 20 incongruent samples,the results of DAT and gene sequencing assay were more consistent than those of PCR-fluorescent probing assay.Conclusion DAT can test the RNA of three enterovirus pathogens and with high sensitivity and specificity,which can provide reasonable evidence and theoretical basis for the prevention,control,diagnosis and treatment of HFMD.
