摘要
目的探讨健脾益气通络汤对24小时尿蛋白定量低于1.5g的2型糖尿病肾病患者的治疗效果。方法选择2016年3月到2017年1月在我医院肾内科进行治疗的24小时尿蛋白定量低于1.5g的2型糖尿病肾病患者104例,按照随机数字表法分为两个组,对照组和治疗组,每个组52名,对照组患者应用降糖,降压,降脂药物和舒洛地特控制蛋白尿,治疗组患者在对照组治疗方法的基础上再加健脾益气通络汤,对治疗的效果进行分析。结果对照组与治疗组患者在进行治疗后,24小时尿蛋白定量、24小时尿微量白蛋白定量、尿IgG、尿微量白蛋白/肌酐、尿转铁蛋白、尿中性粒细胞明胶酶相关脂质运载蛋白、血肌酐、血清尿素氮、肾小球滤过率的数据均出现显著改善,治疗组的改善程度优于对照组,所有项目的处理因素与时间均存在交互作用(P <0.05),对照组与治疗组的治疗有效率差异具有统计学意义(χ2=4.308,P <0.05),治疗组的治疗效果显著优于对照组(z=6.571,P <0.05),两组患者的消化道症状,糖耐量异常,肝损害等不良反应发生率差异均无统计学意义(P>0.05)。结论使用健脾益气通络汤与常规西医方法联合,对24小时尿蛋白定量低于1.5g的2型糖尿病肾病患者进行治疗,可以显著改善患者的各项生理指标,提高治疗的效果,并未使不良反应的概率增加,是值得推广的一种治疗方法。
Objective To investigate the therapeutic effect of Jianpi Yiqi Tongluo Decoction on 24-hour urinary protein quantification less than 1.5 g in type 2 diabetic nephropathy.Methods Between March 2016 and January2017,104 patients with type 2 diabetic nephropathy were treated with 24-hour urine protein quantification less than 1.5 g in my hospital for nephrology,according to the random number method is divided into two groups,the control group and the treatment group,each group of 52 patients,In the control group,patients with hypoglycemic,antihypertensive,lipid-lowering drugs and sulindilide were used to control proteinuria,treatment group of patients in the control group on the basis of treatment plus Jianpi Yiqi Tongluo Decoction,the effect of treatment were compared.Results In the control and treatment groups,24 hours after urinary protein quantification,24-hour urine albumin quantification,urinary IgG,urinary albumin/creatinine,urinary transferrin,urinary neutrophil gelatinrelated lipids The data of transport protein,serum creatinine,serum urea nitrogen and glomerular filtration rate were significantly improved,The improvement of the treatment group was better than that of the control group.The treatment factors and the time had the interaction(P <0.05),the difference of treatment efficiency between the control group and the treatment group was statistically significant(χ2= 4.308,P <0.05).The therapeutic effect of the treatment group was significantly better than that of the control group,and the difference was statistically significant(z =6.571,P <0.05).There were no statistically significant differences in the incidence of adverse events such as digestive tract symptoms,impaired glucose tolerance,and liver damage in both groups(P >0.05).Conclusion The combined use of Jianpi Yiqi Tongluo Decoction and conventional western medicine,patients with type 2 diabetic nephropathy for 24 hours urinary protein under 1.5 g treatment,can significantly improve the patient’s physiological indexes,improve the treatment effect,did not increase the probability of adverse reactions increase,is a method worthy of popularizing.
