摘要
目的探讨黑色素瘤抗原3(MAGE-A3)及黑色素瘤抗原4(MAGE-A4)在食管鳞癌中的表达及其与临床病理特征之间的关系。方法收集2017-06-01-2020-06-01南华大学衡阳医学院附属郴州医院(9例)及广州市番禺区中心医院(34例)手术切除并经病理确诊的43例食管癌组织(其中高分化8例、中分化27例、低分化8例)及相应的癌旁组织(距离食管癌组织切缘>3 cm),采用免疫组化法检测MAGE-A3/A4蛋白在食管组织中的表达情况。结果MAGE-A3/A4蛋白在食管癌组织中阳性表达率分别为67.4%和62.8%,在癌旁组织中分别为11.6%和9.3%,差异有统计学意义,χ^(2)值分别为28.02和26.68,均P<0.001。MAGE-A3/A4蛋白在食管癌组织中的表达与TNM分期(χ^(2)=14.707,P<0.001;χ^(2)=8.555,P=0.003)和淋巴结转移(χ^(2)=19.724,P<0.001;χ^(2)=6.331,P=0.012)有关,与其年龄(χ^(2)=0.973,P=0.324;χ^(2)=0.078,P=0.780)、性别(χ^(2)=3.612,P=0.057;χ^(2)=2.604,P=0.107)和分化程度(χ^(2)=1.218,P=0.270;χ^(2)=0.951,P=0.329)无关。结论MAGE-A3/A4在食管鳞癌组织中高表达与食管癌的发生发展及侵袭转移关系密切,对其早期筛查、临床诊治及疾病预后有着重要影响。
Objective To study the expression of melanoma-associated antigen-A3(MAGE-A3)and melanoma-associated antigen-A4(MAGE-A4)in esophageal squamous cell carcinoma(ESCC)and their relationship with clinicopathological features.Methods The tissues of 43 patients with esophageal cancer(including 8 highly differentiated cases,27 cases of moderately differentiated and 8 cases of poorly differentiated)and adjacent normal tissues(including marginal tissues)were collected from The Affiliated Chenzhou Hospital to University of South China(9 patients)and Panyu Central Hospital of Guangzhou(34 patients)between 1 June 2017 to 1 June 2020.Using immunohistochemistry to detect the protein expression of MAGE-A3 and MAGE-A4.Results The positive expression rates of MAGE-A3 and MAGE-A4 protein in esophageal carcinoma were 67.4%and 62.8%respectively,and the positive expression rates of MAGE-A3 and MAGE-A4 protein in their adjacent tissues were respective 11.6%and 9.3%,respectively.The difference between these groups was statistically significant(χ^(2)=28.02,26.68,P<0.001).The expression of MAGE-A3 and MAGE-A4 was associated with lymph node metastasis(χ^(2)=19.724,P<0.001;χ^(2)=6.331,P=0.012)and clinical staging(χ^(2)=14.707,P<0.001;χ^(2)=8.555,P=0.003),but there was no correlation between tumor differentiation(χ^(2)=1.218,P=0.270;χ^(2)=0.951,P=0.329),gender(χ^(2)=3.612,P=0.057;χ^(2)=2.604,P=0.107)and age(χ^(2)=0.973,P=0.324;χ^(2)=0.078,P=0.780).Conclusion The high expression of MAGE-A3 and MAGE-A4 in ESCC is closely related to the occurrence,development,invasion and metastasis of esophageal carcinoma,and is an important indicator of its diagnosis,treatment and prognosis.
