摘要
目的分析竖脊肌平面阻滞对在腹腔镜患者术中及术后镇痛效果与应激反应的影响。方法选择某医院手术麻醉科2017年1月-2020年1月诊治的腹腔镜手术患者100例,采用简单数字分组法将其分成对照组与观察组,每组50例。对照组患者接受全身麻醉,观察组患者接受超声竖脊肌平面阻滞。比较两组患者一般指标、镇痛效果、应激反应、心肌酶水平和并发症。结果观察组患者丙泊酚和瑞芬太尼总量、术中出血量、输液量、手术时间、住院天数、术后6h、12h观察组患者VAS评分、Cor、Ang-Ⅱ、IL-6、IL-10浓度、并发症率低于对照组(P<0.05)。观察组患者术后24h CK、CK-MB水平低于对照组(P<0.05)。观察组患者并发症率低于对照组(P<0.005)。结论超声竖脊肌平面阻滞既能减少腹腔镜患者术中和术中疼痛,又能改善机体应激反应,提升镇痛效果,建议使用。
Objective To investigate the effect of erector spinae plane block on intra-and postoperative analgesia and stress response of patients with laparoscopic surgery.Methods 100 patients with laparoscopic surgery at department of Anesthesia in some hospital from January 2017 to January 2020 were enrolled,and divided into two groups according to random number table methods,with 50 cases in each group.Control group received general anesthesia,while observation group received ultrasound guided erector spinae plane block.The general indexes,analgesic effect,stress response,myocardial enzyme level and complications were compared between groups.Results The total amount of propofol and remifentanil,intraoperative blood loss,infusion volume,operation time,length of stay,VAS score at postoperative 6 h and 12 h,concentrations of cortisol(Cor),AngiotensinⅡ(AngⅡ),Interleukin-6(IL-6),and Interleukin-10(IL-10),and complication rate in observation group were lower than those in control group(P<0.05).The levels of creatine phosphokinase(CK)and creatine kinase-MB(CK-MB)were lower in observation group than in control group at postoperative 24 h(P<0.05).The complication rate of observation group was lower than that of control group(P<0.05).Conclusion Ultrasound guided erector spinae plane block can effectively alleviate the intra-and postoperative pain degree and stress response,improve the analgesic effect for patients with laparoscopic surgery.
