碎裂QRS波与HCM室性心律失常及猝死的关系 被引量:8

摘要 目的探讨碎裂QRS波与肥厚型心肌病患者发生室性心律失常及心源性猝死的关系。方法选取来我院就诊的肥厚型心肌病患者70例,随访1年。20例出现室性心律失常或心源性猝死入选试验组,50例未出现室性心律失常或心源性猝死入选对照组。所有入选患者均行心电图检查,统计分析两组患者碎裂QRS波特点。结果下壁导联碎裂QRS波发生率高于其它区域。试验组前壁导联碎裂QRS波发生率大于对照组(35%vs.10%,p=0.012);三个或以上区域碎裂QRS波发生率大于对照组(30%vs.4%,p=0.008)。三个或以上区域碎裂QRS波对肥厚型心肌病室性心律失常及心源性猝死的敏感性30%,特异性96%。结论三个或以上区域碎裂QRS波与肥厚型心肌病室性心律失常及心源性猝死有关,在一定程度上可以预警肥厚型心肌病高危患者。 Objective To investigate the association between QRS fragmentation(f QRS)and ventricular arrhythmia or sudden cardiac death in patients with hypertrophic cardiomyopathy.Methods A total of 70 patients with hypertrophic cardiomyopathy(HCM)were studied.The patients were followed up for 1 year.Twenty patients with ventricular arythmia(ventricular tachycardia/Ventricular flutter)or sudden cardiac death were case group.Fifty patients without arythmia or sudden cardiac death were control group.Patients in the two groups were examined by12-lead electrocardiogram,and f QRS was analyzed between the two groups.Results The prevalence of f QRS in inferior leads was the highest.The prevalence of f QRS in anterior leads in the experimental group was higher than that in the control group(35%vs.10%,p=0.012).The prevalence of f QRS in≥3 territories in the experimental group was higher than the control group(30%vs.4%,p=0.008).Sensitivity,Specificity of f QRS for prediction of malignant arrhythmia or sudden cardiac death in patients with HCM were 30%,96%.Conclusions FQRS in≥3 territories are associated with hypertrophic cardiomyopathy arrhythmia or sudden cardiac death,which could identify high-risk patients of HCM.
