摘要
目的姑息照顾和临终关怀服务是典型的团队式协调服务。探讨以社区为基础的临终关怀服务团队的演进,以及通过有效团队工作提高对临终老人及家属的服务质量的策略。方法以塔克曼团队发展模型为框架,采用参与式观察法,对深圳市某社区养护中心的临终关怀服务团队的建立和演进过程进行记录和演绎分析,研究资料包括会议记录、小组讨论记录、工作交班记录、与家属谈话记录、平行病例。结果以社区为基础的临终关怀团队具有团队生命周期短的特点,且多处于形成期,团队面临的挑战是形成阶段的共同特点,即团队成员来自不同的专业或服务领域,各自遵守自己的职业标准和认知,利用团队成员特征是引导团队成为高效团队的有效工具,总结了团队构建的经验教训。结论形成一个高效的团队是一个生命周期过程,塔克曼团队发展模型提供了一个框架来回顾和理解团队的创建过程和服务目标,并提示要创造机会让团队从形成演进到高效阶段。多学科和跨专业学习是准备团队的充足条件。
Objective palliative care and hospice care are typical team coordination services.This paper discusses the evolution of community-based hospice care service team and the strategies to improve the service quality for the dying elderly and their families through effective team work.Methods based on the tacman team development model,the establishment and evolution process of hospice care service team in a community maintenance center in Shenzhen were recorded and analyzed by participatory observation method.The research data included meeting minutes,group discussion records,work handover records,conversations with family members and parallel cases.Results Community-based hospice care teams are characterized by a short team life cycle and are mostly in the formation stage.The challenge the team faces is the common feature of the formation stage,that is,team members come from different professions or service fields and each abide by their own professional standards And cognition,using the characteristics of team members is an effective tool to guide the team to become an efficient team,and summarizes the experience and lessons of team building.Conclusion Forming an efficient team is a life cycle process.Tuckerman’s team development model provides a framework to review and understand the team’s creation process and service goals,and prompts to create opportunities for the team to evolve from the formation to the efficient stage.Multi-disciplinary and inter-professional studies are sufficient conditions to prepare the team.
