摘要
胃轻瘫是胃癌的常见临床并发症之一,以早饱腹胀痛,恶心呕吐、体质量减轻,反复出现为主要症状,严重影响着患者的生活质量,甚至加重、促进肿瘤进展。冲脉为五脏六腑十二经脉之海,是气血运行的重要通道,对于全身气机的升降有重要的调节作用。该文以"冲之为病,逆气而里急"的中医理论入手,关注中焦脾胃,下焦肝肾之脏器与冲脉之间的关系,在此基础上提出脏腑失调、冲气上逆是胃癌并发胃轻瘫的重要病机。作者认为无论是中气虚损,肾虚不摄,还是肝郁气逆,都可导致冲气上逆,胃气并冲气上逆,而出现呕恶、饮食难下;而气机不畅,有形之邪不得消,胃脘癌肿则更甚。针对此重要病机,提出临床治疗应以急性期平冲降逆,和胃安中;缓解期调理脏腑,扶正抗癌作为主要治则,灵活运用旋覆代赭汤随证加减以治疗胃癌并发胃轻瘫,以期为恶性肿瘤的临床治疗提供新思路。
Gastroparesis is one of the common complications of gastric cancer.The main symptoms are early abdominal distension and pain,nausea and vomiting,weight loss and repeated occurrence,seriously affect the quality of life,and even aggravat and promot tumor progression.Chong is the“sea”of and twelve meridians,which is an channel for the movement of Qi and blood.It plays an important role in regulating Qi of the whole body.Based on the traditional Chinese medicine theory of“up-rushing Qi syndrome of Chong”,this paper focuse on the relationship between spleen and stomach of the middle Jiao,liver and kidney of the lower Jiao and Chong.The main pathogenesis of gastric cancer complicated with gastroparesis is the disturbance of Zang-Fu organs and up-flow of Qi.We think that whether Qi deficiency,kidney deficiency,or liver depression and Qi inversion,can lead to up-rushing Qi of Chong,stomach Qi and Chong Qi inversion,resulting in vomiting and eating difficulty.And if Qi is not unobstructed,tumour can not eliminate.In view of this pathogenesis,it issuggested in the acute stage.In the remission stage,regulating Zang-Fu and strengthening the body resistance to fight cancer.Xuanfu Daizhe Decoction(旋覆代赭汤)in the treatment of gastric cancer complicated with gastroparesis.It provide a new idea for the clinical treatment of malignant tumors.
