摘要
目的观察中医牵引舒筋手法与Pavlik吊带联合治疗发育性髋关节发育不良(DDH)患儿临床疗效。方法将我院120例DDH患儿随机分为对照组与观察组各60例。对照组给予Pavlik吊带治疗,观察组给予中医牵引舒筋手法联合Pavlik吊带治疗。比较两组患儿治疗前后中医证候积分、影像学指标[中心边缘角(CE)、髋臼指数(AI)、髋臼Sharp角]、髋关节稳定性(Ortolani征、Barlow征);随访12个月后,比较两组患儿髋关节功能恢复情况[髋关节功能(McKay评级)、Severin X射线评分],记录治疗及随访期间并发症发生情况。结果治疗后,两组患儿中医证候积分、AI、髋臼Sharp角均较治疗前减小(P<0.05),CE较治疗前增大(P<0.05),且观察组上述指标与对照组比较差异有统计学意义(P<0.05);Ortolani征、Barlow征均转阴(P<0.05);观察组Mc Kay评级及Severin X射线评级优良率高于对照组(P<0.05);两组并发症发生率无显著性差异(P>0.05)。结论中医牵引舒筋手法联合Pavlik吊带能够改善DDH患儿症状及影像学指标,提高髋关节稳定性,改善髋关节功能,临床效果显著。
Objective To observe the clinical effects of TCM traction and tendon relaxation manipulation combined with Pavlik harness in treatment of developmental dysplasia of the hip(DDH).Methods A total of 120 children with DDH were randomly divided into control group and observation group with 60 cases in each group.Control group was given Pavlik harness,and observation group was given TCM traction and tendon relaxation manipulation combined with Pavlik harness treatment.The TCM syndromes scores,imaging indicators[central-edge angle(CE),acetabular index(AI),acetabular Sharp angle]and hip stability(Ortolani sign,Barlow sign)before and after treatment and hip function recovery(McKay classification),Severin X-ray score]after 12 months of follow-up were compared between the two groups.The occurrence of complications during treatment and follow-up were recorded.Results After treatment,the TCM syndromes scores,AI and acetabular Sharp angle were reduced in both groups compared with those before treatment(all P<0.05)while the CE was increased compared with that before treatment(P<0.05),and the above indicators in observation group were statistically different from those in control group(P<0.05);Ortolani sign and Barlow sign turned negative(all P<0.05);the good and excellent rates of McKay classification and Severin X-ray classification in observation group were higher compared with those in control group(all P<0.05),but there were no significant differences in the incidence rates of complications(P>0.05).Conclusion TCM traction and tendon relaxation manipulation combined with Pavlik harness can improve the symptoms and imaging indicators,enhance the hip stability,and promote the hip function of children with DDH,it has significant clinical effects.
