Objective:To understand the current situation of stigma among elderly hemodialysis patients and to analyze its influencing factors.Methods:Convenience sampling was used to select 242 elderly patients undergoing hemodi...Objective:To understand the current situation of stigma among elderly hemodialysis patients and to analyze its influencing factors.Methods:Convenience sampling was used to select 242 elderly patients undergoing hemodialysis in one of the two tertiary hospitals in Mianyang City.The survey was carried out from December 2022 to March 2023,with the Basic Information Questionnaire,the Social Influence Scale,and the Collaborative Social Support Scale.All influencing factors were analyzed by correlation analysis,univariate analysis,and multiple linear regression.Results:The average stigma score among elderly hemodialysis patients was 72.94±9.58,and the influencing factors of stigma among elderly hemodialysis patients were determined to be social support and economic status(P<0.05).Conclusion:The occurrence of stigma among elderly hemodialysis patients was at a moderate to high level,and the influencing factors were mainly economic status and social support.To reduce the occurrence of stigma in elderly hemodialysis patients,appropriate economic support from families and social support from medical personnel are needed.展开更多
Objective To investigate variation in levels of transforming growth factor beta 1(TGF-β1)before and after radiotherapy in patients with esophageal cancer in order to evaluate the predictive value of TGF-β1 for the e...Objective To investigate variation in levels of transforming growth factor beta 1(TGF-β1)before and after radiotherapy in patients with esophageal cancer in order to evaluate the predictive value of TGF-β1 for the effects of radiotherapy Methods A total of 140 patients with esophageal squamous carcinoma undergoing radical radiation therapy in the Department of Oncology from March 2015 to December 2017 were enrolled.The patients were divided into the effective(115 cases)and ineffective(25 cases)groups according to World Health Organization(WHO)criteria for the evaluation of solid tumors(2009 RECIST standard).TGF-β1 levels were measured in all patients by using enzyme-linked immunosorbent assay(ELISA).Multiple-factor analysis of the predictive value of the treatment efficacy was performed by Cox regression analysis.Results After radiotherapy,36,79,and 25 cases experienced complete response(CR),partial response(PR),and no response(NR),respectively,with a total effective rate of 82.14%.The TGF-β1 level was significantly lower in the effective group than that in the ineffective group(P<0.05)and covariance analysis revealed significantly reduced TGF-β1 level in esophageal cancer patients following radiotherapy.The multi-factor Cox regression model revealed that the predictive value of TGF-β1 for the effect of radiotherapy was largest,with a hazard ratio[HR]of 1.955(P=0.002),followed by exposure dose,with(HR=1.367;P=0.035).Conclusion Serum TGF-β1 level can serve as a predictor for the short-term effects of radiotherapy in patients with esophageal cancer.展开更多

Methods: Thirty-seven elderly patients (age ranged from 60 to 75 years) with the stages of II and III esophageal cancer underwent 2 cycles chemotherapy preoperatively with single-drug regimen (docetaxel, 35 mg/m2 once a week, on days 1, 8 and 15, at interval of 2 weeks for one cycle). Surgery were performed three weeks later. Blood samples were drawn separately on the day of admission, 1 day before operation, 7 day and 1 month after operation, and we conducted the Flow Cytometry to detect the levels of CD3+, CD4+, CD8+,CD4+/CD8+ and NK cells. Results: There were no significant differences in the levels of CD3+, CD4+, CD8+, CD4+/CD8+ and NK cells between before and after chemotherapy (P > 0.05). On day 7 after operation, the levels of CD3+, CD4+, CD4+/CD8+ and NK cells were degraded and CD8+ increased significantly (P < 0.05). One month after operation, the levels of CD3, CD4+, CD4+/CD8 and NK cells were higher than normal, and CD8 was depressed significantly (P < 0.05). Conclusion: Neoadjuvant chemotherapy has no significant impact on cellular immune function in elderly patients with the stages of II and III esophageal cancer, it is an effective and safe treatment.展开更多
