摘要
目的探讨自拟止痒散结汤结合中医火针对神经性皮炎T细胞亚群及外周血P物质、单胺氧化酶(monoamine oxdase,MAO)的影响。方法前瞻性选取该院2017年1月—2020年1月收治的神经性皮炎患者共120例,将120例患者采用数字表法分为对照组以及研究组,每组60例。对照组使用中医火针治疗,研究组接受自拟止痒散结汤结合中医火针治疗。对比两组患者的临床疗效、中医临床症状积分、心理健康、血清T细胞亚群及外周血P物质、MAO水平。结果研究组治疗有效率为98.67%(59/60)显著高于对照组83.33%(50/60)(χ^(2)=8.106,P<0.05)。治疗前,两组患者的HAMA、HAMD指标之间差异无统计学意义(P>0.05);治疗后,两组患者的HAMA、HAMD均显著下降(P<0.05),且研究组HAMA(13.84±3.92)分、HAMD(15.66±2.93)分评分低于对照组(18.37±1.85)分、(18.95±3.85)分,(t=8.095,5.267,P<0.05)。治疗前,两组患者的皮损面积、皮损厚度、瘙痒程度水平差异无统计学意义(P>0.05);治疗后,两组患者的皮损面积、皮损厚度、瘙痒程度水平显著下降(P<0.05),且研究组患者的皮损面积(1.48±0.55)分、皮损厚度(1.01±0.05)分、瘙痒程度(1.14±0.12)分改善程度显著优于对照组(2.58±1.82)分、(2.85±0.34)分、(2.84±0.11)分(t=4.481,41.473,80.891,P<0.05)。治疗前,两组患者血清P物质及MAO水平差异无统计学意义(P>0.05);治疗后,两组患者血清P物质及MAO水平均显著下降(P<0.05),且研究组血清P物质(131.72±3.85)mg/L及MAO(8.48±1.84)U/L水平显著小于对照组(140.81±3.75)mg/L、(11.33±1.74)U/L(t=13.100,8.717,P<0.05)。治疗前,两组患者血清血清T淋巴细胞亚群水平差异无统计学意义(P>0.05);治疗后,两组患者血清CD4、CD8、CD25水平均显著上升,均显著上升(P<0.05),且研究组血清CD4(36.86±4.91)、CD8(46.81±1.95)、CD25(25.82±2.85)水平显著高于对照组(31.82±2.95)(41.82±2.22)(17.83±2.94)(t=6.851,13.081,15.119,P<0.05)。结论综上所述,自拟止痒散结汤结合中医火针治疗,能有效的改善神经性皮炎T细胞亚群,降低外周血P物质、MAO水平,临床症状改善显著,显著促进疾病的好转。受样本量限制,本研究结果有待扩大化的中心试验证实。
Objective To explore the effects of Self-made Zhiyang Sanjie Decoction(自拟止痒散结汤)combined with traditional Chinese medicine fire acupuncture on T cell subsets,substance P and monoamine oxdase(MAO)in the peripheral blood of neurodermatitis patients.Methods A total of 120 patients with neurodermatitis treated in the hospital from January 2017 to January 2020 were prospectively selected and were divided into the control group and the study group by digital table method,with 60 cases in each group.The control group was treated with traditional Chinese medicine fire acupuncture,while the study group was treated with Self-made Zhiyang Sanjie Decoction combined with traditional Chinese medicine fire acupuncture.The clinical efficacy,TCM clinical symptom scores,mental health,levels of serum T cell subsets,substance P and MAO in the peripheral blood were compared between the two groups.Results The effective rate of the study group was 98.67%(59/60),which was significantly higher than that(83.33%,50/60)of the control group(χ^(2)=8.106,P<0.05).Before treatment,there was no significant difference in Hamilton Anxiety Rating Scale(HAMA)or Self-rating Depression Scale(HAMD)between the two groups(P>0.05).After treatment,the scores of HAMA and HAMD in both groups were decreased significantly(P<0.05),and those in the study group[HAMA(13.84±3.92)points,HAMD(15.66±2.93)points]were lower than those in the control group[HAMA(18.37±1.85)points,HAMD(18.95±3.85)points](t=8.095,5.267,P<0.05).Before treatment,there was no significant difference in skin lesion area,skin lesion thickness and itching degree between the two groups(P>0.05).After treatment,the skin area,skin thickness and itching degree of patients in the two groups were decreased significantly(P<0.05),and the improvement degree of skin area(1.48±0.55),skin thickness(1.01±0.05)and itching degree(1.14±0.12)in the study group was significantly better than that in the control group[skin area(2.58±1.82),skin thickness(2.85±0.34),itching degree(2.84±0.11)](t=4.481,41.473,80.891,P<0.05).Before treatment,there was no significant difference in serum levels of substance P or MAO between the two groups(P>0.05).After treatment,the serum levels of substance P and MAO in both groups were decreased significantly(P<0.05),and the levels of substance P[(131.72±3.85)mg/L]and MAO[(8.48±1.84)U/L]in the study group were significantly lower than those in the control group[substance P(140.81±3.75)mg/L,MAO(11.33±1.74)U/L](t=13.100,8.717,P<0.05).Before treatment,there was no significant difference in serum T lymphocyte subsets between the two groups(P>0.05).After treatment,the levels of CD4+,CD8+and CD25+in the serum of the two groups were increased significantly(P<0.05),and the levels of CD4+(36.86±4.91),CD8(46.81±1.95)and CD25+(25.82±2.85)in the study group were significantly higher(t=13.100,8.717,P<0.05).Conclusion To sum up,Self-made Zhiyang Sanjie Decoction combined with traditional Chinese medicine traditional Chinese medicine fire acupuncture can effectively improve T cell subsets of neurodermatitis,reduce the levels of substance P and MAO in the peripheral blood,improve the clinical symptoms significantly and significantly promote the improvement of diseases.Due to the limitation of sample size,the results of this study need to be confirmed by the enlarged central experiment.
