摘要
目的探讨双重血浆分子吸附在肝衰竭治疗中的临床应用,总结应用体会。方法运用Aquarius机器DPMAS模式进行6次治疗。将血浆分离器、胆红素吸附器、血浆灌流器进行预处理后与体外循环管路连接,监测患者临床症状、体征、重要生化指标及机器参数,实施个性化护理。结果患者肝功能恢复正常,黄疸消失,痊愈出院。结论双重血浆分子吸附(DPMAS)能大量、持续清除中大分子、蛋白结合毒素,特异性清除胆红素胆汁酸,对内环境影响小,且不依赖血浆,避免了感染及过敏风险,是等待肝细胞再生,肝功能恢复或肝移植的前驱治疗,但该技术是集治疗性操作与护理于一体的特殊方式,是一种血液净化的新技术,需要更高的专业性与技术性护理。
Objective To explore the clinical application of double plasma molecular adsorption in the treatment of liver failure,and summarize the application experience.Methods Six times of treatment were performed by using the dpmas mode of the Aquarius machine.After pretreatment,the plasma separator,bilirubin adsorber and hemoperfusion device were connected with the cardiopulmonary bypass pipeline to monitor the clinical symptoms,physical signs,important biochemical indicators and machine parameters of patients,and personalized nursing was carried out.Results The patient’s liver function returned to normal,jaundice disappeared,and recovered.Conclusion DPMAS can effectively remove harmful substances,improve the internal environment,and create conditions for liver cell regeneration and liver function recovery.However,artificial liver technology is a special way integrating therapeutic operation and nursing.It is a new technology of blood purification,which needs higher professional and technical nursing.