Background and Objectives:Microneedling has been introduced as a new technique to address the growing concern of facial skin photoaging.Kangfuxin liquid has been found to promote the process of skin wound repair,inclu...Background and Objectives:Microneedling has been introduced as a new technique to address the growing concern of facial skin photoaging.Kangfuxin liquid has been found to promote the process of skin wound repair,including reducing inflammatory response,improving immunity and enhancing antioxidant levels.In this prospective randomized double-blinded study,we wanted to explore the clinical efficacy and safety of Kangfuxin liquid combined with microneedling in treating facial skin photoaging.Methods:57 patients with facial skin photoaging were randomly divided into two groups.The treatment group(28 cases)received microneedle therapy with Kangfuxin liquid,while the control group(29 cases)received microneedle therapy with physiological saline.The treatment interval was 4 weeks,and a total of 3 treatments were performed.Compare the VISIA scores of facial photoaging features such as wrinkles,texture,pores,spots and ultraviolet pigmentation between two groups of patients before and after treatment,Global Score for Photoaging,satisfaction evaluation,and record the occurrence of adverse reactions.Results:After treatment,the treatment group showed more significant improvement in wrinkles,texture,pores,spots and ultraviolet pigmentation,and the Global Score for Photoaging was better than the control group(P<0.05).The satisfaction rate with improving skin in the treatment group was 85.71%,which was higher than 75.86%in the control group(P<0.05).Both groups did not experience adverse reactions such as skin infection,pigmentation or hypopigmentation,scar formation,or worsening of melasma.Conclusion:Kangfuxin liquid combined with microneedling therapy has a good improvement effect on facial skin photoaging,with a low incidence of adverse reactions and high patient satisfaction.It is worthy of clinical promotion.展开更多

(TSRP) on focal cerebral ischemia and reperfusion injury in rats. Focal cerebral ischemia and reperfusion injury was performed in rats using the suture method. The results indicate that intragastric injection of TSRP, at 5, 10 and 20 mg/kg, could decrease neurological impairment, reduce cerebral infarct volume, diminish pathological changes, and significantly inhibit the apoptosis of neurons surrounding the ischemic area. In addition, TSRP upregulated the expression of the anti-apoptotic factor Bcl-2, at the protein and mRNA levels, and it downregulated the expression of the pro-apoptotic factor Bax, at the protein and mRNA levels. These findings indicate that TSRP protects against cerebral ischemia/reperfusion injury, and that it may do so by regulating the expression of Bcl-2 and Bax.展开更多

However, our knowledge of these genetic risk factors i...Genetic factors play a critical role in autoimmune hepatitis (AIH), and numerous studies have been conducted to identify variants associated with the risk of AIH. However, our knowledge of these genetic risk factors is still limited. In this study, we aim to provide a comprehensive synopsis of the genetic architecture of this disease. A systematic search was conducted to identify published studies on the associations between genetic variants and the risk of AIH. Meta-analyses were conducted to calculate the pooled odds ratio (OR) and 95% confidence interval (CI). Then, the cumulative evidence was evaluated for significant associations according to the Venice criteria and false-positive report probability. Finally, functional annotations and pathway analyses were conducted to identify potential pathogenic loci and related pathways. In total, 62 studies involving 11,068 cases and 45,482 controls were included to assess the association between 75 genetic variants and the risk of AIH. Among them, 24 variants were associated with the risk of AIH, and there is strong cumulative evidence supporting these associations. Importantly, HLA DRB1*0301 (OR: 3.023, 95% CI: 2.443 - 1.678, P = 2.81 × 10?24) and DRB3*0101 (OR: 3.667, 95% CI: 2.649 - 5.075, P = 4.69 × 10?15) are newly identified genome-wide significant risk loci. In addition, the rs3184504 variant (OR: 1.305, 95% CI: 1.122 - 1.516, P = 0.001) in the SH2B3 gene is a potential functional mutation. GO pathway analysis suggests that these genes are enriched in antigen processing and presentation, response to interferon-gamma, and immune response-regulating signaling pathways. This study comprehensively summarizes the genetic architecture of AIH and provides cumulative evidence. We have identified two new loci that exceed genome-wide significance. The findings from this study will offer new insights into the pathogenesis of AIH.展开更多
BACKGROUND Digestive cancer has traditionally been thought of as a disease that mainly occurs in elderly individuals,and it has been ignored in young adults by both patients and physicians.AIM To describe the worldwid...BACKGROUND Digestive cancer has traditionally been thought of as a disease that mainly occurs in elderly individuals,and it has been ignored in young adults by both patients and physicians.AIM To describe the worldwide profile of digestive cancer incidence,mortality and corresponding trends among 20–39-year-olds,with major patterns highlighted by age,sex,development level,and geographical region.METHODS I performed a population-based study to quantify the burden of young adult digestive cancers worldwide.Global,regional,sex,and country-specific data estimates of the number of new cancer cases and