摘要
目的探讨益气活血汤、芪苈强心胶囊治疗左心室射血分数(LVEF)正常的心力衰竭患者的治疗效果。方法选取蚌埠医学院第一附属医院接受诊治的180例LVEF正常的心力衰竭(气虚血瘀证)患者作为研究对象,采用随机数表法分为四组,每组45例。其中,对照组采用基础治疗,芪苈强心胶囊组采用基础治疗+芪苈强心胶囊治疗,益气活血汤组采用基础治疗+益气活血汤治疗,联合组采用基础治疗+芪苈强心胶囊+益气活血汤治疗,四组均治疗12周。比较四组患者治疗前后中医证候积分、临床疗效、心功能变化及不良反应发生情况。结果治疗后四组中医证候积分均下降,且联合组最低,差异有统计学意义(P<0.01);四组总有效率比较差异有统计学意义,且联合组最高,差异有统计学意义(P<0.01);治疗后四组左心室收缩末期内径(LVESD)和左室舒张末期内径(LVEDD)均降低,且联合组最低,心脏指数(CI)最高,差异有统计学意义(P<0.05);四组不良反应发生率比较,差异无统计学意义(P>0.05)。结论在基础治疗的基础上,采用益气活血汤和芪苈强心胶囊辅助治疗LVEF正常的心力衰竭,能够缓解临床症状体征,提高临床疗效,改善心功能,安全性好。
Objective To investigate the therapeutic effect of Yiqi Huoxue decoction and Qili Qiangxin capsule on normal left ventricular ejection fraction(LVEF)patients with heart failure.Methods A total of 180 patients with heart failure of Qi deficiency and blood stasis type were chosen and randomly divided into 4 groups,45 cases in each group.The control group was treated with basic treatment;Qili Qiangxin capsule group was treated with basic treatment and Qili Qiangxin capsule;Yiqi Huoxue decoction group was treated with basic treatment and Yiqi Huoxue decoction;the combined group was treated with basic treatment,Qili Qiangxin capsule,and Yiqi Huoxue decoction.All the groups were treated for 12 weeks.The changes of TCM syndrome score,clinical efficacy,cardiac function and adverse events were observed and compared between the two groups before and after treatment.Results After treatment,TCM syndrome scores of the four groups were all decreased(P<0.05),and the combined group was lower than others(P<0.01).The differences in total effective rate of the four groups were statistically significant(P<0.05),and the combined group was higher than others(P<0.01).After treatment,the left ventricular end-systolic diameter(LVESD)and left ventricular enddiastolic diameter(LVEDD)in the four groups were decreased significantly(P<0.05),and LVESD and LVEDD in the combined group were lower than those in other groups;cardiac indexes(CI)in the combined group was higher than that in other groups(P<0.05).There was no significant difference in the incidence of adverse events among the four groups(P>0.05).Conclusion On the basis of basic treatment for patient with heart failure and normal LVEF,the use of Yiqi Huoxue decoction and Qili Qiangxin capsule can alleviate clinical symptoms,improve clinical efficacy and cardiac function,and it is a safe method.
