摘要
目的比较基于减影的径向采集逐点编码时间缩减的超短TE静音磁共振血管成像(Petra qMRA)与三维飞行时间磁共振血管成像(3D-TOF MRA)对颈内动脉C2~C7段的成像质量。方法前瞻性搜集81例怀疑脑血管疾病的患者行Petra qMRA与3D-TOF MRA检查,对颈内动脉C2~C7段血管结构采用评分法进行评估。采用Wliconxon秩和检验比较Petra qMRA与3D-TOF MRA在颈内动脉C2~C7段血管结构的评分差异。采用一致性Kappa分析评估观察者间的一致性。结果两名观察者对81例患者的Petra qMRA与3D-TOF MRA颈内动脉C2~C7段图像评估具有良好的一致性(Kappa=0.774)。Petra qMRA与3D-TOF MRA颈内动脉C2~C7段图像优良率差异具有统计学差异(χ^2=12.14,P=0.000)。Petra qMRA与3D-TOF MRA显示颈内动脉C2~C7段血管评分分别是(3.93±0.589)分、(3.49±0.572)分,差异具有统计学差异(Z=-4.698,P=0.000)。Petra qMRA与3D-TOF-MRA对颅内动脉狭窄或闭塞者与脑动脉瘤术后者,两种技术图像优良率之间,有统计学差异(χ^2=4.121,P=0.042和χ^2=6.667,P=0.010)。结论Petra qMRA的颈内动脉C2~C7段图像质量高于3D-TOF MRA;对于颅内动脉狭窄或闭塞者以及脑动脉瘤术后者其成像优于3D-TOF MRA。
Objective To compare the imaging quality of ultra-short TE silent magnetic resonance angiography(Petra qMRA)and three-dimensional time-of-flight magnetic resonance angiography(3 D-TOF MRA)for C2-C7 segments of internal carotid artery based on subtraction radial acquisition and point-by-point coding time reduction.Methods 81 patients suspected of cerebrovascular diseases were prospectively collected and examined with Petra qMRA and 3 D-TOF MRA.The vascular structure of C2-C7 segment of internal carotid artery was evaluated by scoring method.Wliconxon rank sum test was used to compare the difference in scoring between Petra qMRA and 3 D-TOF MRA in C2-C7 segment of internal carotid artery.Consistency Kappa analysis was used to evaluate the consistency between observers.Results Two observers have good consistency in the evaluation of Petra qMRA and 3 D-TOF MRA images of C2-C7 segments of internal carotid artery in 81 patients(Kappa=0.774).There was a statistical difference between Petra qMRA and 3 D-TOF MRA in the excellent and good image rate of C2-C7 segment of internal carotid artery(χ^2=12.14,P=0.000).Petra qMRA and 3 D-TOF MRA showed that the vascular scores of C2-C7 segment of internal carotid artery were(3.930589)and(3.490.572)respectively,with statistical difference(Z=-4.698,P=0.000).Petra qMRA and 3 DTOF-MRA have statistical differences in the excellent and good image rates of intracranial artery stenosis or occlusion and cerebral aneurysm surgery(χ^2=4.121,P=0.042 andχ^2=6.667,P=0.010).Conclusion Petra qMRA is superior to 3 D-TOF MRA in image quality of C2-C7 segment of internal carotid artery.Petra qMRA is superior to TOF MRA in imaging of patients with intracranial artery stenosis or occlusion and patients after cerebral aneurysm surgery.
