CD4+T淋巴细胞计数联合AIS-APS及ICH-APS对卒中相关性肺炎预测价值 被引量:5

The Predictive Value of CD4+T Lymphocyte Count Combined with AIS-APS and ICH-APS in Stroke-Associated Pneumonia

摘要 目的探讨CD4+T淋巴细胞计数联合AIS-APS及ICH-APS对卒中相关性肺炎(SAP)的预测价值。方法收集脑卒中发病24 h内入院患者57例,根据脑卒中发生7 d内是否发生肺炎分为SAP组和非SAP组。据患者入院时脑梗死及脑出血分别对其进行AIS-APS及ICH-APS评分。利用串联实验和并联实验分析CD4+T淋巴细胞计数分别联合AIS-APS及ICH-APS预测SAP发生的准确性。结果以CD4+T淋巴细胞计数<1077cell/ul和AIS-APS>17分联合预测SAP串联实验的敏感性和特异性是59.7%和88.9%,并联实验的敏感性和特异性是96.0%和43.2%;以CD4+T淋巴细胞计数<1077cell/ul和ICH-APS>11分联合预测SAP串联实验的敏感性和特异性是60.2%和77.9%,并联实验的敏感性和特异性是96.2%和27.0%。结论脑卒中早期CD4+T淋巴细胞计数联合AIS-APS及ICH-APS可提高识别发生SAP高危患者的准确性和特异性。 Objective To observe CD4+T lymphocyte count combined with acute ischemic strokeassociated pneumonia score(AIS-APS)and intracecerebral hemorrhage-associated pneumonia score(ICH-APS)assess the predictive value of stroke-associated pneumonia(SAP)respectively.Methods The consecutive patients with actue stroke admitted to hospital in the NICU within24 hours after symptom onset.57 cases were recruited as the research objects.They were divided into the SAP group and non-SAP group according to whether they had SAP or not within one weeks after symptom onset.Patients according to cerebral infarction and cerebral hemorrhage were graded by AIS-APS and ICH-APS score respcetively.The accuracy of CD4+T lymphocyte count combined with AIS-APS and ICH-APS were analyed by series prediction experiment and parallel experiment.Results CD4+T lymphocyte count<1077 cell/ul and AIS-APS>17 were uinited to predict SAP,the sensitivity and specificity of series experiment were 59.7% and 88.9%.the sensitivity and specificity of parallel test were 96% and 43.2%.CD4+T lymphocyte count<1077 cell/ul and ICH-APS>17 were uinited to predict SAP,the sensitivity and specificity of series experiment were 60.2%and 77.9%.the sensitivity and specificity of parallel test were 96.2%and 27.0%.Conclusion CD4+T lymphocyte count combined with AIS-APS and ICH-APS could improve the accuracy and specificity in recognition of high risk SAP patients.