cancer-associated deaths that occurred in 2020 were extracted from the GLOBOCAN Cancer Today database.To assess long-term trends in young adult digestive cancer,cancer incidence data and mortality data were obtained from the Cancer in Five Continents Plus database and the World Health Organization mortality database,respectively.The associations between the human development index(HDI)and digestive cancer burden in young adults were evaluated by linear regression analyses.RESULTS In 2020,there were an estimated 19292789 new cancer cases,resulting in 9958133 deaths worldwide,which equated to an age-standardized incidence rate(ASIR)of 5.16 and age-standardized mortality rate(ASMR)of 3.04,accounting for 12.24%of all new cancer cases and 25.26%of all cancer deaths occurring in young adults.The burden was disproportionally greater among males,with male:female ratios of 1.34 for incidence and 1.58 for mortality.The ASIRs were 2.1,1.4,and 1.0 per 100000 people per year,whereas the ASMRs were 0.83,1.1,and 0.62 per 100000 people per year for colorectal,liver,and gastric cancer,respectively.When assessed by geographical region and HDI levels,the cancer profile varied substantially,and a strong positive correlation between the mortality-to-incidence ratio of digestive cancer and HDI ranking was found(R^(2)=0.7388,P<0.001).CONCLUSION The most common digestive cancer types are colorectal,liver and gastric cancer.The global digestive cancer burden among young adults is greater among males and exhibits a positive association with socioeconomic status.The digestive cancer burden is heavy in young adults,reinforcing the need for primary and secondary prevention strategies.展开更多

BACKGROUND Ischemic stroke is an entity with high incidence,morbidity,and mortality rates.Carotid artery stenosis is an important and independent risk factor for ischemic stroke.The three current approaches for treati...BACKGROUND Ischemic stroke is an entity with high incidence,morbidity,and mortality rates.Carotid artery stenosis is an important and independent risk factor for ischemic stroke.The three current approaches for treating carotid artery stenosis are drug treatment,carotid endarterectomy(CEA),carotid angioplasty and stenting(CAS).The approach is chosen based on the degree of stenosis.CEA or CAS could have been chosen for the current patient,who had severe carotid stenosis and an iodinated contrast allergy.After thoroughly communicating with the patient,the patient chose CAS for treatment.Therefore,we performed ultrasound-guided CAS to avoid the use of iodinated contrast.CASE SUMMARY The main symptoms of the patient were numbness and weakness of the left limb.Computed tomography angiography of the head and neck at another hospital indicated multiple sites of stenosis in the arteries of the head and neck.The patient requested CAS for treatment but was allergic to iodinated contrast media.Thus,routine digital subtraction angiography(DSA)with iodinated contrast could not be used for the procedure.The diagnosis of this patient was as follows:(1)Right parietal lobe cerebral infarction;(2)multiple sites of stenosis in the arteries of the head and neck(severe stenosis of the right internal carotid artery,severe stenosis of the right subclavian artery);(3)right subclavian steal syndrome;and(4)hypertension(stage 3,high risk).The interventions included routine treatment for cerebral infarction,oral administration of clopidogrel(75 mg qd)and aspirin(100 mg qd),ultrasound-guided CAS,and postoperative follow-up.Postoperative color Doppler ultrasound and cerebrovascular magnetic resonance angiography of the carotid artery showed good vascular recovery,and the postoperative follow-up indicated a good prognosis.CONCLUSION This case study suggests that ultrasound-guided endovascular treatment is a potential option for patients with contraindications to the iodinated contrast agents used in DSA-guided surgery,although excellent surgical operating skills are needed.展开更多

Methods: Four hundred and eighty cases of NSCLC were retrospectively analyzed from January 2003 to January 2010, and the prevalence of hepatic metastasis of NSCLC in patients with and without hepatitis B virus infection were compared. Results: In the HBV carriers' group, the prevalence of synchronous hepatic metastasis and metachronous hepatic metastasis were 13.2% and 5.9%, respectively. Meanwhile in the non-HBV group, those were 21.6% and 9.5% respectively. A significant difference between the two groups was found(P < 0.05). Conclusion: The prevalence of synchronous hepatic metastasis and metachronous hepatic metastasis in non-small cell lung cancer with HBV infection are lower than those in non-HBV infection group. Hepatic metastasis is infrequent in HBV infected cases of NSCLC.展开更多

