摘要
目的运用属性层次分析模型赋权(AHM)-加权逼近理想解排序(TOPSIS)法对临床药师专项处方点评工作实施前后注射用丹参多酚酸盐的用药合理性进行分析和评价,为临床合理用药提供参考。方法利用医院PASS系统采取等间距随机抽样法,抽取2021年4至6月使用注射用丹参多酚酸盐的病历297份作为对照组,2021年7月临床药师专项处方点评工作开始实施,抽取2021年10至12月使用注射用丹参多酚酸盐的病历297份作为观察组。通过AHM-TOPSIS法对所有病历进行用药合理性评价,对专项处方点评工作实施前后注射用丹参多酚酸盐的用药合理性进行分析。结果观察组注射用丹参多酚酸盐用药合理例数占比高于对照组(P<0.05)。各评价指标中,除溶媒种类在实施前后均合理外,在适应证、禁忌证、用法用量、给药浓度、用药疗程、联合用药及医保限制方面,观察组用药合理性均高于对照组(P<0.05)。结论注射用丹参多酚酸盐的用药合理性在临床药师专项处方点评工作实施后明显提高,该院将继续细化临床药师专项处方点评工作,进一步促进合理用药。基于AHM-TOPSIS法使对注射用丹参多酚酸盐的用药合理性进行评价,结果更直观、可靠,值得推广。
AIM To analyze and evaluate the utilization of salvianolate for injection before and after the implementation of clinical pharmacist's specialized prescription review,using AHM(attribute hierarchical analysis)and TOPSIS(weighted approximation of ideal solutions),aiming to provide a reference for clinical rational drug use.METHODS Using the PASS system of hospital and employing a uniform random sampling method to select 297 medical records using salvianolate for injection from April to June 2021 as the control group.The clinical pharmacist's specialized prescription review program was initiated in July 2021,and 297 medical records from October to December 2021 were selected as the observation group.Through the AHM-TOPSIS method,the rationality of medication was assessed for all medical records,and a comparative analysis was conducted on the rationality of salvianolate for injection use before and after the implementation of the specialized prescription review program.RESULTS In the observation group,the overall rate of appropriate use of salvianolate for injection was significantly higher than that in the control group(P<0.05).For each evaluation index,except for the types of vehicles,which remained reasonable before and after the intervention,the observation group exhibited higher rates of appropriateness compared to the control group in terms of indications,contraindications,usage and dosage,administration concentration,duration of medication,combined medication,and medical insurance restrictions(P<0.05).CONCLUSION The AHM and TOPSIS methods utilize multiple indicators to evaluate the rationality of salvianolate for injection,providing more intuitive and reliable results.The rational use of salvianolate for injection is significantly improved after intervention.The hospital will continue to refine clinical pharmacist's specialized prescription review measures to further enhance the rational use of drugs.
