Cardiac arrest can lead to severe neurological impairment as a result of inflammation,mitochondrial dysfunction,and post-cardiopulmonary resuscitation neurological damage.Hypoxic preconditioning has been shown to impr...Cardiac arrest can lead to severe neurological impairment as a result of inflammation,mitochondrial dysfunction,and post-cardiopulmonary resuscitation neurological damage.Hypoxic preconditioning has been shown to improve migration and survival of bone marrow–derived mesenchymal stem cells and reduce pyroptosis after cardiac arrest,but the specific mechanisms by which hypoxia-preconditioned bone marrow–derived mesenchymal stem cells protect against brain injury after cardiac arrest are unknown.To this end,we established an in vitro co-culture model of bone marrow–derived mesenchymal stem cells and oxygen–glucose deprived primary neurons and found that hypoxic preconditioning enhanced the protective effect of bone marrow stromal stem cells against neuronal pyroptosis,possibly through inhibition of the MAPK and nuclear factor κB pathways.Subsequently,we transplanted hypoxia-preconditioned bone marrow–derived mesenchymal stem cells into the lateral ventricle after the return of spontaneous circulation in an 8-minute cardiac arrest rat model induced by asphyxia.The results showed that hypoxia-preconditioned bone marrow–derived mesenchymal stem cells significantly reduced cardiac arrest–induced neuronal pyroptosis,oxidative stress,and mitochondrial damage,whereas knockdown of the liver isoform of phosphofructokinase in bone marrow–derived mesenchymal stem cells inhibited these effects.To conclude,hypoxia-preconditioned bone marrow–derived mesenchymal stem cells offer a promising therapeutic approach for neuronal injury following cardiac arrest,and their beneficial effects are potentially associated with increased expression of the liver isoform of phosphofructokinase following hypoxic preconditioning.展开更多
BACKGROUND Circulating tumor cells(CTCs)are crucial for improving our knowledge regarding tumor progress,prognosis,and recurrence possibility.AIM To evaluate the role of CTCs in the early diagnosis and treatment of ga...BACKGROUND Circulating tumor cells(CTCs)are crucial for improving our knowledge regarding tumor progress,prognosis,and recurrence possibility.AIM To evaluate the role of CTCs in the early diagnosis and treatment of gastric cancer.METHODS From June 2020 to December 2021,a randomized study was conducted in our institution involving 80 patients scheduled for surgery for gastric cancer.The patients were divided into two groups:A control group that was tested for traditional serum markers and a study group that was assessed for serum CTCs.RESULTS In the study cohort,CTC levels did not correlate significantly with patient age,gender,or degree of tumor differentiation(P>0.05).However,there was a significant correlation with the tumor-node-metastasis stage of the tumor(P<0.05).In the study group,the CTC diagnostic positivity rate was 62.50%(25 out of 40 patients),while the positivity rate for conventional serum markers in the control group was 47.50%(19 out of 40 patients).The positive detection rate in the study group was significantly higher than that of the control group(P<0.05).CONCLUSION CTCs have slight invasion and high sensitivity and specificity,presenting great value for early clinical diagnosis of recurrence and metastasis.It will improve the deceleration of disease development and increase the survival rate.展开更多

BACKGROUND Colon cancer is one of the most common malignancies worldwide,and chemo-therapy is a widely used strategy in colon cancer clinical therapy.Chemotherapy resistance is the main cause of recurrence and progres...BACKGROUND Colon cancer is one of the most common malignancies worldwide,and chemo-therapy is a widely used strategy in colon cancer clinical therapy.Chemotherapy resistance is the main cause of recurrence and progression in colon cancer.Thus,novel drugs for treatment are urgently needed.Tetramethylpyrazine(TMP),a component of the traditional Chinese medicine Chuanxiong Hort,has been proven to exhibit a beneficial effect in tumors.AIM To investigate the potential anticancer activity of TMP in colon cancer and the underlying mechanisms.METHODS Colon cancer cells were incubated with different concentrations of TMP.Cell viability was evaluated by crystal violet staining assay,and cell apoptosis was assessed by flow cytometry.Apoptosis-associated protein expression was measured using Western blot analysis.Intracellular reactive oxygen species(ROS)levels were assessed by flow cytometry using DCF fluorescence intensity.Xeno-grafts were established by the subcutaneous injection of colon cancer cells into nude mice;tumor growth was monitored and intracellular ROS was detected in tumors by malondialdehyde assay.RESULTS TMP induced apoptosis of colon cancer cells via the activation of the mitochon-drial pathway.TMP increased the generation of intracellular ROS and triggered mitochondria-mediated apoptosis in a caspase-dependent manner.CONCLUSION Our study demonstrates that TMP induces the apoptosis of colon cancer cells and increases the generation of intracellular ROS.TMP triggers mitochondria-mediated apoptosis in a caspase-dependent manner.The accumu-lation of intracellular ROS is involved in TMP-induced apoptosis.Our findings suggest that TMP may be a potential therapeutic drug for the treatment of colon cancer.展开更多