Methods: By adopting a design of multi-centered randomized controlled trial (RCT), a total of 270 patients with anxiety were randomized into a treatment group and a medication group. The treatment group was intervened by music electric stimulation of points, while the medication group was intervened by oral administration of doxepin. The two groups were evaluated by using Hamilton anxiety rating scale (HAMA) and Chinese revised edition of self-rating anxiety scale (SAS-CR) before and after the intervention. The therapeutic efficacies were also compared. Results: The total effective rate was 93.6% in the treatment group versus 92.3% in the medication group, and the between-group difference was statistically insignificant (P〉O.05). After the treatment, the aggregate scores of HAMA and SAS-CR were significantly changed in both groups (both P〈0.001), and the inter-group differences were statistically insignificant (P〉O.05). Conclusion: Music electric stimulation of points can produce equivalent efficacy in treating anxiety compared to doxepin. Thus, it can be taken as a choice in the treatment of anxiety.展开更多
Objective:To investigate the radio-sensitizing effect of salicylic acid(SA)on human cervical cancer cells and its potential molecular mechanism.Methods:Cervical cancer cells were treated with SA and ionizing radiation...Objective:To investigate the radio-sensitizing effect of salicylic acid(SA)on human cervical cancer cells and its potential molecular mechanism.Methods:Cervical cancer cells were treated with SA and ionizing radiation.The expression ofγ-H2AX was evaluated by immunofluorescence(IF)assay.Cell cycle and apoptosis were analyzed by flow cytometry.Western blot was performed to detect the protein level of AMPK/TSC2/mTOR pathway.Results:SA inhibited basal proliferation of cervical cancer cells in a dose and time dependent manner.In addition,SA increased radiation-induced DNA damage,promoted apoptosis,triggered a redistribution of cell cycle from G2-M phase to G1-S phase of cervical cancer cells,and hence increased cell sensitivity to radiation.Moreover,SA treatment elevated the expression levels of p-AMPKα(t=3.996,P<0.05)and p-TSC2(t=5.308,P<0.05),whereas the level of p-mTOR(t=10.160,P<0.05)was significantly decreased.Conclusion:SA enhances the radiosensitivity of cervical cancer cells by targeting AMPK/TSC2/mTOR signaling pathway,and might serve as a promising therapeutic strategy to improve the efficacy of radiotherapy for cervical cancer.展开更多
文摘Objective:To understand the current situation of stigma among elderly hemodialysis patients and to analyze its influencing factors.Methods:Convenience sampling was used to select 242 elderly patients undergoing hemodialysis in one of the two tertiary hospitals in Mianyang City.The survey was carried out from December 2022 to March 2023,with the Basic Information Questionnaire,the Social Influence Scale,and the Collaborative Social Support Scale.All influencing factors were analyzed by correlation analysis,univariate analysis,and multiple linear regression.Results:The average stigma score among elderly hemodialysis patients was 72.94±9.58,and the influencing factors of stigma among elderly hemodialysis patients were determined to be social support and economic status(P<0.05).Conclusion:The occurrence of stigma among elderly hemodialysis patients was at a moderate to high level,and the influencing factors were mainly economic status and social support.To reduce the occurrence of stigma in elderly hemodialysis patients,appropriate economic support from families and social support from medical personnel are needed.
文摘Objective To investigate variation in levels of transforming growth factor beta 1(TGF-β1)before and after radiotherapy in patients with esophageal cancer in order to evaluate the predictive value of TGF-β1 for the effects of radiotherapy Methods A total of 140 patients with esophageal squamous carcinoma undergoing radical radiation therapy in the Department of Oncology from March 2015 to December 2017 were enrolled.The patients were divided into the effective(115 cases)and ineffective(25 cases)groups according to World Health Organization(WHO)criteria for the evaluation of solid tumors(2009 RECIST standard).TGF-β1 levels were measured in all patients by using enzyme-linked immunosorbent assay(ELISA).Multiple-factor analysis of the predictive value of the treatment efficacy was performed by Cox regression analysis.Results After radiotherapy,36,79,and 25 cases experienced complete response(CR),partial response(PR),and no response(NR),respectively,with a total effective rate of 82.14%.The TGF-β1 level was significantly lower in the effective group than that in the ineffective group(P<0.05)and covariance analysis revealed significantly reduced TGF-β1 level in esophageal cancer patients following radiotherapy.The multi-factor Cox regression model revealed that the predictive value of TGF-β1 for the effect of radiotherapy was largest,with a hazard ratio[HR]of 1.955(P=0.002),followed by exposure dose,with(HR=1.367;P=0.035).Conclusion Serum TGF-β1 level can serve as a predictor for the short-term effects of radiotherapy in patients with esophageal cancer.

