摘要
目的:分析强斜低位空心钉联合缝匠肌骨瓣治疗老年股骨颈骨折的治疗效果。方法:2017年1月~2019年1月,选择90例在我院接受治疗的老年股骨颈骨折患者随机分为对照组和观察组,各45例。对照组给予强斜低位空心钉内固定术,观察组给予强斜低位空心钉联合缝匠肌骨瓣术。术后,观察两组患者在治疗效果、手术时间、住院时间、术中出血量及生活质量。结果:观察组的总有效率为93.3%,明显高于对照组的77.8%,差异有统计学意义。患者为一典型病例,女性,66岁,在卫生间滑倒摔伤,术前1h,X射线图片显示,患者左股骨颈出现骨折,无法正常行走;术后1d,对患者行强斜低位空心钉联合缝匠肌骨瓣治疗方法,X线片显示,空心钉将患者骨折处固定完好,但无法正常行走;术后6个月,X线片显示,患者骨折处基本痊愈,且行走正常。观察组的住院及手术时间与对照组相比明显缩短,且术中出血量与对照组相比显著减少,差异变化明显。观察组的生活质量(物质功能、躯体功能、社会功能、心理功能)评分与对照组相比明显升高,差异变化显著。观察组并发症发生率为2.2%,明显低于对照组的13.2%,差异具有统计学意义。结论:强斜低位空心钉联合缝匠肌骨瓣的治疗效果明显,在临床中值得推广应用。
Objective To study efficacy in patients with transcervical fracture by hollow screw combined with sartorial-pedicled ilium bone transplantation.Methods 90 cases of patients were divided into the observation group(45 cases)and the control group(45 cases)from January 2017 to January 2018.The control group was given hollow screw while the observation group was received hollow screw combined with sartorial-pedicled ilium bone transplantation.Treatment effect,surgery and hospital stay,intraoperative blood loss and quality of life were compared.Results The total efficiency(93.3%)of the observation group was significantly rather than the control group(77.8%),the difference was statistically significant.Fractures were appeared in left femoral neck and could not walk before 1 hour surgery.Fractures were fixed perfectly and could not walk after 1 day surgery.Fractures were got well and could walk well after 6 month.Surgery and hospital stay of the observation group were statistically shorter than the control group while intraoperative blood loss were statistically less than the control group,the difference was statistically significant.Physical,psychological,social and physical functions of the observation group were statistically rather than the control group,the difference was statistically significant.Complication rates of the observation group(2.2%)were statistically lower than the control group(13.2%),the difference was statistically significant.Conclusion The treatment effect of the hollow screw combined with sartorial-pedicled ilium bone transplantation was significantly,which could significantly shorten the time of surgery and hospitalization,reduced the amount of intraoperative blood loss,improved the quality of life,safe and reliable,and was worthy of popularization in clinical practice..