BACKGROUND The upregulation of serpin family B member 5(SERPINB5)has been linked to the progression of rectal cancer.However,the specific roles and underlying mechanisms of SERPINB5 in rectal cancer are not fully unde...BACKGROUND The upregulation of serpin family B member 5(SERPINB5)has been linked to the progression of rectal cancer.However,the specific roles and underlying mechanisms of SERPINB5 in rectal cancer are not fully understood.AIM To investigate the roles and mechanisms of SERPINB5 in rectal cancer.METHODS SERPINB5 protein level in rectal cancer tissues and cell lines was measured through western blot analysis.SW480 cells were transfected with pcDNASERPINB5 or short-hairpin RNA targeting SERPINB5(sh-SERPINB5).Cell proliferation,invasion,and apoptosis were then evaluated.The interaction between SERPINB5 and heat shock protein 90 alpha class A member 1(HSP90AA1)was confirmed through a co-immunoprecipitation assay.Subsequently,pcDNAHSP90AA1 or sh-HSP90AA1 was transfected into SW480 cells,and cell progression was then detected.Moreover,rescue experiments were used to investigate the effect of the SERPINB5/HSP90AA1 axis on rectal cancer progression.Additionally,sh-SERPINB5-transfected SW480 cells were implanted into nude mice,and xenograft tumor growth was then evaluated.RESULTS SERPINB5 was prominently upregulated in rectal cancer tissues and cells.SERPINB5 overexpression increased SW480 cell proliferation and invasion while reducing apoptosis.In contrast,SERPINB5 knockdown had the opposite effects.Moreover,SERPINB5 could interact with HSP90AA1 and promote HSP90AA1 expression in SW480 cells.HSP90AA1 overexpression facilitated SW480 cell proliferation and invasion and restrained apoptosis.By contrast,HSP90AA1 knockdown suppressed cell progression.The upregulation of HSP90AA1 reversed the SERPINB5 silencing-mediated inhibition of SW480 cell progression.Additionally,SERPINB5 knockdown retarded the growth of rectal cancer tumors in vivo.CONCLUSION SERPINB5 knockdown inhibited rectal cancer cell proliferation and invasion and retarded xenograft tumor growth by inhibiting HSP90AA1 expression.展开更多
BACKGROUND The surgical management of rectal cancer is continuously advancing,with a current emphasis on minimising the need for a permanent stoma.Understanding the risk factors influencing sphincter preservation is c...BACKGROUND The surgical management of rectal cancer is continuously advancing,with a current emphasis on minimising the need for a permanent stoma.Understanding the risk factors influencing sphincter preservation is crucial for guiding clinical decision-making and optimising preoperative patient evaluation.AIM To examine the risk factors influencing sphincter preservation in laparoscopic radical rectal cancer surgery.METHODS A retrospective analysis of the demographics,preoperative and intraoperative data,and pathological findings of 179 patients with rectal cancer who underwent laparoscopic radical rectal cancer surgery at our hospital between January 2022 and December 2023 was conducted.These clinical data were compared between two groups:Patients with sphincter preservation and those without,categorised as the sphincter-preserved and sphincter-unpreserved groups,respectively.RESULTS Of the 179 patients analysed,150 were in the sphincter-preserved group and 29 were in the sphincter-unpreserved group.Tumour height was significantly greater in the sphincter-preserved group compared to the sphincter-unpreserved group.Conversely,elevated levels of carcinoembryonic antigen,carbohydrate antigen 19-9,and plasma D-dimer were significantly higher in the sphincter-unpreserved group.Significant differences were also observed between the two groups in terms of place of residence,presence of colonic polyps,neoadjuvant chemotherapy,preoperative radiotherapy,mucinous adenocarcinoma,nerve invasion,and tumour height.No significant differences were observed for other parameters.Logistic regression analysis identified colonic polyps,mucinous adenocarcinoma,nerve invasion,and tumour height as independent risk factors for sphincter preser-vation.CONCLUSION Several risk factors influencing sphincter preservation in laparoscopic radical rectal cancer surgery were identified.These factors could be valuable tools for guiding clinical decision-making and optimising preoperative patient evaluations.展开更多

