摘要
目的探讨社区老年慢性阻塞性肺疾病(COPD)患者肌少症与跌倒风险的关系。方法选取潍坊市385例老年COPD患者作为研究对象,分为非肌少症组和肌少症组。测量各组患者体质量指数(BMI)、肌肉质量指数(MMI)和下肢肌肉质量指数(LLMMI);同时进行握力、功能性步行能力(TUG)、平衡能力(BBS)测试。肌少症与跌倒风险相关因素进行单因素分析,BMI、MMI、LLMMI与TUG及BBS应用直线回归相关性分析。结果385例老年COPD患者发生肌少症108例,其发生率为28.05%.肌少症组患者BMI、MMI、LLMMI、握力及BBS显著低于非肌少症组,而TUG显著高于非肌少症组,差异均有统计学意义(P<0.01)。BMI、MMI、LLMMI与TUG呈负相关,与BBS呈正相关(P<0.05)。单因素分析显示老年COPD患者肌少症跌倒风险与年龄、吸烟、接触粉尘、COPD病史及呼吸困难评分密切相关,多数存在过去1年内跌倒史、下肢肌力减退、平衡功能受损。结论社区老年COPD患者肌少症跌倒风险与年龄、吸烟史、过去1年内跌倒史、下肢肌力减退、平衡功能受损有关,应积极给予营养等干预策略,以防跌倒发生。
Objective To explore the correlation between sarcopenia and the risk of falls in elderly patients with chronic obstructive pulmonary disease(COPD)in the community.Methods A total of 385 elderly patients with COPD in Weifang City were selected as the study subjects and divided into sarcopenia group and non sarcopenia group.The body mass index(BMI),muscle mass index(MMI),and lower limb muscle mass index(LLMMI)were measured in each group;At the same time,grip strength,Timed Up and Go test(TUG),and Berg balance scale(BBS)were tested;Univariate analysis of variance was used to analyze the correlation between sarcopenia and fall risk,and linear regression was used to analyze the correlation among BMI,MMI,LLMMI,TUG and BBS.Results Among 385 elderly patients with COPD,108 had sarcopenia,with an incidence of 28.05%;The BMI,MMI,LLMMI,grip strength and balance ability in the sarcopenia group were lower than those in the non sarcopenia group,but walking ability was higher than those in the non sarcopenia group(P<0.05);BMI,MMI and LLMMI were negatively correlated with TUG,and positively correlated with BBS(P<0.05).The risk of falls in sarcopenia is closely related to age,smoking,dust exposure,history and severity of COPD,and dyspnea scores.Most patients have a history of falls within the past year,decreased lower limb muscle strength,and impaired balance function.Conclusion The risk of falls in elderly COPD patients with sarcopenia in the community is related to age,smoking history,history of falls within the past year,decreased muscle strength of the lower limbs,and impaired balance function.Active intervention should be taken to prevent falls.