文摘Although the incidence and mortality of gastric cancer(GC)have been decreasing steadily worldwide,especially in East Asia,the disease burden of this malignancy is still very heavy.Except for tremendous progress in the management of GC by multidisciplinary treatment,surgical excision of the primary tumor is still the cornerstone intervention in the curative-intent treatment of GC.During the relatively short perioperative period,patients undergoing radical gastrectomy will suffer from at least part of the following perioperative events:Surgery,anesthesia,pain,intraoperative blood loss,allogeneic blood transfusion,postoperative complications,and their related anxiety,depression and stress response,which have been shown to affect long-term outcomes.Therefore,in recent years,studies have been carried out to find and test interventions during the perioperative period to improve the long-term survival of patients following radical gastrectomy,which will be the aim of this review.
文摘Objective: We aimed to study the influence of neoadjuvant chemotherapy on immunity function in elderly patients with the stages of II and III esophageal cancer. Methods: Thirty-seven elderly patients (age ranged from 60 to 75 years) with the stages of II and III esophageal cancer underwent 2 cycles chemotherapy preoperatively with single-drug regimen (docetaxel, 35 mg/m2 once a week, on days 1, 8 and 15, at interval of 2 weeks for one cycle). Surgery were performed three weeks later. Blood samples were drawn separately on the day of admission, 1 day before operation, 7 day and 1 month after operation, and we conducted the Flow Cytometry to detect the levels of CD3+, CD4+, CD8+,CD4+/CD8+ and NK cells. Results: There were no significant differences in the levels of CD3+, CD4+, CD8+, CD4+/CD8+ and NK cells between before and after chemotherapy (P > 0.05). On day 7 after operation, the levels of CD3+, CD4+, CD4+/CD8+ and NK cells were degraded and CD8+ increased significantly (P < 0.05). One month after operation, the levels of CD3, CD4+, CD4+/CD8 and NK cells were higher than normal, and CD8 was depressed significantly (P < 0.05). Conclusion: Neoadjuvant chemotherapy has no significant impact on cellular immune function in elderly patients with the stages of II and III esophageal cancer, it is an effective and safe treatment.
文摘Background and Objectives:Microneedling has been introduced as a new technique to address the growing concern of facial skin photoaging.Kangfuxin liquid has been found to promote the process of skin wound repair,including reducing inflammatory response,improving immunity and enhancing antioxidant levels.In this prospective randomized double-blinded study,we wanted to explore the clinical efficacy and safety of Kangfuxin liquid combined with microneedling in treating facial skin photoaging.Methods:57 patients with facial skin photoaging were randomly divided into two groups.The treatment group(28 cases)received microneedle therapy with Kangfuxin liquid,while the control group(29 cases)received microneedle therapy with physiological saline.The treatment interval was 4 weeks,and a total of 3 treatments were performed.Compare the VISIA scores of facial photoaging features such as wrinkles,texture,pores,spots and ultraviolet pigmentation between two groups of patients before and after treatment,Global Score for Photoaging,satisfaction evaluation,and record the occurrence of adverse reactions.Results:After treatment,the treatment group showed more significant improvement in wrinkles,texture,pores,spots and ultraviolet pigmentation,and the Global Score for Photoaging was better than the control group(P<0.05).The satisfaction rate with improving skin in the treatment group was 85.71%,which was higher than 75.86%in the control group(P<0.05).Both groups did not experience adverse reactions such as skin infection,pigmentation or hypopigmentation,scar formation,or worsening of melasma.Conclusion:Kangfuxin liquid combined with microneedling therapy has a good improvement effect on facial skin photoaging,with a low incidence of adverse reactions and high patient satisfaction.It is worthy of clinical promotion.