BACKGROUND There is no effective treatment for Alzheimer's disease(AD),and pharmacological treatment of AD in clinical settings is expensive and prolonged,resulting in a huge psychological and economic burden on t...BACKGROUND There is no effective treatment for Alzheimer's disease(AD),and pharmacological treatment of AD in clinical settings is expensive and prolonged,resulting in a huge psychological and economic burden on the patient's family and caregivers and society as a whole,AD is characterized by progressive,worsening cognitive impairment,and there are currently no drugs that can effectively reverse cognitive impairment.However,it is important to intervene early or delay cognitive impairment so that the condition can be delayed and,ultimately,the burden on patients and families can be reduced through maintenance treatment.It may be that non-pharmacological interventions such as cognitive stimulation therapy(CST)can help with cognitive dysfunction.AIM To provide a better treatment plan for AD patients and delay the deterioration of cognitive function,the effect of CST on cognitive function in AD was studied by Meta-analysis.METHODS Comprehensive search the Chinese and English databases were comprehensively searched by computer.Chinese databases:China Biomedical Literature Database(CBM),Wanfang Database,VIP Database,and China Periodicals Full-text Database(CNKI).The collection time limit is from July 21,2010 to July 21,2022 randomized controlled trials literature on the effects of CST on cognitive function in patients with AD.According to the inclusion and exclusion criteria,literature screening,data extraction,and quality evaluation were performed.Standardized mean difference(SMD)and 95%CI were used as evaluation criteria to evaluate the cognitive function of CST in AD patients.Sensitivity analysis and publication bias detection were performed on the results.Publication bias was assessed using funnel plots,and funnel plot symmetry was assessed with Eggr's test.RESULTS CST can not improve Mental State Examination Scale(MMSE)scores in AD patients.Meta-analysis of CST on MMSE scores showed that the heterogeneity was P=0.14,I2=35%.I2=35%<50%,and the Q test P>0.1,choose the random effect model to integrate statistics,get SMD=0.02,95%CI:-0.37,0.42,P>0.05.Meta-analysis of CST on AD Cognitive Functioning Assessment Scale scores showed that the heterogeneity was P=0.13,I2=36%.I2=36%<50 choose a fixed effect model to integrate statistics,get SMD=-0.01,95%CI:-0.40,0.39,P>0.05,the difference is not statistically significant.Meta-analysis of CST on the cognitive function indicators of patients showed that the heterogeneity was P=0.17,I2=31%.I2=31%<50%,the fixed effect model showed SMD=0.01,95%CI:-0.37,0.38,P>0.05,the difference was not statistically significant.CONCLUSION CST may not improve the cognitive function of AD patients,not improve the cognitive function of AD patients,not improve the ability of daily living,and not reduce mental behavior can improve the cognitive function of AD patients.展开更多
BACKGROUND For patients with advanced gastric cancer,surgical resection remains the main treatment option.Total gastrectomy combined with radical resection of gastric cancer lesions and sentinel lymph nodes can signif...BACKGROUND For patients with advanced gastric cancer,surgical resection remains the main treatment option.Total gastrectomy combined with radical resection of gastric cancer lesions and sentinel lymph nodes can significantly prolong the survival of patients.Digestive tract reconstruction after total gastrectomy is essential to maintain gastrointestinal function and optimize postoperative recovery.Therefore,it is very important to choose a suitable reconstruction method to improve the quality of life of total gastrectomy patients.AIM To evaluate the effects of different digestive tract reconstruction methods in gastric cancer patients undergoing total gastrectomy.METHODS This retrospective study included 172 patients who underwent total gastrectomy for gastric cancer at The First Hospital of Hebei Medical University for analysis.The patients were categorized into two groups:Group A,consisting of 90 patients who underwent modified Roux-en-Y gastrojejunostomy,and group B,consisting of 82 patients who underwent uncut Roux-en-Y gastrojejunostomy.The general patient characteristics,perioperative indicators,postoperative gastrointestinal mucosal barrier function,nutritional status,immunological markers,and occurrence of complications were compared between the two groups.RESULTS Group A showed shorter digestive tract reconstruction time than group B(P<0.05).On the first postoperative day,group A showed lower serum levels of D-lactate,diamine oxidase,and endotoxin than group B(P<0.05).One month postoperatively,group A showed higher prognostic nutritional index,serum albumin,total protein,and body weight than group B(P<0.05).One month postoperatively,the levels of cluster of differentiation(CD)3+,CD4+,and CD8+cells were not significantly different between two groups(P>0.05).The complication rates were 10.00%in group A and 24.39%in group B;group A had a significantly lower complication rate than group B(P<0.05).CONCLUSION Using modified Roux-en-Y gastrojejunostomy during total gastrectomy shortens the time required for gastrointestinal anastomosis,reduces surgery-induced gastrointestinal mucosal damage,and mitigates postoperative declines in nutritional status.展开更多
BACKGROUND SLC16A8,a lactate efflux transporter,is upregulated in various cancers,but its effects on tumor microenvironments remain understudied.This research explores its role in colorectal cancer(CRC)and the impact ...BACKGROUND SLC16A8,a lactate efflux transporter,is upregulated in various cancers,but its effects on tumor microenvironments remain understudied.This research explores its role in colorectal cancer(CRC)and the impact on the associated microenvir-onment consisting of vascular endothelial cells.AIM To explore the role in CRC and the impact on the associated microenvironment consisting of vascular endothelial cells.METHODS Hypoxic conditions prompted examination of SLC16A8 expression,glycolysis,lactate efflux,and Warburg effect correlations in CRC cell lines.Co-culture with HUVEC allowed for endothelial-mesenchymal transition(EndMT)character-ization,revealing lactate efflux's influence.Knockdown of SLC16A8 in CRC cells enabled relevant phenotype tests and tumorigenesis experiments,investigating tumor growth,blood vessel distribution,and signaling pathway alterations.RESULTS SLC16A8 expression was significantly upregulated in CRC tissues compared to adjacent normal tissues and correlated with disease progression(P<0.05).Under hypoxic conditions,HIF-1αinduced SLC16A8 expression,leading to enhanced metabolic reprogramming and increased lactate production.siRNA-mediated SLC16A8 knockdown effectively reversed hypoxia-induced changes,including reduced glucose consumption and lactate production.Co-culture experiments revealed that SLC16A8 knockdown significantly inhibited hypoxia-induced EndMT in HUVEC cells.In vivo studies demonstrated that SLC16A8 knockdown suppressed tumor growth,reduced Ki67 expression,and decreased HIF-1αlevels.Furthermore,SLC16A8 silencing led to decreased ex-pression of key metabolic enzymes PKM2 and LDHA,indicating its role in glycolytic regulation.CONCLUSION Our findings reveal that SLC16A8 functions as a critical mediator of hypoxia-induced metabolic reprogramming in CRC progression.展开更多
With the global population aging,the care of elderly cancer patients has become increasingly complex and significant.Comprehensive geriatric assessment(CGA),a multidimensional evaluation tool,has been widely implement...With the global population aging,the care of elderly cancer patients has become increasingly complex and significant.Comprehensive geriatric assessment(CGA),a multidimensional evaluation tool,has been widely implemented in oncology nursing to enhance the precision of treatment decisions and improve patient outcomes.This review examines the application of CGA in oncology nursing,drawing on literature published between 2010 and 2024 in major databases using keywords such as“Comprehensive Geriatric Assessment”and“Oncology Nursing”.It highlights how CGA contributes to optimizing treatment selection,monitoring the treatment process,and improving patients’quality of life and long-term outcomes.CGA provides a comprehensive evaluation of elderly cancer patients,including physical,psychological,and social aspects,enabling the identification of high-risk patients and reducing treatment-related side effects and complications.It also offers a critical foundation for developing personalized care plans.The article discusses various practical examples of CGA implementation across different countries and regions,including multidisciplinary collaborative models in France,the United States,and Australia,demonstrating CGA’s flexible application in diverse healthcare settings.Although significant progress has been made in applying CGA in oncology nursing,numerous challenges remain in its implementation,such as resource limitations and insufficient personnel training.Future research will focus on integrating CGA with emerging technologies,such as artificial intelligence and precision medicine,to further improve the quality of care and treatment outcomes for elderly cancer patients.By summarizing the current status and challenges of CGA in oncology nursing,this review provides guidance for future research and clinical practice,emphasizing the importance of advancing CGA application to meet the growing demands of elderly oncology care.展开更多

