摘要
“将健康融入所有政策”是新时代我国卫生与健康工作方针的重要组成部分,也是推进健康中国建设的关键实施策略。它植根于马克思主义关于影响健康的政治经济因素的分析,强调国民健康的维护不应仅仅依靠医疗和公共卫生部门,而是要将其他部门也纳入其中,因为所有部门的政策都会对健康和健康公平产生影响。近几十年来,尤其是新世纪以来,一些国家积极探索“将健康融入所有政策”的实践方式,并取得了一些积极成果。然而,由于这些国家的实践主要是基于“健康的社会决定因素”这一“去政治化”的公共卫生研究范式,缺乏对生产方式和权力关系的考察,导致政策效果受到限制。本文尝试从政治经济学视角出发,总结和归纳芬兰、澳大利亚、美国等国推行“将健康融入所有政策”的经验和教训,为我国顺利落实这一战略任务提供政策依据和参考。
Health in All Policies(HiAP)is a crucial guiding principle and a key strategic approach for promoting a healthy China in the New Era.Rooted in Marxist theories that analyze the political and economic determinants of health,HiAP emphasizes that a country's health policies should not be confined to the healthcare and public health sectors alone.Instead,all policy sectors should be involved and engaged,as they all have an impact on health.In recent decades,especially as we move into the 21^(st) century,there has been a worldwide recognition and adoption of HiAP approaches,leading to some positive results.However,these practices predominantly rely on the“social determinants of health”paradigm,which is the mainstream research paradigm in public health in understanding how population health is shaped.Such a de-politicalized perspective lacks critical analyses of the mode of production and power relations,limiting the effectiveness of HiAP.Through a political economy lens,this article summarizes and draws insights from the experiences and lessons learned in countries such as Finland,Australia,the United States at both central and local levels,which have important policy implications for further implementation of HiAP in China.
