摘要
目的研究椎间孔镜微创手术联合新型超强黏附材料治疗腰椎间盘突出症合并骨质疏松症患者的临床疗效及对患者止血和组织再生的影响。方法回顾性分析2018年9月—2019年9月在本院接受椎间孔镜微创手术的80例腰椎间盘突出症合并骨质疏松症患者的临床治疗资料进行。按照患者手术治疗中使用的敷料不同将其分为研究组和对照组两组,每组各40例。研究组患者在手术中使用新型超强黏附材料制成的敷料进行伤口处理,对照组则使用传统纱布敷料进行处理。评估两组患者出血量、止血时间、VAS疼痛评分、愈合效果和愈合时间及并发症发生,并进行对比。结果研究组患者术后出血量、止血时间及VAS疼痛评分均显著低于对照组,差异具有统计学意义(P<0.01);研究组患者愈合等级为甲级的患者明显多于对照组,研究组患者愈合时间比对照组短,差异均具有统计学意义(P<0.05);研究组患者皮下血肿、伤口感染及皮肤瘀斑等并发症发生率显著少于对照组,差异具有统计学意义(P<0.05)。结论椎间孔镜微创手术联合新型超强黏附材料作为术后敷料治疗腰椎间盘突出症合并骨质疏松症患者具有良好的止血及促进创面愈合的效果,在临床应用中患者并发症的发生风险更低,安全性更高。
Objective To evaluate the clinical efficacy of minimally invasive transforaminal endoscopic surgery combined with new super adhesive materials for patients with lumbar disc herniation and osteoporosis,and its influence on hemostasis and tissue regeneration.Methods The clinical data of 80 patients with lumbar disc herniation complicated with osteoporosis who underwent minimally invasive surgery via transforaminal endoscope in our hospital from September 2018 to September 2019 were retrospectively analyzed.According to the different dressings used in the surgical treatment,the patients were divided into study group and control group.Surgical dressing made by new super adhesive materials was applied in the wound treatment in study group,and traditional gauze made surgical dressing was adopted in the control group.The amount of bleeding,time of hemostasis,VAS pain score,healing effect,healing time,and the occurrence of complications of the two groups were assessed and compared.Results The postoperative bleeding volume,hemostasis time and VAS score of the study group were significantly lower than those of the control group(P<0.01).The proportion of grade A healing in the study group was significantly higher than that in the control group,the healing time of patients in the study group wasshorter than that of the control group,with statistically significant(P<0.05).The incidence of subcutaneous hematoma,wound infection and skin ecchymosis complications in the study group was significantly lower than that in the control group(P<0.05).Conclusions Transforaminal endoscopic minimally invasive surgery combined with new super adhesive material as postoperative dressing possesses favourite hemostasis effect and promoting wound healing in patients with lumbar disc herniation and osteoporosis.In the application,the complication rate was relatively low,and with high safety.
