摘要
目的探究痛风患者外周血单个核细胞中miR-223及核苷酸结合寡聚化结构域样受体蛋白3(NLRP3)表达水平变化,并探讨其对痛风患者病情评估的意义。方法选取2018年9月-2019年12月于上海健康医学院附属崇明医院门诊收治的痛风患者150例为研究对象(研究组),另选取同期健康体检者150名作为对照(对照组)。收集受试对象的一般资料与相关实验室指标,包括空腹血糖(FPG)、尿酸(UA)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、C反应蛋白(CRP)和红细胞沉降率(ESR);采用实时荧光定量PCR(qRT-PCR)技术检测各组受试对象外周血单个核细胞中miR-223及NLRP3的相对表达量;Pearson法分析痛风患者外周血单个核细胞中miR-223及NLRP3与实验室指标的相关性。结果2组FPG、HDL-C、LDL-C水平比较差异均无统计学意义(P均>0.05)。研究组UA、CRP、ESR、NLRP3水平[(512.06±49.61)μmol/L、(4.23±0.56)ng/L、(41.62±4.34)mm/h、(2.32±0.37)]明显高于对照组的(296.31±33.56)μmol/L、(2.27±0.32)ng/L、(13.59±1.26)mm/h、(1.00±0.00),差异均有统计学意义(t=44.117、37.218、75.964、43.694,P均<0.05);研究组血清miR-223水平(0.61±0.08)低于对照组(1.00±0.00),差异有统计学意义(t=59.706,P<0.05)。痛风患者外周血单个核细胞中miR-223与NLRP3表达水平成负相关(r=-0.825,P<0.001);痛风患者外周血单个核细胞中miR-223水平与UA、CRP、ESR成负相关(P均<0.05),NLRP3水平与UA、CRP、ESR成正相关(P均<0.05)。结论痛风患者外周血单个核细胞中miR-223呈低表达,NLRP3呈高表达,且二者水平与患者UA、CRP、ESR有关,可能作为指示痛风疾病的潜在生物指标。
Objective To explore the changes in the expression levels of miR-223 and nucleotide-binding oligomerization domain-like receptor protein 3(NLRP3)in peripheral blood mononuclear cells of patients with gout,and study their significance in the evaluation of the condition of patients with gout.Methods A total of 150 gout patients who were admitted to the outpatient clinic of the Chongming Hospital Affiliated to Shanghai University of Medicine and Health Sciences from September 2018 to December 2019 were selected as the research object(study group),and 150 healthy patients during the same period were selected as the control(control group).The general information and related laboratory indicators of the subjects were collected,including fasting blood glucose(FPG),uric acid(UA),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),C-reactive protein(CRP)and erythrocyte sedimentation rate(ESR);real-time fluorescence quantitative PCR(qRT-PCR)technology was used to detect the relative expression levels of miR-223 and NLRP3 in peripheral blood mononuclear cells of subjects in each group;Pearson method was used to analyze the correlation between miR-223 and NLRP3 in peripheral blood mononuclear cells and laboratory indicators of patients with gout.Results There were no significantly differences in FPG,HDL-C and LDL-C between two groups(all P>0.05).The levels of UA,CRP,ESR,NLRP3 in study group[(512.06±49.61)μmol/L,(4.23±0.56)ng/L,(41.62±4.34)mm/h,(2.32±0.37)]were higher than those in the control group[(296.31±33.56)μmol/L,(2.27±0.32)ng/L,(13.59±1.26)mm/h,(1.00±0.00)];the differences were significance(t=44.117,37.218,75.964,43.694;all P<0.05);the level of serum miR-223 in the study group(0.61±0.08)was lower than that in the control group(1.00±0.00),the difference was significance(t=59.706,P<0.05).The expression levels of miR-223 and NLRP3 in peripheral blood mononuclear cells of patients with gout were negatively correlated(r=-0.825,P<0.001);the level of miR-223 in peripheral blood mononuclear cells of patients with gout was negatively correlated with UA,CRP and ESR(all P<0.05),and the level of NLRP3 was positively correlated with UA,CRP and ESR(all P<0.05).Conclusion The miR-223 was expressed in a low level in peripheral blood mononuclear cells of patients with gout,while NLRP3 was highly expressed,and the levels of both were related to UA,CRP and ESR of patients,and might be used as potential biological indicators for gout disease.
