摘要
目的探讨白细胞介素-12(IL-12)基因rs2243115、rs3212227位点多态性与慢性阻塞性肺疾病(COPD)患者肺部侵袭性真菌感染(IFI)的关系。方法选取2019年1月-2021年6月京东中美医院收治的96例COPD继发肺部IFI患者作为研究组,另选取同期未继发肺部IFI的COPD患者80例作为对照组,统计两组患者IL-12基因rs2243115、rs3212227位点多态性和临床病例资料,并分析其与肺部IFI的关系。结果研究组IL-12 rs2243115位点GG基因型、G等位基因频率高于对照组,IL-12 rs3212227位点CC基因型、C等位基因频率高于对照组,差异均有统计学意义(χ^(2)=10.733、12.092、13.143、14.550,P均<0.05)。采用二元logistic回归分析,吸烟史(OR=3.918,P=0.004)、糖尿病(OR=3.499,P=0.006)、使用皮质类固醇(OR=5.871,P<0.001)、机械通气(OR=3.467,P=0.003)、抗生素使用时间>2周(OR=4.336,P<0.001)、血清白蛋白≤35 g/L(OR=3.139,P=0.006)、IL-12 rs2243115基因(OR=5.146,P=0.004)、IL-12 rs3212227(OR=5.236,P=0.004)基因均为影响COPD患者肺部IFI的独立危险因素(P<0.05)。结论IL-12rs2243115位点G基因和IL-12 rs3212227位点C基因是COPD患者肺部IFI的易感基因。
Objective To explore the relationship between polymorphisms of interleukin-12(IL-12)gene at rs2243115 and rs3212227 loci and pulmonary invasive fungal infection(IFI)in patients with chronic obstructive pulmonary disease(COPD).Methods A total of 96 patients with COPD secondary to pulmonary IFI were enrolled as study group in the Jingdong Zhongmei Hospital from January 2019 to June 2021,while 80 COPD patients without pulmonary IFI during the same period were enrolled as control group.The polymorphisms of IL-12 gene at rs2243115 and rs3212227 loci,and clinical data in both groups were statistically analyzed.And their relationship with pulmonary IFI was analyzed.Results The frequencies of GG genotype and G allele at IL-12 rs2243115 locus in study group were higher than those in control group,and frequencies of CC genotype and C allele at IL-12 rs3212227 locus were higher than those in control group(χ^(2)=10.733,12.092,13.143,14.550;all P<0.05).Binary logistic regression analysis showed that smoking history(OR=3.918,P=0.004),diabetes mellitus(OR=3.499,P=0.006),usage of corticosteroid(OR=5.871,P<0.001),mechanical ventilation(OR=3.467,P=0.003),usage time of antibiotics>2 weeks(OR=4.336,P<0.001),serum albumin≤35 g/L(OR=3.139,P=0.006),IL-12 rs2243115 gene(OR=5.146,P=0.004)and IL-12 rs3212227 gene(OR=5.236,P=0.004)were independent risk factors of pulmonary IFI in COPD patients(P<0.05).Conclusion G allele at IL-12 rs2243115 locus and C allele at IL-12 rs3212227 locus were susceptibility genes of pulmonary IFI in COPD patients.
