摘要
目的 探讨改良下睑缩肌缩短术治疗老年性下睑内翻的临床效果。方法 选择60岁-80岁的老年病人74例(101眼),年龄60-82(平均62.5)岁。病程1-16(平均7.7)年。术后随访6月-3年。分为下睑缩肌缩短手术组48例(65只眼),其中男20例(28眼),女28例(37眼)和眼轮匝肌缩短联合皮肤去除手术组26例(36眼),其中男11例(16眼),女15例(20眼)两组年龄及性别构成无显著差异。结果 下睑缩肌缩短术患者共65眼,64眼治愈,1眼术后3月复发,治愈率为98%,并且外观改善良好,所有患者均表示满意;眼轮匝肌缩短联合皮肤切除治疗组36眼,26眼治愈,术后半年复发5眼,术后1年复发5眼,治愈率为72%。两组治愈率比较有统计学差异P=0.00(<0.05)。结论 下睑缩肌缩短术治疗老年性睑内翻效果良好,值得在临床上推广应用。
Objective To investigate the clinical effect of modified lower eyelid retractor shortening in the treatment of senile lower eyelid entropion.Methods 74elderly patients(101 eyes)aged from 60 to 80 years old were selected.The average age was 62.5 years old.The course of disease ranged from 1 to 16(mean 7.7)years.The patients were followed up for 6 months to 3 years.There were 48 cases(65 eyes)who were treated with lower eyelid constrictor shortening operation,including 20 males(28 eyes)and 28 females(37 eyes),and 26 cases(36 eyes)who were treated with orbicularis oculi shortening combined with skin removal operation group,including 11 males(16 eyes)and 15 females(20 eyes).Results There were 65 eyes of patients with lower eyelid constrictor shortening.Among them,64 cases were cured,while 1 eye recurred 3 months after operation.The cure rate was 98%,and the appearance improved well,and all patients were satisfied.There were 36 eyes with orbicularis oculi shortening combined with skin resection.Among them,26 eyes were cured,5 eyes recurred half a year after operation,and 5 eyes recurred 1 year after operation.The recovery rate was 72%.There was significant difference in the recoveryrate between the two groups(P<0.05).Conclusion The effect of lower eyelid constrictor shortening in the treatment of senile spastic entropion is good,which is worthy of clinical application.
