摘要
目的探讨卡诺模型的护理干预在老年肾功能衰竭血液透析患者的应用效果。方法选取2019年10月至2021年9月在我院进行治疗的老年肾功能衰竭血液透析患者78例,按照随机分组的方法将其分为对照组和观察组,每组各39例,对照组采用常规护理方式,观察组在对照组基础上采用卡诺模型的护理干预,比较两组患者护理干预前后的主观营养评分(SGA)和营养不良-炎症评分(MIS)、焦虑和抑郁评分、两组患者的生活质量和满意度。结果护理干预前两组患者的营养情况无明显变化(P>0.05),干预后SGA和MIS评分均降低,且观察组低于对照组(P<0.05);两组患者护理干预前SAS和SDS评分均无明显差异(P>0.05),干预后两组患者评分均降低,且观察组SAS和SDS评分均低于对照组(P<0.05);观察组患者生理功能、社会功能、情感职能、精神情况、生理精力5个维度评分均高于对照组(P<0.05);观察组患者总满意率高于对照组(P<0.05)。结论卡诺模型的护理干预在老年肾功能衰竭血液透析患者的临床应用具有可行性,可以改善患者的生活质量,降低患者的焦虑抑郁概率,从而减少营养不良的状况。
Objective:To explore the effect of nursing intervention of Carnot model in elderly hemodialysis patients with renal failure.Methods:78 elderly hemodialysis patients with renal failure who were treated in our hospital from October 2019 to September 2021 were selected as the research objects.According to the method of random grouping,they were divided into the control group and the observation group,with 39 cases in each group.The control group adopted conventional nursing,and the observation group adopted Carnot model nursing intervention on the basis of the control group.Subjective nutrition score(SGA),malnutrition-inflammation score(MIS),anxiety and depression score,quality of life and patients'satisfaction were compared between the two groups before and after nursing intervention.Results:Before nursing intervention,there was no significant change in the nutritional status of the two groups(P>0.05),SGA and MIS scores were decreased after intervention,and the observation group was lower than the control group(P<0.05).There was no significant difference in SAS and SDS scores between the two groups before nursing intervention(P>0.05),but after intervention,the scores of SAS and SDS in the observation group were lower than those in the control group(P<0.05).The scores of physiological function,social function,emotional function,mental condition and physical energy in observation group were higher than those in control group(P<0.05).The total satisfaction rate of observation group was higher than control group(P<0.05).Conclusion:Carnot model nursing intervention is feasible in the clinical application of elderly hemodialysis patients with renal failure,which can improve the quality of life,reduce the probability of anxiety and depression,and reduce the status of malnutrition.