A 20-year-old woman with ulcerative colitis developed acute severe bloody diarrhea following proctocolectomy, ileal pouchanal anastomosis and diverting ileostomy. She was diagnosed as having a secondary pouchitis mainly caused by a peripouch abscess and partly concerned with the abnormal pouch formation. The remnantrectum and ileal pouch were excised and ileal pouch-anal anastomosis and diverting ileostomy were constructed.The postoperative course was uneventful with no sign of pouchitis. Salvage surgery may be indicated to treat secondary pouchitis when caused by surgery-related complications.展开更多
In this editorial,we comment on the article entitled“Advances and key focus areas in gastric cancer immunotherapy:A comprehensive scientometric and clinical trial review(1999-2023),”which was published in the recent...In this editorial,we comment on the article entitled“Advances and key focus areas in gastric cancer immunotherapy:A comprehensive scientometric and clinical trial review(1999-2023),”which was published in the recent issue of the World Journal of Gastroenterology.We focused on the results of the authors’bibliometric analysis concerning gastric cancer immunotherapy,which they analyzed in depth by compiling the relevant publications of the last 20 years.Before that,we briefly describe the most recent data concerning the epidemiological parameters of gastric cancer(GC)in different countries,attempting to give an interpretation based on the etiological factors involved in the etiopathogenesis of the neoplasm.We then briefly discuss the conservative treatment(chemotherapy)of the various forms of this malignant neoplasm.We describe the treatment of resectable tumors,locally advanced neoplasms,and unresectable(advanced)cases.Special attention is given to modern therapeutic approaches with emphasis on immunotherapy,which seems to be the future of GC treatment,especially in combination with chemotherapy.There is also a thorough analysis of the results of the study under review in terms of the number of scientific publications,the countries in which the studies were conducted,the authors,and the scientific centers of origin,as well as the clinical studies in progress.Finally,an attempt is made to draw some conclusions and to point out possible future directions.展开更多
In this editorial,we comment on the article by Lyu et al published in the recent issue of the World Journal of Gastroenterology(2023;2219-2840).Hepatocellular carcinoma(HCC)is a frequently encountered and highly aggre...In this editorial,we comment on the article by Lyu et al published in the recent issue of the World Journal of Gastroenterology(2023;2219-2840).Hepatocellular carcinoma(HCC)is a frequently encountered and highly aggressive primary liver cancer,which remains the third-commonest cause of cancer-related death despite the current therapeutic modalities.There is urgency in developing novel thera-peutic approaches,such as by manipulating extracellular vesicles,which con-stitute a highly heterogeneous nanoparticle population that contains various cargoes.These cargoes have a pivotal role in cell-to-cell communication and can modify the functional level of the recipient cells via their uptake by other recipient cells.Exosomal non-coding RNAs have particular evolving significance in HCC,such as circular RNAs,which have been found differentially expressed in normal hepatic and HCC tissues.The aberrations in their expression levels have a key role in the HCC development and progression and the overall prognosis.In this editorial,we will shed light on the emerging role of exosomal circular RNAs in HCC development and progression,focusing on the oncogenic or potentially tumor suppressive effect of mesenchymal stem cells-derived exosomal non-coding RNAs.展开更多
BACKGROUND Colorectal cancer(CRC)is the third most common cancer and a significant cause of cancer-related mortality globally.Resistance to chemotherapy,especially during CRC treatment,leads to reduced effectiveness o...BACKGROUND Colorectal cancer(CRC)is the third most common cancer and a significant cause of cancer-related mortality globally.Resistance to chemotherapy,especially during CRC treatment,leads to reduced effectiveness of drugs and poor patient outcomes.Long noncoding RNAs(lncRNAs)have been implicated in various pathophysiological processes of tumor cells,including chemotherapy resistance,yet the roles of many lncRNAs in CRC remain unclear.AIM To identify and analyze the lncRNAs involved in oxaliplatin resistance in CRC and to understand the underlying molecular mechanisms influencing this resistance.METHODS Gene Expression Omnibus datasets GSE42387 and GSE30011 were reanalyzed to identify lncRNAs and mRNAs associated with oxaliplatin resistance.Various bioinformatics tools were employed to elucidate molecular mechanisms.The expression levels of lncRNAs and mRNAs were assessed via quantitative reverse transcription-polymerase chain reaction.Functional assays,including MTT,wound healing,and Transwell,were conducted to investigate the functional implications of lncRNA alterations.Interactions between lncRNAs and trans-cription factors were examined using RIP and luciferase reporter assays,while Western blotting was used to confirm downstream pathways.Additionally,a xenograft mouse model was utilized to study the in vivo effects of lncRNAs on chemotherapy resistance.RESULTS LncRNA prion protein testis specific(PRNT)was found to be upregulated in oxaliplatin-resistant CRC cell lines and negatively correlated with homeodomain interacting protein kinase 2(HIPK2)expression.PRNT was demonstrated to sponge transcription factor zinc finger protein 184(ZNF184),which in turn could regulate HIPK2 expression.Altered expression of PRNT influenced CRC cell sensitivity to oxaliplatin,with overexpression leading to decreased sensitivity and decreased expression reducing resistance.Both RIP and luciferase reporter assays indicated that ZNF184 and HIPK2 are targets of PRNT.The PRNT/ZNF184/HIPK2 axis was implicated in promoting CRC progression and oxaliplatin resistance both in vitro and in vivo.CONCLUSION The study concludes that PRNT is upregulated in oxaliplatin-resistant CRC cells and modulates the expression of HIPK2 by sponging ZNF184.This regulatory mechanism enhances CRC progression and resistance to oxaliplatin,positioning PRNT as a promising therapeutic target for CRC patients undergoing oxaliplatin-based chemotherapy.展开更多

文摘Cardiac arrest can lead to severe neurological impairment as a result of inflammation,mitochondrial dysfunction,and post-cardiopulmonary resuscitation neurological damage.Hypoxic preconditioning has been shown to improve migration and survival of bone marrow–derived mesenchymal stem cells and reduce pyroptosis after cardiac arrest,but the specific mechanisms by which hypoxia-preconditioned bone marrow–derived mesenchymal stem cells protect against brain injury after cardiac arrest are unknown.To this end,we established an in vitro co-culture model of bone marrow–derived mesenchymal stem cells and oxygen–glucose deprived primary neurons and found that hypoxic preconditioning enhanced the protective effect of bone marrow stromal stem cells against neuronal pyroptosis,possibly through inhibition of the MAPK and nuclear factor κB pathways.Subsequently,we transplanted hypoxia-preconditioned bone marrow–derived mesenchymal stem cells into the lateral ventricle after the return of spontaneous circulation in an 8-minute cardiac arrest rat model induced by asphyxia.The results showed that hypoxia-preconditioned bone marrow–derived mesenchymal stem cells significantly reduced cardiac arrest–induced neuronal pyroptosis,oxidative stress,and mitochondrial damage,whereas knockdown of the liver isoform of phosphofructokinase in bone marrow–derived mesenchymal stem cells inhibited these effects.To conclude,hypoxia-preconditioned bone marrow–derived mesenchymal stem cells offer a promising therapeutic approach for neuronal injury following cardiac arrest,and their beneficial effects are potentially associated with increased expression of the liver isoform of phosphofructokinase following hypoxic preconditioning.