文摘Objective:To investigate the epidemiological characteristics and genotypes of human papillomavirus(HPV)infection in Mianyang.Methods:The cervical samples of 27,040 patients,who visited the Department of Gynecology of The Third Hospital of Mianyang from January 2018 to January 2020,were collected.Results:The HPV-positive infection rate was 21.40%(5,787/27,040);the single HPV infection rate was 72.04%(4,169/5,787);the double HPV infection rate was 19.73%(1,142/5,787);the triple and above HPV infection rate was 8.22%(476/5,787);the top five high-risk HPV subtypes with the highest infection rates were HPV52,16,58,53,and 51,while the top five low-risk HPV subtypes with the highest infection rates were HPV81,42,43,6,and 11;there were significant differences in the HPV-positive infection rate,high-risk infection rate,low-risk infection rate,and multiple infection rate among different age groups(P<0.05).Conclusion:The HPV infection rate in Mianyang was 21.4%,in which the majority of the cases were single infection;the high-risk HPV subtypes were HPV52,16,58,53,and 51;the HPV-positive infection rate,high-risk infection rate,low-risk infection rate,and multiple infection rate were high in the middle but low at both ends in the context of age distribution;the top three age groups with the highest infection rates were 45-49,40-44,and 30-34.
文摘Multiple lentigines syndrome is an autosomal dominant genetic disease,and its expressivity and penetrance are variable.It is also known as LEOPARD syndrome(LS).The genes known to be associated with LS include PTPN11,RAF1 and BRAF.The diagnosis of LS(OMIM 151100)is based on the observation of key features in the clinical background.LS caused by a germline PTPN11 mutation are characterized as multisystemic anomalies and variable marked phenotypes such as multiple lentigines and cafénoir spots,electrocardiographic conduction abnormalities,ocular hypertelorism/obstructive cardiomyopathy,pulmonary stenosis,abnormal genitalia,retardation of growth,and deafness.Phenotype overlap complicates clinical discrimination within RASopathies,making the diagnosis of LS more confusing and challenging.Besides,LS patients do not usually present with all these typical clinical features,increasing the possibility of underdiagnosis or misdiagnosis.Herein,we report a case of a 41-year-old male presenting with multiple dark pigmented macules all over the body,thoracic deformity and family history.And we followed up the patients.

文摘This study examines the neuroprotective effects and mechanisms of action of total saponins from Rubus parvifolius L. (TSRP) on focal cerebral ischemia and reperfusion injury in rats. Focal cerebral ischemia and reperfusion injury was performed in rats using the suture method. The results indicate that intragastric injection of TSRP, at 5, 10 and 20 mg/kg, could decrease neurological impairment, reduce cerebral infarct volume, diminish pathological changes, and significantly inhibit the apoptosis of neurons surrounding the ischemic area. In addition, TSRP upregulated the expression of the anti-apoptotic factor Bcl-2, at the protein and mRNA levels, and it downregulated the expression of the pro-apoptotic factor Bax, at the protein and mRNA levels. These findings indicate that TSRP protects against cerebral ischemia/reperfusion injury, and that it may do so by regulating the expression of Bcl-2 and Bax.
文摘Obesity,the global pandemic since industrialization,is the number one lifestylerelated risk factor for premature death,which increases the incidence and mortality of various diseases and conditions,including cancer.In recent years,the theory of cancer stem cells(CSCs),which have the capacity for self-renewal,metastasis and treatment resistance,has been bolstered by increasing evidence.However,research on how obesity affects CSCs to facilitate cancer initiation,progression and therapy resistance is still in its infancy,although evidence has already begun to accumulate.Regarding the ever-increasing burden of obesity and obesity-related cancer,it is pertinent to summarize evidence about the effects of obesity on CSCs,as elucidating these effects will contribute to the improvement in the management of obesity-related cancers.In this review,we discuss the association between obesity and CSCs,with a particular focus on how obesity promotes cancer initiation,progression and therapy resistance through CSCs and the mechanisms underlying these effects.In addition,the prospect of preventing cancer and targeting the mechanisms linking obesity and CSCs to reduce cancer risk or to improve the survival of patients with cancer is considered.

文摘Intervertebral disc degeneration(IDD)is the main cause of lower back pain.Lower back pain places a huge burden on society,and all current treatments for IDD cannot restore the original function of the intervertebral disc.Proposing new treatments for IDD requires clarifying the mechanisms of IDD.Physiological hypoxia is an important feature of the nucleus pulposus because of its special anatomical structure.Using RNA sequencing(RNA-seq),we obtained the whole transcriptome of nucleus pulposus cells(NPCs)under hypoxia and hypoxia-inducible factor(HIF1A)deletion.Results demonstrated possible effects of oxygen concentration and HIF1A on NPCs.In addition,our results showed that hypoxia can affect lipid metabolism in NPCs.
