摘要
目的观察中医针刺联合健脾熄风活络汤对脑卒中后吞咽功能障碍患者血清P物质(SP)、5-羟色胺(5-HT)的影响,为临床治疗提供参考。方法选取医院2016年1月—2020年2月收治的脑卒中后吞咽功能障碍患者104例,根据治疗方法分为两组,每组52例。对照组给予健脾熄风活络汤治疗,观察组给予中医针刺联合健脾熄风活络汤治疗。记录两组治疗前后洼田饮水试验评分、吞咽功能评价量表(SSA)评分及SP、5-HT、去甲肾上腺素(NE)、脑源性神经营养因子(BDNF)水平变化,并统计两组不良反应情况。结果治疗前,两组中医证候积分、洼田饮水试验评分、SSA评分组间比较,差异不具有统计学意义(P>0.05)。治疗后,两组中医证候积分、洼田饮水试验评分、SSA评分较治疗前下降较治疗前下降,观察组中医证候积分、洼田饮水试验评分、SSA评分低于对照组(P<0.05)。治疗前,两组SP、5-HT、NE、BDNF水平组间比较,差异不具有统计学意义(P>0.05)。治疗后,两组SP、5-HT、NE、BDNF水平较治疗前上升,观察组SP、5-HT、NE、BDNF水平高于对照组(P<0.05)。观察组不良反应率(11.54%)与对照组(9.62%)比较,差异不具有统计学意义(P>0.05)。结论中医针刺联合健脾熄风活络汤治疗脑卒中后吞咽功能障碍可改善患者吞咽功能,减轻症状,调节SP、5-HT、NE、BDNF的表达水平,且不增加不良反应。
Objective To observe the effect of acupuncture combined with Jianpi Xifeng Huoluo Decoction(健脾熄风活络汤)on serum P substances(SP)and serotonin(5-hydroxytryptamine)in patients with dysphagia after stroke and provide reference for clinical treatment.Methods A total of 104 patients with post-stroke dysphagia in our hospital(January 2016-February 2020)were selected and divided into two groups according to the treatment method,52 cases in each group.The control group was treated with Jianpi Xifeng Huoluo Decoction,while the observation group was treated with acupuncture combined with Jianpi Xifeng Huoluo Decoction.The scores of drinking water test,swallowing function evaluation scale(SSA),SP,5-HT,norepinephrine(NE)and brain-derived neurotrophic factor(BDNF)were recorded before and after treatment.Results Before treatment,there was no significant difference of TCM symptom score,Wada drinking water test score or SSA score between the two groups(P>0.05).After treatment,the scores of traditional Chinese medicine symptom,drinking water test and SSA in two groups were lower than those before treat⁃ment.The scores of traditional Chinese medicine symptom,drinking water test and SSA in two groups were lower than those in con⁃trol group(P<0.05).Before treatment,the difference was not statistical significance of SP,5-HT,NE or BDNF between the two groups(P>0.05).After treatment,levels of SP,5-HT,NE and BDNF in the observation group were higher than those in the con⁃trol group(P<0.05).The adverse reaction rate in the observation group(11.54%)was not statistically significant compared with that of the control group(9.62%)(P>0.05).Conclusion Acupuncture combined with Jianpi Xifeng Huoluo Decoction in the treat⁃ment of dysphagia after stroke can improve the swallowing function of patients,alleviate symptoms,regulate the expression levels of SP,5-HT,NE and BDNF,and do not increase adverse reactions.
