微创锁定加压钢板内固定对老年骨质疏松性肱骨近端骨折疼痛程度、肩关节功能的影响 被引量:1

Effect of Minimally Invasive Locking Compression Plate Internal Fixation on Pain Degree and Shoulder Joint Function of Senile Osteoporotic Proximal Humeral Fracture

摘要 目的探讨对老年骨质疏松性肱骨近端骨折患者使用微创锁定加压钢板内固定治疗的临床疗效。方法选取2018年1月-2019年12月期间在我院治疗的60例老年骨质疏松性肱骨近端骨折患者,将其随机分为甲组和乙组,各30例。乙组使用传统胸三角肌入路切开复位锁定钢板内固定手术进行治疗,甲组使用肩外侧小切口闭合复位+微创锁定加压钢板内固定手术进行治疗,比较两组的手术相关指标、视觉模拟评分法(VAS)评分及Neer肩关节功能评分情况。结果术后1 d、3 d、1周及2周,甲组VAS评分均显著低于乙组,差异有统计学意义(P<0.05);甲组固定时间及固定后住院时间比乙组短,术中出血量比少于乙组少,差异有统计学意义(P<0.05);术后0.5个月、1个月及3个月,甲组Neer肩关节功能优良率均明显高于乙组,差异有统计学意义(P<0.05)。结论老年骨质疏松性肱骨近端骨折患者经微创锁定加压钢板内固定手术治疗,可有效控制术中出血量,减轻疼痛程度,利于提高肩关节功能。 Objective This paper aims to investigate the clinical efficacy of minimally invasive locking compression plate internal fixation in treatment of senile osteoporotic proximal humeral fracture.Methods 60 senile patients with osteoporotic proximal humeral fracture who treated in the hospital from January 2018 to December 2019 were randomly divided into A group and B group,with 30 cases in each group.B group underwent conventional thoracic deltoid approach of open reduction locking plate internal fixation,and A group was treated with small lateral shoulder incision closed reduction and minimally invasive locking compression plate internal fixation.The surgical indexes,visual analogue scale(VAS)and Neer shoulder joint function score were compared between two groups.Results After surgery for 1 d,3 d,1 w and 2 w,the VAS scores of A group were significantly lower than those of B group(P<0.05);The fixation time and hospital stays after fixation in A group were shorter than those in B group,and the intraoperative bleeding volume of A group was less than that of B group(P<0.05);After surgery for 0.5,1 and3 months,the excellent rate of Neer shoulder joint function in A group was higher than that in B group(P<0.05).Conclusion Minimally invasive locking compression plate internal fixation in treatment of senile osteoporotic proximal humeral fracture can control intraoperative bleeding volume,reduce pain degree,and improve shoulder joint function.