文摘BACKGROUND Circulating tumor cells(CTCs)are crucial for improving our knowledge regarding tumor progress,prognosis,and recurrence possibility.AIM To evaluate the role of CTCs in the early diagnosis and treatment of gastric cancer.METHODS From June 2020 to December 2021,a randomized study was conducted in our institution involving 80 patients scheduled for surgery for gastric cancer.The patients were divided into two groups:A control group that was tested for traditional serum markers and a study group that was assessed for serum CTCs.RESULTS In the study cohort,CTC levels did not correlate significantly with patient age,gender,or degree of tumor differentiation(P>0.05).However,there was a significant correlation with the tumor-node-metastasis stage of the tumor(P<0.05).In the study group,the CTC diagnostic positivity rate was 62.50%(25 out of 40 patients),while the positivity rate for conventional serum markers in the control group was 47.50%(19 out of 40 patients).The positive detection rate in the study group was significantly higher than that of the control group(P<0.05).CONCLUSION CTCs have slight invasion and high sensitivity and specificity,presenting great value for early clinical diagnosis of recurrence and metastasis.It will improve the deceleration of disease development and increase the survival rate.
文摘Inappropriate use of antibiotics leads to microbial resistance.Single-dose antibio-tic prophylaxis prior to laparoscopic cholecystectomy is well known for reducing the risk of postoperative infection in high-risk patients despite some conflicting aspects.High-risk patients are those who are older than 70 years,have diabetes mellitus,whose operation time exceeded 120 minutes,have acute cholecystitis,experienced iatrogenic intraoperative gallbladder perforation resulting in bile or gallstone spillage,suffered from obstructive jaundice,or were deemed immuno-compromised.For gallbladder perforation,one dose of antibiotic prophylaxis is sufficient.Therefore,guidelines are needed and must be strictly followed.Prophy-lactic treatment is not needed for patients at low risk of developing sepsis fo-llowing elective laparoscopic cholecystectomy,although the opposite is suppor-ted.Similarly,superficial surgical infections are related to low morbidity.Patients without risk factors have a very low risk of infection.Thus,the routine use of anti-biotic prophylaxis in elective laparoscopic cholecystectomy is not recommended.
文摘BACKGROUND Colon cancer is one of the most common malignancies worldwide,and chemo-therapy is a widely used strategy in colon cancer clinical therapy.Chemotherapy resistance is the main cause of recurrence and progression in colon cancer.Thus,novel drugs for treatment are urgently needed.Tetramethylpyrazine(TMP),a component of the traditional Chinese medicine Chuanxiong Hort,has been proven to exhibit a beneficial effect in tumors.AIM To investigate the potential anticancer activity of TMP in colon cancer and the underlying mechanisms.METHODS Colon cancer cells were incubated with different concentrations of TMP.Cell viability was evaluated by crystal violet staining assay,and cell apoptosis was assessed by flow cytometry.Apoptosis-associated protein expression was measured using Western blot analysis.Intracellular reactive oxygen species(ROS)levels were assessed by flow cytometry using DCF fluorescence intensity.Xeno-grafts were established by the subcutaneous injection of colon cancer cells into nude mice;tumor growth was monitored and intracellular ROS was detected in tumors by malondialdehyde assay.RESULTS TMP induced apoptosis of colon cancer cells via the activation of the mitochon-drial pathway.TMP increased the generation of intracellular ROS and triggered mitochondria-mediated apoptosis in a caspase-dependent manner.CONCLUSION Our study demonstrates that TMP induces the apoptosis of colon cancer cells and increases the generation of intracellular ROS.TMP triggers mitochondria-mediated apoptosis in a caspase-dependent manner.The accumu-lation of intracellular ROS is involved in TMP-induced apoptosis.Our findings suggest that TMP may be a potential therapeutic drug for the treatment of colon cancer.

文摘This letter compares the clinical efficacy and economic feasibility of the scoliocorrector fatma-UI(SCFUI)with direct vertebral rotation(DVR)in treating adolescent idiopathic scoliosis(AIS).SCFUI has shown promising results in threedimensional spinal correction,providing superior rotational alignment compared to DVR and achieving significant improvements in coronal and sagittal planes.Additionally,SCFUI’s advanced design reduces risks associated with AIS surgeries and enhances overall patient outcomes.Economic analysis reveals SCFUI as a cost-effective option,potentially lowering long-term healthcare costs by minimizing complications and revisions.Our findings suggest that SCFUI is a viable,innovative approach in AIS treatment,meeting clinical and economic demands in orthopedic care.

