摘要
目的通过分析表观扩散系数(ADC)值在非小细胞肺癌(NSCLC)不同的病理类型、分化程度及T、N分期的差异,探讨磁共振扩散加权成像(DWI)对NSCLC的诊断价值。方法40例经病理证实为NSCLC的患者接受3.0 T磁共振常规序列联合DWI检查,测量病变的ADC值。用单因素方差分析比较不同病理类型、分化程度、T、N分期的ADC值的差异,并计算受试者工作特征曲线(ROC)曲线下面积(AUC)评价诊断效能;用两变量间相关分析计算Spearson相关系数,评价ADC值与上述变量的相关性。结果不同病理类型及分化程度的NSCLC ADC值之间存在统计学差异(P<0.05),高分化腺癌ADC值(1.115±0.246)×10-3mm2/s·S-1,高于鳞癌和低分化腺癌且差异有统计学意义(P<0.05),鳞癌与低分化腺癌ADC值差异无统计学意义(P>0.05);且ADC值与分化程度存在相关性(r=-0.451,P=0.014),AUC为0.774±0.096(P<0.05)。NSCLC不同T分期患者的ADC值差异具有统计学意义(P<0.05),且ADC值与T分期存在相关性(r=-0.358,P=0.027),AUC为0.897±0.055(P<0.05)。非小细胞肺癌不同N分期患者的ADC值差异具有统计学意义(P<0.05),且ADC值与N分期存在相关性(r=-0.528,P=0.002);AUC为0.723±0.092(P<0.05)。结论DWI对NSCLC的病理类型、分化程度及分期诊断具有一定的价值。
Objective To investigate the value of diffusion-weighted imaging(DWI)in the diagnosis of non-small cell lung cancer(NSCLC)by analyzing the value of apparent diffusion coefficient(ADC)in different pathological types,differentiation degree and T,N stages of NSCLC.Methods 40 patients with NSCLC confirmed by pathology received 3.0 T MR conventional sequence combined with DWI to measure the ADCs value of the lesions.The ADC values of different pathological types,differentiation degree and stages were compared by single factor analysis ANOVA.Evaluation of diagnostic efficiency was performed by area under ROC curve.Spearson correlation coefficient was used to analyze the correlations of the ADC with above indicators.Results The ADC values of NSCLC with different pathological types and differentiation degrees were statistically different(P<0.05).The ADC values of highly differentiated adenocarcinoma(1.115±0.246)×10-3mm2/s·S-1 were significantly higher than those of squamous carcinoma and poorly differentiated adenocarcinoma(P<0.05).The ADC values of squamous carcinoma and poorly differentiated adenocarcinoma were not statistically different(P>0.05),and there was a correlation between ADC values and differentiation degrees(r=-0.451,P=0.014),the area under ROC curve was 0.774±0.096(P<0.05).There was significant difference in ADC value between different T stages of NSCLC(P<0.05),and there was correlation between ADC value and T stage(r=-0.358,P=0.027).The area under ROC curve was 0.897±0.055(P<0.05).The ADC value of NSCLC patients with different N stages was statistically significant(P<0.05),and there was a correlation between ADC value and N stages(r=-0.528,P=0.002).The area under ROC curve was 0.723±0.092(P<0.05).Conclusion Diffusion weighted imaging(DWI)has certain value in the pathological type,differentiation degree and staging diagnosis of NSCLC.
