摘要
目的评估齿形结扎线形切剥微创术治疗静脉曲张性混合痔伴HIV的手术疗效及对细胞免疫功能的影响。方法采用临床对照组研究,选取2018年3月-2020年8月本院肛肠科收治的36例静脉曲张性混合痔伴HIV感染患者为研究对象,随机分为观察组与对照组,每组18例,对照组予以分段齿形结扎术,观察组予以齿形结扎线形切剥微创术。治疗后比较两组手术效果,评估手术对患者细胞免疫的影响。结果治疗后观察组治疗疗效显著优于对照组,差异具有统计学意义(P<0.05);且观察组创面愈合时间、创面无渗血时间水肿消失时间均低于对照组(P<0.05);治疗后,观察组的CD4细胞显著高于对照组,差异具有统计学意义(P<0.05);两组的CD4/CD8、白细胞计数指标无统计学意义(P均>0.05)。观察组无并发症发生,对照组总发生率16.67%,但差异无统计学意义(P<0.05)。结论齿形结扎线形切剥微创术治疗静脉曲张性混合痔伴HIV感染疗效较好,且未增加不良反应,安全性较高。
Objective To evaluate the effect of tooth ligation and linear dissection in the treatment of varicose mixed hemorrhoids with HIV.Methods 36 patients with variceal mixed hemorrhoids complicated with HIV infection admitted to department of anorectal of our hospital from March 2018 to August 2020 were selected as the research objects and randomly divided into the observation group and the control group,with 18 cases in each group.The control group was given segmented tooth ligation,and the observation group was given tooth ligation and linear stripping minimally invasive surgery.After treatment,the effect of surgery was compared between the two groups,and the effect of surgery on cellular immunity was evaluated.Results After treatment,the curative effect of the observation group was significantly better than that of the control group,the difference was statistically significant(P<0.05);and the wound healing time,wound bleeding free time and edema disappearance time of the observation group were lower than that of the control group(P<0.05);after treatment,the CD4 cells of the observation group were significantly higher than that of the control group,the difference was statistically significant(P<0.05);the CD4/CD8 and white blood cell count indexes of the two groups were not statistically significant.There was no statistical significance(P>0.05).There was no complication in the observation group,and the total incidence rate in the control group was 16.67%,but the difference was not statistically significant(P<0.05).Conclusion Minimally invasive surgery with tooth ligation and linear dissection is effective in the treatment of varicose mixed hemorrhoids with HIV infection,and it has high safety without increasing adverse reactions.