文摘Genetic factors play a critical role in autoimmune hepatitis (AIH), and numerous studies have been conducted to identify variants associated with the risk of AIH. However, our knowledge of these genetic risk factors is still limited. In this study, we aim to provide a comprehensive synopsis of the genetic architecture of this disease. A systematic search was conducted to identify published studies on the associations between genetic variants and the risk of AIH. Meta-analyses were conducted to calculate the pooled odds ratio (OR) and 95% confidence interval (CI). Then, the cumulative evidence was evaluated for significant associations according to the Venice criteria and false-positive report probability. Finally, functional annotations and pathway analyses were conducted to identify potential pathogenic loci and related pathways. In total, 62 studies involving 11,068 cases and 45,482 controls were included to assess the association between 75 genetic variants and the risk of AIH. Among them, 24 variants were associated with the risk of AIH, and there is strong cumulative evidence supporting these associations. Importantly, HLA DRB1*0301 (OR: 3.023, 95% CI: 2.443 - 1.678, P = 2.81 × 10?24) and DRB3*0101 (OR: 3.667, 95% CI: 2.649 - 5.075, P = 4.69 × 10?15) are newly identified genome-wide significant risk loci. In addition, the rs3184504 variant (OR: 1.305, 95% CI: 1.122 - 1.516, P = 0.001) in the SH2B3 gene is a potential functional mutation. GO pathway analysis suggests that these genes are enriched in antigen processing and presentation, response to interferon-gamma, and immune response-regulating signaling pathways. This study comprehensively summarizes the genetic architecture of AIH and provides cumulative evidence. We have identified two new loci that exceed genome-wide significance. The findings from this study will offer new insights into the pathogenesis of AIH.

文摘BACKGROUND Digestive cancer has traditionally been thought of as a disease that mainly occurs in elderly individuals,and it has been ignored in young adults by both patients and physicians.AIM To describe the worldwide profile of digestive cancer incidence,mortality and corresponding trends among 20–39-year-olds,with major patterns highlighted by age,sex,development level,and geographical region.METHODS I performed a population-based study to quantify the burden of young adult digestive cancers worldwide.Global,regional,sex,and country-specific data estimates of the number of new cancer cases and cancer-associated deaths that occurred in 2020 were extracted from the GLOBOCAN Cancer Today database.To assess long-term trends in young adult digestive cancer,cancer incidence data and mortality data were obtained from the Cancer in Five Continents Plus database and the World Health Organization mortality database,respectively.The associations between the human development index(HDI)and digestive cancer burden in young adults were evaluated by linear regression analyses.RESULTS In 2020,there were an estimated 19292789 new cancer cases,resulting in 9958133 deaths worldwide,which equated to an age-standardized incidence rate(ASIR)of 5.16 and age-standardized mortality rate(ASMR)of 3.04,accounting for 12.24%of all new cancer cases and 25.26%of all cancer deaths occurring in young adults.The burden was disproportionally greater among males,with male:female ratios of 1.34 for incidence and 1.58 for mortality.The ASIRs were 2.1,1.4,and 1.0 per 100000 people per year,whereas the ASMRs were 0.83,1.1,and 0.62 per 100000 people per year for colorectal,liver,and gastric cancer,respectively.When assessed by geographical region and HDI levels,the cancer profile varied substantially,and a strong positive correlation between the mortality-to-incidence ratio of digestive cancer and HDI ranking was found(R^(2)=0.7388,P<0.001).CONCLUSION The most common digestive cancer types are colorectal,liver and gastric cancer.The global digestive cancer burden among young adults is greater among males and exhibits a positive association with socioeconomic status.The digestive cancer burden is heavy in young adults,reinforcing the need for primary and secondary prevention strategies.
文摘Acute appendicitis(AA)is the most common acute abdomen,and appendectomy is the most common nonelective surgery performed worldwide.Despite the long history of understanding this disease and enhancements to medical care,many challenges remain in the diagnosis and treatment of AA.One of these challenges is the timing of appendectomy.In recent decades,extensive studies focused on this topic have been conducted,but there have been no conclusive answers.From the onset of symptoms to appendectomy,many factors can cause delay in the surgical intervention.Some are inevitable,and some can be modified and improved.The favorable and unfavorable results of these factors vary according to different situations.The purpose of this review is to discuss the causes of appendectomy delay and its risk-related costs.This review also explores strategies to balance the positive and negative effects of delayed appendectomy.

