摘要
宫腔粘连是由各种原因造成子宫内膜损伤继发宫腔和(或)宫颈管部分或全部闭塞的一种临床常见获得性疾病,其发病率日益增高,相关并发症严重影响着当代女性的生殖健康。目前,宫腔镜下宫腔粘连分离术是宫腔粘连的首选治疗方式,亦属金刃器械创伤胞宫,损伤胞宫脉络,影响冲任为病,且术后复粘概率较高,往往是越分越粘。学生师从导师姜丽娟教授,老师临证中主张从子宫内膜血流变化为切入点,基于"子宫络脉理论",以"和畅子宫络脉法"为指导法则,并结合彩色多普勒超声辅助技术,找准宫腔粘连分离术后子宫内膜损伤症结,运用中医药联合适宜电生理技术施治,以期达到中医精准治疗,从而更好促进子宫内膜损伤后再修复,防治TCRA后再粘连。该文旨在总结学生跟师过程中对导师临证思路的体会,探讨通过"和畅子宫络脉法"防治宫腔粘连分离术(transcervical resection of uterine adhesions,TCRA)后宫腔再粘连相关作用机制、诊疗方案及临床应用前景,为TCRA后宫腔再粘连的防治探索新方案。
Intrauterine adhesion is a clinically common acquired disease secondary to endometrial injury and/or cervical canal partial or total occlusion,which is caused by various reasons.Its incidence is increasing,and the short-term or long-term complications seriously affect the reproductive health of contemporary women.At present,hysteroscopy for intrauterine adhesion separation is the preferred treatment for intrauterine adhesion disease,which is also a disease caused by trauma of the golden blade instrument,damage of the uterine vein,and influence of Chong and Ren meridians.Moreover,the probability of postoperative adhesion is relatively high,which is usually more adhesion.The author learned from tutor professor JIANGLijuan and endometrium was showed as the breakthrough point in clinical practice.Based on the uterus collaterals theory,guided by the method of harmonizing uterine collaterals and combined with color doppler ultrasound assistive technology,the crux of postoperative endometrial injury caused by intrauterine adhesions separation can be found.TCM combined with suitable electrophysiological technique is used to achieve precisiontraditional Chinese medicine treatment,so as to better promote the endometrial injury repair,preventand controlTCRA before adhesion.This paper summarized the clinical thoughtof the author when learning from tutor JIANG Lijuan and discussed the relevant mechanism of action,diagnosis and treatment scheme and clinical application promotion of the preventionand treatment of readhesion of uterine cavity after TCRA through the method of harmonizing uterine collaterals,so as to explore a new scheme for the prevention and treatment of readhesion of uterine cavity after TCRA.
