摘要
目的:应用组织多普勒成像模式下测量的心肌作功指数评价慢性肾衰继发甲状旁腺功能亢进患者左、右心室功能,进一步探讨组织多普勒Tei指数及其相关参数评价SHPT患者心室功能的应用价值。方法:选取继发甲状旁腺功能亢进患者150例为病例组,依据甲状旁腺激素水平分为A亚组、B亚组、C亚组,另选取健康志愿者50例为对照组。应用常规二维超声心动图测量LVIDs、LVIDd、RVIDs、RVIDd和LV/RVEF,并应用组织多普勒技术测量左心室和右心室Tei指数。结果:A亚组左心室Tei指数较对照组增高,差异具有统计学意义,其余参数与对照组的差异均无统计学意义;B亚组LVIDs、LVIDd、左心室Tei指数、右心室Tei指数较对照组增高,其余参数差异均无统计学意义;C亚组LVEF、RVEF、LVIDs、LVIDd、RVIDs、RVIDd、左/右心室Tei指数与对照组相比,差异均有统计学意义(均P<0.05);Tei指数与LVEF、RVEF呈负相关(r=-0.634、-0.596,P<0.01),与LVIDs、LVIDd、RVIDs、RVIDd呈正相关(r=0.655、0.683、0.591、0.616,P<0.01)。结论:与常规二维超声心动图指标相比,组织多普勒Tei指数可以更准确、更全面地反映继发性甲状旁腺功能亢进患者的整体心室功能。
Objective:To evaluatethe left and right ventricular function in patientswith chronic hyperthyroidism(SHPT)by tissue Doppler Tei index,to explore the application value of tissue Doppler Tei index and related parameters for evaluation of ventricular function in patients with SHPT.Methods:150 patients with secondary hyperparathyroidism were selected as the case group,and they were divided into subgroup A,subgroup B,and subgroup C according to the parathyroid hormone(PTH)level.Another 50 healthy volunteers were selected as Control group.We use conventional two-dimensional echocardiography to measure left ventricular end-systolic diameter(LVIDs),left ventricular end-diastolic diameter(LVIDd),right ventricular end-systolic diameter(RVIDs),right ventricular end-diastolic diameter(RVIDd),and left/right ventricular ejection fraction(EF),and tissue Doppler mod was used to measure the left and right Teiindex.Results:The left ventricular Tei index of subgroup A was higher than that of the control group,and the differences were statistically significant;The LVIDs,LVIDd,left ventricular Tei index,and right ventricular Tei index of subgroup B were higher than those of the control group,and the other parameters were not statistically significant;LVEF,RVEF,LVIDs,LVIDd,RVIDs,RVIDd,left/right ventricular Tei index of subgroup C were significantly different from the control group(all P<0.05).L/R-Tei index was negatively correlated with LVEF and RVEF(r=-0.634,-0.596,all P<0.01)and positive correlation with LVIDs,LVIDd,RVIDs,and RVIDd(r=0.655,0.683,0.591,0.616,all P<0.01).Conclusion:Compared with conventional two-dimensional echocardiographic indicators,the tissue Doppler Tei index can more accurately and comprehensively reflect the overall ventricular function in patients with secondary hyperparathyroidism.
