摘要
目的探讨磁共振三维稳态构成干扰序列(3D-CISS)联合三维时间飞跃法血管成像序列(3D-TOF)及多平面重建技术(MPR)在原发性三叉神经痛(PTN)中评估神经与邻近血管关系的诊断价值。方法回顾性分析34例经手术证实因血管压迫导致三叉神经痛患者的影像资料,术前均行3D-CISS和3D-TOF两种序列扫描并对所得图像行MPR重建,以手术实际结果为金标准,观察分析神经与邻近血管毗邻关系。结果34例患者中,单独使用3D-CISS序列、3D-TOF序列及二者联合多平面重建(MPR)显示神经—血管压迫(NVC)的阳性率分别为91.18%、76.47%、97.06%,3D-CISS序列对NVC的显示能力优于3D-TOF序列,3D-CISS、3D-TOF序列联合MPR的检查阳性率高于二者单独应用(P<0.05)。结论3D-CISS序列及3D-TOF序列对于显示三叉神经和邻近血管的关系可以很好地进行互补,结合MPR可以提高检查阳性率,对术前诊断及手术方案的制定有重要意义。
Objective To investigate the diagnostic value of three-dimensional steady-state composition interference sequence(3D-CISS)combined with three-dimensional time-leap angiography sequence(3D-TOF)and multi-planar reconstruction(MPR)in primary trigeminal neuralgia(PTN).Methods The imaging data of 34 patients with trigeminal neuralgia confirmed by surgery due to vascular compression were retrospectively analyzed.All patients underwent 3D-CISS and 3D-TOF sequence scanning and MPR reconstruction before surgery.The relationship between nerves and adjacent vessels was analyzed using the actual surgical observation results as the gold standard.Results Among the 34 patients,the positive rates of nerve-vascular compression(NVC)were 91.18%,76.47%,and 97.06%respectively using 3D-CISS sequence,3D-TOF sequence,and multiplanar reconstruction(MPR)through 3D-CISS sequencecombine with 3D-TOF sequence.The ability of 3D-CISS sequence to display NVC was better than 3D-TOF sequence,and the positive rate of 3D-CISS and 3D-TOF sequence combined with MPR was higher than that of the two sequence alone(P<0.05).Conclusion 3D-CISS sequence and 3D-TOF sequence can complement each other well for displaying the relationship between trigeminal nerve and adjacent blood vessels.Combined with MPR reconstruction,the positive rate of examination can be improved,which is of great significance for preoperative diagnosis and the formulation of surgical plan.