文摘The tumor,nodes,metastasis(TNM)staging system has long been the gold standard for the classification and prognosis of solid tumors.However,the TNM staging system is not without limitations.Prognostic heterogeneity exists within patients at the same stage.Therefore,the pursuit of other biomarkers with the potential to classify patients with cancer has never stopped.One of them,tumor budding(TB),has gained much success in colorectal cancer.In recent years,TB in gastric cancer has attracted much attention from researchers,beginning to reveal the molecular and biological aspects of this phenomenon in gastric cancer,and has emerged as a promising prognostic biomarker in gastric cancer,predicting disease progression and unfavorable survival.Therefore,it is time and essential to provide a holistic overview of TB in gastric cancer,which has not been achieved and is the aim of this review.

文摘BACKGROUND Ischemic stroke is an entity with high incidence,morbidity,and mortality rates.Carotid artery stenosis is an important and independent risk factor for ischemic stroke.The three current approaches for treating carotid artery stenosis are drug treatment,carotid endarterectomy(CEA),carotid angioplasty and stenting(CAS).The approach is chosen based on the degree of stenosis.CEA or CAS could have been chosen for the current patient,who had severe carotid stenosis and an iodinated contrast allergy.After thoroughly communicating with the patient,the patient chose CAS for treatment.Therefore,we performed ultrasound-guided CAS to avoid the use of iodinated contrast.CASE SUMMARY The main symptoms of the patient were numbness and weakness of the left limb.Computed tomography angiography of the head and neck at another hospital indicated multiple sites of stenosis in the arteries of the head and neck.The patient requested CAS for treatment but was allergic to iodinated contrast media.Thus,routine digital subtraction angiography(DSA)with iodinated contrast could not be used for the procedure.The diagnosis of this patient was as follows:(1)Right parietal lobe cerebral infarction;(2)multiple sites of stenosis in the arteries of the head and neck(severe stenosis of the right internal carotid artery,severe stenosis of the right subclavian artery);(3)right subclavian steal syndrome;and(4)hypertension(stage 3,high risk).The interventions included routine treatment for cerebral infarction,oral administration of clopidogrel(75 mg qd)and aspirin(100 mg qd),ultrasound-guided CAS,and postoperative follow-up.Postoperative color Doppler ultrasound and cerebrovascular magnetic resonance angiography of the carotid artery showed good vascular recovery,and the postoperative follow-up indicated a good prognosis.CONCLUSION This case study suggests that ultrasound-guided endovascular treatment is a potential option for patients with contraindications to the iodinated contrast agents used in DSA-guided surgery,although excellent surgical operating skills are needed.
文摘Objective: The purpose of the study was to explore the relationship between hepatitis B virus(HBV) infection and hepatic metastasis in non-small cell lung cancer(NSCLC). Methods: Four hundred and eighty cases of NSCLC were retrospectively analyzed from January 2003 to January 2010, and the prevalence of hepatic metastasis of NSCLC in patients with and without hepatitis B virus infection were compared. Results: In the HBV carriers' group, the prevalence of synchronous hepatic metastasis and metachronous hepatic metastasis were 13.2% and 5.9%, respectively. Meanwhile in the non-HBV group, those were 21.6% and 9.5% respectively. A significant difference between the two groups was found(P < 0.05). Conclusion: The prevalence of synchronous hepatic metastasis and metachronous hepatic metastasis in non-small cell lung cancer with HBV infection are lower than those in non-HBV infection group. Hepatic metastasis is infrequent in HBV infected cases of NSCLC.

文摘To observe the therapeutic efficacy of music electric stimulation of points in treating anxiety. Methods: By adopting a design of multi-centered randomized controlled trial (RCT), a total of 270 patients with anxiety were randomized into a treatment group and a medication group. The treatment group was intervened by music electric stimulation of points, while the medication group was intervened by oral administration of doxepin. The two groups were evaluated by using Hamilton anxiety rating scale (HAMA) and Chinese revised edition of self-rating anxiety scale (SAS-CR) before and after the intervention. The therapeutic efficacies were also compared. Results: The total effective rate was 93.6% in the treatment group versus 92.3% in the medication group, and the between-group difference was statistically insignificant (P〉O.05). After the treatment, the aggregate scores of HAMA and SAS-CR were significantly changed in both groups (both P〈0.001), and the inter-group differences were statistically insignificant (P〉O.05). Conclusion: Music electric stimulation of points can produce equivalent efficacy in treating anxiety compared to doxepin. Thus, it can be taken as a choice in the treatment of anxiety.
