摘要
目的分析基层医院慢性阻塞性肺病(COPD)患者血嗜酸性粒细胞(EOS)分布特点,以及高EOS型COPD患者的临床特征及其影响因素。方法选取2019-08-01-2020-11-30泰州市第四人民医院收治的235例稳定期COPD患者为研究对象,根据血清EOS计数高低(以100μL^(-1)为标准)分为高EOS组(145例)和低EOS组(90例),分析比较2组患者的一般资料、职业因素、呼吸道症状、感染加重、肺功能分级、炎症情况、X射线资料等指标。结果高EOS组男性比例为85.5%,高于低EOS组的74.4%,χ^(2)=4.397,P=0.035;高EOS组的肺功能分级较低EOS组更高,χ^(2)=11.132,P=0.011;中性粒细胞比例为61.8%,低于低EOS组的64.8%,u=5373.000,P=0.014。多因素Logistic回归分析结果显示,高EOS与患者年龄增加(OR=1.036,95%CI为1.004~1.067,P=0.029)、中性粒细胞比例较低(OR=0.956,95%CI为0.923~0.991,P=0.015)以及肺功能分级较重(P=0.015)均有关。结论基层多数COPD患者血清EOS≥100μL^(-1),高EOS与性别、年龄、低中性粒细胞及肺功能分级严重程度相关。
Objective To analyze the distribution characteristics of eosinophil(EOS)in patients with chronic obstructive pulmonary disease(COPD)in primary hospitals,and to study the clinical characteristics of patients with high EOS COPD.Methods A total of 235 stable COPD patients admitted to the Fourth People’s Hospital of Taizhou city from August 1,2019 to November 30,2020 were selected.According to the blood EOS count(100μL^(-1)as the standard),they were divided into high EOS group(145 cases)and low EOS group(90 cases).The general information,occupational factors,respiratory symptoms,exacerbation of infection,lung function grade,inflammation,X-ray data and other indicators were analyzed between the two groups.Results The proportion of males in the group with high EOS was higher(85.5%vs 74.4%,χ^(2)=4.397,P=0.035),the lung function grading was heavier(χ^(2)=11.132,P=0.011),and the proportion of neutrophils was lower(61.8%vs 64.8%,u=5373.000,P=0.014),with statistical significance.Results of multivariate Logistic regression analysis showed that higher EOS was associated with increased age(OR=1.036,95%CI:1.004-1.067,P=0.029)and lower proportion of neutrophils(OR=0.956,95%CI:0.923-0.991,P=0.015)and higher lung function grading(P=0.015).Conclusion Blood EOS≥100μL^(-1)was correlated with age,low neutrophils and severity of pulmonary function grading in most patients with COPD at primary level.
