术后细菌性颅内感染患者脑脊液炎性因子水平与血脑屏障完整度的关系 被引量:2

Relationship between inflammatory factors in cerebrospinal fluid and the integrity of blood-brain barrier in patients with bacterial intracranial infection

摘要 目的探讨神经外科术后细菌性颅内感染患者脑脊液白细胞介素-1β(IL-1β)、IL-6水平与血脑屏障完整度的关系。方法收集2019年11月-2021年11月北京市平谷区医院收治的78例神经外科手术后疑似颅内感染患者,根据颅内感染诊断标准将其分为感染组(n=13)和非感染组(n=65),采集所有患者脑脊液和外周静脉血,检测脑脊液白细胞计数(WBC)、C反应蛋白(CRP)、降钙素原(PCT)、IL-1β、IL-6和基质金属蛋白酶-9(MMP-9)水平,以及白蛋白(C_(Alb))和血清白蛋白(S_(Alb))水平,并计算白蛋白指数(C_(Alb)/S_(Alb));采用Pearson相关性分析脑脊液炎症指标与白蛋白指数的关系。结果感染组患者的脑脊液WBC、CRP、PCT、IL-1β、IL-6、MMP-9、C_(Alb)水平以及白蛋白指数分别为(20.25±9.93)×10^(6)/L、(35.30±8.90)mg/L、(0.22±0.07)ng/mL、(40.28±10.72)pg/mL、(25.30±7.24)pg/mL、(417.05±99.14)μg/L、(1.07±0.18)g/L、(26.65±3.92),均高于非感染组的(6.35±2.58)×106/L、(6.13±1.97)mg/L、(0.03±0.02)ng/mL、(10.96±3.36)pg/mL、(7.44±1.85)pg/mL、(128.80±20.96)μg/L、(0.19±0.06)g/L、(4.80±0.84),差异均有统计学意义(t=9.942、24.173、18.766、18.352、17.598、21.642、32.089、41.380,P均<0.001)。颅内感染患者脑脊液IL-1β、IL-6和MMP-9与白蛋白指数均成正相关(r=0.525、0.577、0.648,P均<0.05)。结论神经外科术后颅内感染患者脑脊液中IL-1β、IL-6和MMP-9表达水平显著升高,IL-1β、IL-6和MMP-9可能参与了颅内感染血脑屏障功能损伤的病理过程。 Objective To explore the relationship between levels of interleukin-1β(IL-1β)and IL-6 in cerebrospinal fluid and the integrity of blood-brain barrier in patients with bacterial intracranial infection after neurosurgery.Methods A total of 78 patients with suspected intracranial infection after neurosurgery admitted to the Beijing Pinggu District Hospital were enrolled between November 2019 and November 2021.According to the diagnostic criteria of intracranial infection,they were divided into infection group(n=13)and non-infection group(n=65).The cerebrospinal fluid and peripheral venous blood were collected to detect levels of white blood cell count(WBC),C-reactive protein(CRP),procalcitonin(PCT),IL-1β,IL-6 and matrix metalloproteinase-9(MMP-9)in cerebrospinal fluid,and levels of cerebrospinal fluid albumin(C_(Alb))and serum albumin(S_(Alb))were detected.The albumin index(C_(Alb)/S_(Alb))was calculated.The relationship between inflammation indexes in cerebrospinal fluid and albumin index was analyzed by Pearson correlation analysis.Results The levels of WBC,CRP,PCT,IL-1β,IL-6,MMP-9,C_(Alb) and albumin index in cerebrospinal fluid in infection group were(20.25±9.93)×10^(6)/L,(35.30±8.90)mg/L,(0.22±0.07)ng/mL,(40.28±10.72)pg/mL,(25.30±7.24)pg/mL,(417.05±99.14)μg/L,(1.07±0.18)g/L and(26.65±3.92),respectively,which were significantly higher than those in non-infection group[(6.35±2.58)×106/L,(6.13±1.97)mg/L,(0.03±0.02)ng/mL,(10.96±3.36)pg/mL,(7.44±1.85)pg/mL,(128.80±20.96)μg/L,(0.19±0.06)g/L and(4.80±0.84),respectively;the differences were statistically significant(t=9.942,24.173,18.766,18.352,17.598,21.642,32.089,41.380;all P<0.001).IL-1β,IL-6 and MMP-9 levels in cerebrospinal fluid were significantly positively correlated with albumin index(r=0.525,0.577,0.648;all P<0.05).Conclusion The expression levels of IL-1β,IL-6 and MMP-9 in cerebrospinal fluid significantly increased in patients with intracranial infection after neurosurgery,which might be involved in the pathological injury of blood-brain barrier function.
