摘要
目的 探讨低钾性周期性麻痹患者的心电图表现,提升医师对该类患者的认识。方法 收集北海市人民医院住院低钾性周期性麻痹患者83例,分析其心电图表现。结果 患者平均年龄37.39±14.00岁,出现四肢肌力下降66例,单纯出现下肢肌力下降17例。患者平均体温36.68±0.42℃;平均脉搏:87.77±16.64次/分;平均呼吸:19.98±0.96次/分;平均血钾:2.23±0.47mmol/L;血钠:139.73±3.19mmol/L;HCO_(3)^(-):24.04±4.32mmol/L,PH:7.40±0.05。低钾性周期性麻痹患者83例,其发作时所做的心电图中74例(89.2%)心电图异常,随着血钾降低,患者心电图异常比率上升。低钾性周期性麻痹患者的血钾浓度与心电图异常发生率呈负相关。结论 低钾性周期性麻痹心电图异常比例高,且血钾越低,心电图异常比率越高,心电图检查有利于低钾性周期性麻痹的诊断。
Objective To study the electrocardiogram(ECG) of patients with periodic paralysis caused by hypokalemia and to improve the understanding of physicians. Methods A total of 83 patients with hypokalemic periodic paralysis in Beihai People’s Hospital were collected and analyzed. Results The mean age of the patients was37.39±14.00 years old. Sixty-six cases showed decreased muscle strength in the extremities, and 17 cases only showed decreased muscle strength in the lower extremities. The mean body temperature of the patients was 36.68±0.42 ℃. The average heart rate was 87.77 ±16.64 bpm. The respiration rate was 19.98 ±0.96 per minute. The average serum potassium concentration was 2.23±0.47mmol/L, and the serum sodium concentration was 139.73±3.19mmol/L, and the level of HCO_(3)^(-)was 24.04±4.32mmol/L, and the p H value was 7.40±0.05. There were 74(89.2%) cases had abnormal ECG manifestation out of the 83 patients with periodic paralysis of hypokalemia. With the decrease of serum potassium, the rate of abnormal ECG increased. There was a negative correlation between serum potassium and rate of abnormal ECG changes in hypokalemic periodic paralysis. Conclusions The proportion of abnormal ECG in hypokalemic periodic paralysis is high, and the lower the serum potassium is, the higher the abnormal rate of electrocardiogram is. The ECG examination is helpful for the diagnosis of hypokalemic periodic paralysis.
