摘要
目的研究HCV/HIV合并感染者的肝功能变化、机体免疫等情况,探讨其合并感染及单独感染的机制。方法用生化分析仪测定肝功能,流式细胞术测定CD4+和CD8+T细胞,实时荧光定量PCR检测HCV-RNA,ELISA检测HCV抗体。结果HCV/HIV合并感染者的ALT、AST与HCV感染患者相比明显更高;HCV/HIV合并感染患者的CD4+、CD8+T细胞比例明显低于HCV感染者的比例;HCV/HIV合并感染患者的HCV-RNA载量比单独感染者的高;36.7%的HCV/HIV合并感染者出现抗HCV阴性,13.3%单独感染HCV者出现阴性结果,差异均有统计学意义(P<0.05);而两组患者的ALB、TBIL及血清球蛋白相比,差异无统计学意义(P>0.05)。结论HCV/HIV合并感染的病毒载量增多,抗体阴性率升高,CD4+、CD8+T细胞比例降低,免疫抑制加重,免疫功能更加低下,肝脏的损伤严重,肝病的发展进程也更快。
Objective To study the changes of liver function and immunity in HCV/HIV co-infected patients and to explore the mechanism of co-infection and mono-infection.Methods Liver function was measured by biochemical analyzer.CD4~+and CD8~+T cells were determined by flow cytometry.Real-time fluorescence quantitative PCR was used to detect HCV-RNA.The antibodies against HCV was detected by ELISA.Results ALT and AST in HCV/HIV co-infected patients were significantly higher than those in HCV mono-infected patients.The proportion of CD4~+and CD8~+T cells in patients with HCV/HIV co-infection was significantly lower than that in patients with HCV mono-infection.Compared with patients with mono-infection,those with HCV/HIV co-infection had higher HCV-RNA load.Anti-HCV negative results were found in 36.7%of HCV/HIV co-infected patients,while 13.3%HCV mono-infected patients had negative results.The differences were statistically significant(P<0.05).There was no statistically significant difference in ALB,TBIL and serum globulin between the two groups(P>0.05).Conclusion There was an increase in viral load of HCV/HIV co-infection and the negative rate of antibodies.The proportion of CD4~+and CD8~+T cells was reduced.With the aggravation of immunosuppression,the immune function was weakened.The liver damage was severe and the development of liver disease was faster.