文摘BACKGROUND The upregulation of serpin family B member 5(SERPINB5)has been linked to the progression of rectal cancer.However,the specific roles and underlying mechanisms of SERPINB5 in rectal cancer are not fully understood.AIM To investigate the roles and mechanisms of SERPINB5 in rectal cancer.METHODS SERPINB5 protein level in rectal cancer tissues and cell lines was measured through western blot analysis.SW480 cells were transfected with pcDNASERPINB5 or short-hairpin RNA targeting SERPINB5(sh-SERPINB5).Cell proliferation,invasion,and apoptosis were then evaluated.The interaction between SERPINB5 and heat shock protein 90 alpha class A member 1(HSP90AA1)was confirmed through a co-immunoprecipitation assay.Subsequently,pcDNAHSP90AA1 or sh-HSP90AA1 was transfected into SW480 cells,and cell progression was then detected.Moreover,rescue experiments were used to investigate the effect of the SERPINB5/HSP90AA1 axis on rectal cancer progression.Additionally,sh-SERPINB5-transfected SW480 cells were implanted into nude mice,and xenograft tumor growth was then evaluated.RESULTS SERPINB5 was prominently upregulated in rectal cancer tissues and cells.SERPINB5 overexpression increased SW480 cell proliferation and invasion while reducing apoptosis.In contrast,SERPINB5 knockdown had the opposite effects.Moreover,SERPINB5 could interact with HSP90AA1 and promote HSP90AA1 expression in SW480 cells.HSP90AA1 overexpression facilitated SW480 cell proliferation and invasion and restrained apoptosis.By contrast,HSP90AA1 knockdown suppressed cell progression.The upregulation of HSP90AA1 reversed the SERPINB5 silencing-mediated inhibition of SW480 cell progression.Additionally,SERPINB5 knockdown retarded the growth of rectal cancer tumors in vivo.CONCLUSION SERPINB5 knockdown inhibited rectal cancer cell proliferation and invasion and retarded xenograft tumor growth by inhibiting HSP90AA1 expression.

文摘BACKGROUND The surgical management of rectal cancer is continuously advancing,with a current emphasis on minimising the need for a permanent stoma.Understanding the risk factors influencing sphincter preservation is crucial for guiding clinical decision-making and optimising preoperative patient evaluation.AIM To examine the risk factors influencing sphincter preservation in laparoscopic radical rectal cancer surgery.METHODS A retrospective analysis of the demographics,preoperative and intraoperative data,and pathological findings of 179 patients with rectal cancer who underwent laparoscopic radical rectal cancer surgery at our hospital between January 2022 and December 2023 was conducted.These clinical data were compared between two groups:Patients with sphincter preservation and those without,categorised as the sphincter-preserved and sphincter-unpreserved groups,respectively.RESULTS Of the 179 patients analysed,150 were in the sphincter-preserved group and 29 were in the sphincter-unpreserved group.Tumour height was significantly greater in the sphincter-preserved group compared to the sphincter-unpreserved group.Conversely,elevated levels of carcinoembryonic antigen,carbohydrate antigen 19-9,and plasma D-dimer were significantly higher in the sphincter-unpreserved group.Significant differences were also observed between the two groups in terms of place of residence,presence of colonic polyps,neoadjuvant chemotherapy,preoperative radiotherapy,mucinous adenocarcinoma,nerve invasion,and tumour height.No significant differences were observed for other parameters.Logistic regression analysis identified colonic polyps,mucinous adenocarcinoma,nerve invasion,and tumour height as independent risk factors for sphincter preser-vation.CONCLUSION Several risk factors influencing sphincter preservation in laparoscopic radical rectal cancer surgery were identified.These factors could be valuable tools for guiding clinical decision-making and optimising preoperative patient evaluations.

文摘This study highlights the importance of identifying and addressing risk factors associated with wound complications following transtibial amputation in diabetic patients.These amputations,often necessitated by severe diabetic foot ulcers,carry significant risks of postoperative complications such as infection and delayed wound healing.Elevated hemoglobin A1c levels,indicative of poor glycemic control,and a history of kidney transplantation,due to required immunosuppressive therapy,are key factors influencing these outcomes.This paper emphasizes the need for enhanced glycemic management and personalized postoperative care,particularly for immunocompromised individuals,to minimize complications and improve patient prognosis.Future research should focus on prospective studies to validate targeted interventions and optimize care strategies,ultimately aiming to reduce the healthcare burden associated with diabetic foot complications.

文摘Necrotizing fasciitis(NF)is a rapidly progressing,life-threatening soft tissue infection,with upper limb NF posing a particularly serious threat to patient survival and quality of life.Negative pressure wound therapy(NPWT)has shown considerable advantages in accelerating wound healing and mitigating functional impairment.A retrospective study by Lipatov et al.demonstrated that NPWT significantly reduced the time needed for wound closure preparation while enhancing the success rate of local repair.Despite its benefits,certain limitations highlight the need for further optimization.This paper investigates the potential for personalized dynamic regulation of NPWT,its integration with adjunctive therapies,and the role of multidisciplinary collaboration.Furthermore,it explores the incorporation of advanced technologies such as artificial intelligence,imaging modalities,and biomaterials,presenting novel pathways for the personalized management and global standardization of NF treatment.

