摘要
目的探讨电针治疗对职业性噪声聋(ONID)并发抑郁症状患者皮质醇与炎性因子水平的影响。方法采用随机数表法将78例ONID伴发抑郁症状患者分为对照组(40例)和电针组(38例)。对照组患者予常规治疗,治疗组患者在对照组基础上加用电针治疗,隔天1次,每周治疗3次,连续治疗4周。比较2组患者治疗前后唾液皮质醇水平及血清中皮质醇、白细胞介素(IL)-6、肿瘤坏死因子-α(TNF-α)及C反应蛋白(CRP)的水平。结果治疗前,2组患者早晨、晚间唾液皮质醇水平和血清的皮质醇、IL-6、TNF-α、CRP的水平分别比较,差异均无统计学意义(P>0.05)。电针组患者治疗后血清皮质醇水平较同组治疗前下降(P<0.01);对照组和电针组患者治疗后早晨、晚间唾液皮质醇及血清皮质醇水平分别与同组治疗前比较,差异均无统计学意义(P>0.05)。电针组患者治疗后血清中IL-6和TNF-α水平均低于同组治疗前及对照组治疗后(P<0.01)。结论电针治疗可降低ONID伴发抑郁症状患者血清TNF-α和IL-6水平;电针对炎性因子的调节可能是治疗ONID并发的抑郁症状的作用机制之一。
Objective To explore the effect of electroacupuncture treatment on the levels of cortisol and immune factors in patients with occupational noise-induced deafness(ONID)combined with depressive symptoms.Methods A total of 78 ONID patients combined with depressive symptoms were randomly divided into control group(40 cases)and electroacupuncture group(38 cases)by a random number table method.The control group received routine therapy,while the electroacupuncture group was treated with routine therapy plus electroacupuncture on alternate days,3 times a week for 4 consecutive weeks.The levels of salivary cortisol,as well as the serum levels of cortisol,tumor necrosis factor alpha(TNF-α),interleukin-6(IL-6)and C-reactive protein(CRP)of the two groups were observed and compared before and after treatment.Results Before treatment,the levels of saliva cortisol in the morning and night,and the serum levels of cortisol,IL-6,TNF-αand CRP in the two groups were compared,and the differences were not statistically significant(P>0.05).After treatment,the serum cortisol level of patients in electroacupuncture group reduced than that before treatment in the same group(P<0.05).However,there was no significant difference in the levels of salivary cortisol in the morning and night,and serum cortisol before and after treatment both in control group and electroacupuncture group(P>0.05).After treatment,the serum levels of IL-6 and TNF-αdecreased in electroacupuncture group as compared with those before treatment and those after treatment in control group(P<0.01).Conclusion Electroacupuncture treatment could reduce the levels of TNF-αand IL-6 in serum of ONID patients combined with depressive symptoms.The regulation of electroacupuncture on the inflammatory cytokines may be one of the mechanisms in treating ONID combined with depressive symptoms.
