摘要
目的宽胸气雾剂治疗冠心病心绞痛临床使用日益广泛。本研究旨在系统评价该药治疗冠心病心绞痛的临床疗效和安全性。方法计算机检索PubMed、Medline、Embase、the Cochrane Library、中国期刊全文数据库(CNKI)、万方数据库、维普数据库(VIP)等数据库,搜集宽胸气雾剂在治疗冠心病心绞痛的随机对照研究(对照组采用硝酸甘油、观察组采用宽胸气雾剂),检索时间自建库至2019-08-31。根据纳入、排除标准筛选文献、提取数据并进行方法学系统评价,通过RevMan 5.3.3工具进行荟萃分析(Meta分析)并解读结果。结果本研究最终纳入12个随机对照试验(randomized controlled trial,RCT),涉及冠心病心绞痛病例总计2222例;纳入的12个研究中,有1个明确说明采用随机号码法随机分组,有4个明确说明采用随机数字表法随机分组,其余7个研究未提及采用何种随机分组方法。Meta分析结果表明,宽胸气雾剂治疗冠心病心绞痛临床疗效略高于对照组(RR=1.06,95%CI为1.03~1.08),差异有统计学意义,Z=4.34,P<0.05;心绞痛患者心电图改善与对照组相比(OR=1.14,95%CI为0.91~1.14),差异无统计学意义,Z=1.12,P=0.26。结论宽胸气雾剂在冠心病心绞痛临床治疗方面有效,建议开展更多优质RCT加以验证。
OBJECTIVE Kuanxiong aerosol is widely used in the treatment of coronary heart disease with angina pectoris.This study aimed to evaluate the clinical efficacy and safety of kuanxiong aerosol in the treatment of angina pectoris of coronary heart disease.METHODS The PubMed,Medline,Embase,the Cochrane Library,Chinese Journal full-text Database(CNKI),Wanfang Database and Vipp Database(VIP)were searched by computer.A randomized controlled study of kuanxiong aerosol in the treatment of angina pectoris of coronary heart disease was collected.The control group was treated with nitroglycerin and the observation group was treated with kuanxiong aerosol.The retrieval time was from establishment to August 31,2019.According to the inclusion and exclusion criteria,the literature was screened,the data were extracted and the methodological system was evaluated.The meta analysis was carried out by RevMan 5.3.3 tool and the results were interpreted.RESULTS This study eventually included 12 RCTs involving a total of 2222 cases of coronary heart disease and angina Pectoris.Of the 12 studies included,1 clearly stated the use of random number grouping,4 clearly stated the use of random number table grouping,and the remaining 7 studies did not mention the use of random grouping methods.The results of the meta-analysis showed that the clinical efficacy of broad-breather aerosol was slightly higher than that of the control group(RR=1.06,95%CI:1.03-1.08).The difference was statistically significant(Z=4.34,P<0.05).There was no significant difference in the improvement of ECG between angina pectoris group and control group(OR=1.14,95%CI:0.91-1.14,Z=1.12,P=0.26).CONCLUSION Kuanxiong aerosol is effective in the clinical treatment of coronary heart disease and angina pectoris,and more high-quality RCTs are recommended for verification.