文摘BACKGROUND There is no effective treatment for Alzheimer's disease(AD),and pharmacological treatment of AD in clinical settings is expensive and prolonged,resulting in a huge psychological and economic burden on the patient's family and caregivers and society as a whole,AD is characterized by progressive,worsening cognitive impairment,and there are currently no drugs that can effectively reverse cognitive impairment.However,it is important to intervene early or delay cognitive impairment so that the condition can be delayed and,ultimately,the burden on patients and families can be reduced through maintenance treatment.It may be that non-pharmacological interventions such as cognitive stimulation therapy(CST)can help with cognitive dysfunction.AIM To provide a better treatment plan for AD patients and delay the deterioration of cognitive function,the effect of CST on cognitive function in AD was studied by Meta-analysis.METHODS Comprehensive search the Chinese and English databases were comprehensively searched by computer.Chinese databases:China Biomedical Literature Database(CBM),Wanfang Database,VIP Database,and China Periodicals Full-text Database(CNKI).The collection time limit is from July 21,2010 to July 21,2022 randomized controlled trials literature on the effects of CST on cognitive function in patients with AD.According to the inclusion and exclusion criteria,literature screening,data extraction,and quality evaluation were performed.Standardized mean difference(SMD)and 95%CI were used as evaluation criteria to evaluate the cognitive function of CST in AD patients.Sensitivity analysis and publication bias detection were performed on the results.Publication bias was assessed using funnel plots,and funnel plot symmetry was assessed with Eggr's test.RESULTS CST can not improve Mental State Examination Scale(MMSE)scores in AD patients.Meta-analysis of CST on MMSE scores showed that the heterogeneity was P=0.14,I2=35%.I2=35%<50%,and the Q test P>0.1,choose the random effect model to integrate statistics,get SMD=0.02,95%CI:-0.37,0.42,P>0.05.Meta-analysis of CST on AD Cognitive Functioning Assessment Scale scores showed that the heterogeneity was P=0.13,I2=36%.I2=36%<50 choose a fixed effect model to integrate statistics,get SMD=-0.01,95%CI:-0.40,0.39,P>0.05,the difference is not statistically significant.Meta-analysis of CST on the cognitive function indicators of patients showed that the heterogeneity was P=0.17,I2=31%.I2=31%<50%,the fixed effect model showed SMD=0.01,95%CI:-0.37,0.38,P>0.05,the difference was not statistically significant.CONCLUSION CST may not improve the cognitive function of AD patients,not improve the cognitive function of AD patients,not improve the ability of daily living,and not reduce mental behavior can improve the cognitive function of AD patients.
文摘BACKGROUND For patients with advanced gastric cancer,surgical resection remains the main treatment option.Total gastrectomy combined with radical resection of gastric cancer lesions and sentinel lymph nodes can significantly prolong the survival of patients.Digestive tract reconstruction after total gastrectomy is essential to maintain gastrointestinal function and optimize postoperative recovery.Therefore,it is very important to choose a suitable reconstruction method to improve the quality of life of total gastrectomy patients.AIM To evaluate the effects of different digestive tract reconstruction methods in gastric cancer patients undergoing total gastrectomy.METHODS This retrospective study included 172 patients who underwent total gastrectomy for gastric cancer at The First Hospital of Hebei Medical University for analysis.The patients were categorized into two groups:Group A,consisting of 90 patients who underwent modified Roux-en-Y gastrojejunostomy,and group B,consisting of 82 patients who underwent uncut Roux-en-Y gastrojejunostomy.The general patient characteristics,perioperative indicators,postoperative gastrointestinal mucosal barrier function,nutritional status,immunological markers,and occurrence of complications were compared between the two groups.RESULTS Group A showed shorter digestive tract reconstruction time than group B(P<0.05).On the first postoperative day,group A showed lower serum levels of D-lactate,diamine oxidase,and endotoxin than group B(P<0.05).One month postoperatively,group A showed higher prognostic nutritional index,serum albumin,total protein,and body weight than group B(P<0.05).One month postoperatively,the levels of cluster of differentiation(CD)3+,CD4+,and CD8+cells were not significantly different between two groups(P>0.05).The complication rates were 10.00%in group A and 24.39%in group B;group A had a significantly lower complication rate than group B(P<0.05).CONCLUSION Using modified Roux-en-Y gastrojejunostomy during total gastrectomy shortens the time required for gastrointestinal anastomosis,reduces surgery-induced gastrointestinal mucosal damage,and mitigates postoperative declines in nutritional status.

文摘BACKGROUND SLC16A8,a lactate efflux transporter,is upregulated in various cancers,but its effects on tumor microenvironments remain understudied.This research explores its role in colorectal cancer(CRC)and the impact on the associated microenvir-onment consisting of vascular endothelial cells.AIM To explore the role in CRC and the impact on the associated microenvironment consisting of vascular endothelial cells.METHODS Hypoxic conditions prompted examination of SLC16A8 expression,glycolysis,lactate efflux,and Warburg effect correlations in CRC cell lines.Co-culture with HUVEC allowed for endothelial-mesenchymal transition(EndMT)character-ization,revealing lactate efflux's influence.Knockdown of SLC16A8 in CRC cells enabled relevant phenotype tests and tumorigenesis experiments,investigating tumor growth,blood vessel distribution,and signaling pathway alterations.RESULTS SLC16A8 expression was significantly upregulated in CRC tissues compared to adjacent normal tissues and correlated with disease progression(P<0.05).Under hypoxic conditions,HIF-1αinduced SLC16A8 expression,leading to enhanced metabolic reprogramming and increased lactate production.siRNA-mediated SLC16A8 knockdown effectively reversed hypoxia-induced changes,including reduced glucose consumption and lactate production.Co-culture experiments revealed that SLC16A8 knockdown significantly inhibited hypoxia-induced EndMT in HUVEC cells.In vivo studies demonstrated that SLC16A8 knockdown suppressed tumor growth,reduced Ki67 expression,and decreased HIF-1αlevels.Furthermore,SLC16A8 silencing led to decreased ex-pression of key metabolic enzymes PKM2 and LDHA,indicating its role in glycolytic regulation.CONCLUSION Our findings reveal that SLC16A8 functions as a critical mediator of hypoxia-induced metabolic reprogramming in CRC progression.

