摘要
目的了解2015年朝阳区居民的4类主要慢性病的死亡情况,并评价其疾病负担,为朝阳区的疾病防控工作提供数据支持。方法通过计算死亡率、死因顺位、平均潜在减寿年数(average years of life lost,AYLL)、减寿率、潜在工作损失年(working years of potential life lost,WPYLL)、伤残调整寿命年(disability-adjusted life year,DALY)、间接经济负担等来评价朝阳区居民的疾病负担。结果2015年朝阳区前4位死因依次为恶性肿瘤、心脏病、脑血管病和呼吸系统疾病,前4位死因死亡数占总死亡数的82.68%。恶性肿瘤减寿率最高,为19.65%,且男性高于女性。平均工作减寿年数(average working years of life lost,AWYLL)随死因顺位变化递减;女性恶性肿瘤平均工作减寿年数为3.08年/人,明显高于其他疾病。恶性肿瘤对工作年龄段女性的影响明显高于其他3个主要死因,且对于工作年龄段女性的早死危害明显高于男性。恶性肿瘤DALY占总DALY的21.60%,为最高,呼吸系统疾病所致的伤残寿命损失年(years lived with disability,YLD)为9.45个DALY/1000人。间接经济负担为961.93万元,且随年龄增长先升后降。结论恶性肿瘤、心脏病、脑血管病和呼吸系统疾病是影响朝阳居民健康的主要慢性病,应根据其人群分布特点制定相应的防控、干预措施,降低这4类主要慢性病的疾病和经济负担。
Objective To understand the mortality associated with four major chronic diseases among residents in Chaoyang District in 2015,and to evaluate the disease burdens to provide data support for disease prevention and control in Chaoyang District.Methods The disease burden of residents in Chaoyang District was evaluated by calculating mortality,death order,average years of life lost(AYLL),working years of potential life lost(WPYLL),disability adjusted life years(DALY),and indirect economic burden.Results The top four causes of death in Chaoyang District in 2015 were malignant tumors,heart disease,cerebrovascular diseases,and respiratory system diseases(in decreasing order).The top four causes of death accounted for 82.68%of the total number of deaths.The life-saving rate of malignant tumors was the highest(19.65%),with males higher than females.The average years of working life reduction decreased with changes in the cause of death.The average working life reduction years among females with malignant tumor was 3.08 years/person,which was significantly higher than other diseases.Malignant tumors have a significantly higher impact on working-age women than the other three main causes of death,and the harm for premature death of working-age women was significantly higher than men.Malignant tumor-associated DALY accounted for 21.60%of the total DALY,which was the highest,while respiratory disease-associated years lived with disability(YLD)was the highest(9.45 DALY/1000 people).The indirect economic burden of the four chronic diseases was 9619300 yuan,which initially increased,then fell with age.Conclusion Malignant tumors,heart diseases,cerebrovascular diseases,and respiratory diseases are the main chronic diseases that affect the health of Chaoyang residents.Therefore,corresponding prevention and control measures should be formulated according to the population distribution characteristics to reduce the disease and economic burden of these four major chronic diseases.
