摘要
目的分析南京市老年人代谢综合征(metabolic syndrome,MS)患病现状及其影响因素,为采取相应的干预措施提供参考依据。方法2013年3~12月采用整群随机抽样方法抽取南京市某社区1600名≥60岁的老年人作为研究对象,对其进行问卷调查、体格检查和实验室检测,探讨老年人MS患病现状及其影响因素。结果南京市1600名老年人中,MS患者710例,患病率44.38%;MS的表现形式以中心性肥胖+高血糖+高血压+血脂异常(高甘油三酯和或低高密度脂蛋白胆固醇)的组合形式所占比例最高(39.30%)。非条件logistic回归分析结果显示,70~79岁(OR=1.36,95%CI:1.05~1.76)、≥80岁(OR=1.86,95%CI:1.14~3.01)、女性(OR=3.14,95%CI:2.36~4.18)、有高血压家族史(OR=1.48,95%CI:1.15~1.91)、超重(OR=6.32,95%CI:4.91~8.15)、肥胖(OR=11.37,95%CI:7.13~18.14)、静坐时间>5h/d(OR=1.73,95%CI:1.01~2.98)是南京市老年人MS患病的危险因素。结论南京市老年人代谢综合征患病率较高,应积极控制各种危险因素,制定有效的干预措施,降低代谢相关性疾病的患病及其带来的心血管损害。
Objective To understand the prevalence of metabolic syndrome and its influencing factors among the elderly in Nanjing,and to provide references for disease intervention.Methods A total of 1600 older individuals aged 60 years and above in Nanjing were randomly recruited using a cluster sampling method from March to December 2013.A questionnaire survey was conducted,and all participants received physical examination and laboratory test.MS was defined based on the diagnostic criteria recommended by the International Diabetes Federation(IDF).Chi-square test and logistic regression analysis were performed for data statistical analysis.Results Among 1600 participants in Nanjing,710 elderly subjects of MS were defined with the overall prevalence of 44.38%.39.30%subjects with MS also had central obesity,high blood pressure,hyperglycemia and dysplasia(high TG level/low HDL-C level)all together.Unconditional logistic regression analysis showed that age of 70-79,(OR=1.36,95%CI:1.05-1.76),age of 80 and above(OR=1.86,95%CI:1.14-3.01),female(OR=3.14,95%CI:2.36-4.18),family history of hypertension(OR=1.48,95%CI:1.15-1.91),overweight(OR=6.32,95%CI:4.91-8.15),obesity(R=11.37,95%CI:7.13-18.14)and sitting still for over 5 h/day(OR=1.73,95%CI:1.01-2.98)were risk factors for MS in the elderly in Nanjing.Conclusions The prevalence of metabolic syndrome among the elderly in Nanjing is relatively high.Various risk factors should be actively controlled and effective interventions should be implemented to reduce the incidence of metabolic-related diseases and cardiovascular damage.