文摘With the global population aging,the care of elderly cancer patients has become increasingly complex and significant.Comprehensive geriatric assessment(CGA),a multidimensional evaluation tool,has been widely implemented in oncology nursing to enhance the precision of treatment decisions and improve patient outcomes.This review examines the application of CGA in oncology nursing,drawing on literature published between 2010 and 2024 in major databases using keywords such as“Comprehensive Geriatric Assessment”and“Oncology Nursing”.It highlights how CGA contributes to optimizing treatment selection,monitoring the treatment process,and improving patients’quality of life and long-term outcomes.CGA provides a comprehensive evaluation of elderly cancer patients,including physical,psychological,and social aspects,enabling the identification of high-risk patients and reducing treatment-related side effects and complications.It also offers a critical foundation for developing personalized care plans.The article discusses various practical examples of CGA implementation across different countries and regions,including multidisciplinary collaborative models in France,the United States,and Australia,demonstrating CGA’s flexible application in diverse healthcare settings.Although significant progress has been made in applying CGA in oncology nursing,numerous challenges remain in its implementation,such as resource limitations and insufficient personnel training.Future research will focus on integrating CGA with emerging technologies,such as artificial intelligence and precision medicine,to further improve the quality of care and treatment outcomes for elderly cancer patients.By summarizing the current status and challenges of CGA in oncology nursing,this review provides guidance for future research and clinical practice,emphasizing the importance of advancing CGA application to meet the growing demands of elderly oncology care.
文摘Cholangiocarcinoma(CCA)is a highly aggressive and heterogeneous malignancy arising from the epithelial cells of the biliary tract.The limitations of the current methods in the diagnosis of CCA highlight the urgent need for new,accurate tools for early cancer detection,better prognostication and patient monitoring.Liquid biopsy(LB)is a modern and non-invasive technique comprising a diverse group of methodologies aiming to detect tumour biomarkers from body fluids.These biomarkers include circulating tumour cells,cell-free DNA,circulating tumour DNA,RNA and extracellular vesicles.The aim of this review is to explore the current and potential future applications of LB in CCA management,with a focus on diagnosis,prognostication and monitoring.We examine both its significant potential and the inevitable limitations associated with this technology.We conclude that LB holds considerable promise,but further research is necessary to fully integrate it into precision oncology for CCA.
文摘We report a case of secondary pouchitis, defined as a mucosal inflammatory lesion in the ileal reservoir provoked by pouch-related complication following total colectomy and pouch anal anastomosis, which was successfully treated by salvage surgery. A 20-year-old woman with ulcerative colitis developed acute severe bloody diarrhea following proctocolectomy, ileal pouchanal anastomosis and diverting ileostomy. She was diagnosed as having a secondary pouchitis mainly caused by a peripouch abscess and partly concerned with the abnormal pouch formation. The remnantrectum and ileal pouch were excised and ileal pouch-anal anastomosis and diverting ileostomy were constructed.The postoperative course was uneventful with no sign of pouchitis. Salvage surgery may be indicated to treat secondary pouchitis when caused by surgery-related complications.
文摘In this editorial,we comment on the article entitled“Advances and key focus areas in gastric cancer immunotherapy:A comprehensive scientometric and clinical trial review(1999-2023),”which was published in the recent issue of the World Journal of Gastroenterology.We focused on the results of the authors’bibliometric analysis concerning gastric cancer immunotherapy,which they analyzed in depth by compiling the relevant publications of the last 20 years.Before that,we briefly describe the most recent data concerning the epidemiological parameters of gastric cancer(GC)in different countries,attempting to give an interpretation based on the etiological factors involved in the etiopathogenesis of the neoplasm.We then briefly discuss the conservative treatment(chemotherapy)of the various forms of this malignant neoplasm.We describe the treatment of resectable tumors,locally advanced neoplasms,and unresectable(advanced)cases.Special attention is given to modern therapeutic approaches with emphasis on immunotherapy,which seems to be the future of GC treatment,especially in combination with chemotherapy.There is also a thorough analysis of the results of the study under review in terms of the number of scientific publications,the countries in which the studies were conducted,the authors,and the scientific centers of origin,as well as the clinical studies in progress.Finally,an attempt is made to draw some conclusions and to point out possible future directions.
文摘In this editorial,we comment on the article by Lyu et al published in the recent issue of the World Journal of Gastroenterology(2023;2219-2840).Hepatocellular carcinoma(HCC)is a frequently encountered and highly aggressive primary liver cancer,which remains the third-commonest cause of cancer-related death despite the current therapeutic modalities.There is urgency in developing novel thera-peutic approaches,such as by manipulating extracellular vesicles,which con-stitute a highly heterogeneous nanoparticle population that contains various cargoes.These cargoes have a pivotal role in cell-to-cell communication and can modify the functional level of the recipient cells via their uptake by other recipient cells.Exosomal non-coding RNAs have particular evolving significance in HCC,such as circular RNAs,which have been found differentially expressed in normal hepatic and HCC tissues.The aberrations in their expression levels have a key role in the HCC development and progression and the overall prognosis.In this editorial,we will shed light on the emerging role of exosomal circular RNAs in HCC development and progression,focusing on the oncogenic or potentially tumor suppressive effect of mesenchymal stem cells-derived exosomal non-coding RNAs.

文摘BACKGROUND Colorectal cancer(CRC)is the third most common cancer and a significant cause of cancer-related mortality globally.Resistance to chemotherapy,especially during CRC treatment,leads to reduced effectiveness of drugs and poor patient outcomes.Long noncoding RNAs(lncRNAs)have been implicated in various pathophysiological processes of tumor cells,including chemotherapy resistance,yet the roles of many lncRNAs in CRC remain unclear.AIM To identify and analyze the lncRNAs involved in oxaliplatin resistance in CRC and to understand the underlying molecular mechanisms influencing this resistance.METHODS Gene Expression Omnibus datasets GSE42387 and GSE30011 were reanalyzed to identify lncRNAs and mRNAs associated with oxaliplatin resistance.Various bioinformatics tools were employed to elucidate molecular mechanisms.The expression levels of lncRNAs and mRNAs were assessed via quantitative reverse transcription-polymerase chain reaction.Functional assays,including MTT,wound healing,and Transwell,were conducted to investigate the functional implications of lncRNA alterations.Interactions between lncRNAs and trans-cription factors were examined using RIP and luciferase reporter assays,while Western blotting was used to confirm downstream pathways.Additionally,a xenograft mouse model was utilized to study the in vivo effects of lncRNAs on chemotherapy resistance.RESULTS LncRNA prion protein testis specific(PRNT)was found to be upregulated in oxaliplatin-resistant CRC cell lines and negatively correlated with homeodomain interacting protein kinase 2(HIPK2)expression.PRNT was demonstrated to sponge transcription factor zinc finger protein 184(ZNF184),which in turn could regulate HIPK2 expression.Altered expression of PRNT influenced CRC cell sensitivity to oxaliplatin,with overexpression leading to decreased sensitivity and decreased expression reducing resistance.Both RIP and luciferase reporter assays indicated that ZNF184 and HIPK2 are targets of PRNT.The PRNT/ZNF184/HIPK2 axis was implicated in promoting CRC progression and oxaliplatin resistance both in vitro and in vivo.CONCLUSION The study concludes that PRNT is upregulated in oxaliplatin-resistant CRC cells and modulates the expression of HIPK2 by sponging ZNF184.This regulatory mechanism enhances CRC progression and resistance to oxaliplatin,positioning PRNT as a promising therapeutic target for CRC patients undergoing oxaliplatin-based chemotherapy.
