Objective:Traditional Chinese medicine(TCM)and modern medicine have both been used in arresting malignant transformation of chronic atrophic gastritis(CAG)in China with good therapeutic effect.However,no studies have been undertaken to assess the risk of malignant transformation in CAG patients using both modern medicine and TCM features.Our study aimed to develop risk assessment models for malignant transformation of CAG combining indicators of both TCM and modern medicine.These models will facilitate early warning and control of malignant transformation of CAG from the perspective of evidence-based integrative medicine.Methods:In the proposed registry study,a total of 1000 eligible CAG patients will be recruited from four hospitals in China.A 10-year follow-up study will be conducted both on-site and off-site to track the events of malignant transformation.Frequency analysis and chi-squared tests will be used to perform the comparative analysis on the prevalence of malignant transformation events and indicators in TCM or modern medicine in different groups.A multivariate Cox proportional hazard model will be used to perform correlation analyses of malignant transformation events and factors of TCM or modern medicine.Conclusion:The proposed study has been designed with a large sample size and long follow-up period,in which wide-ranging modern medicine and TCM indicators can be gathered over the whole process of malignant transformation of CAG.Based on this study,risk assessment models for malignant transformation ofCAGmaybe constructed fromthe perspective of integrative medicine.This may provide clinicians and patients with an optimized early warning system as well as prevention strategies for malignant transformation of CAG. 展开更多

The neurotrophin-Trk receptor pathway is an intrinsic pathway to relieve damage to the central nervous system. The present study observed the effects of Tongluo Jiunao (TLJN), which comprises Panax Notoginseng and Gardenia Jasminoides, on expression of brain-derived neurotrophic factor (BDNF) and tropomyosin-related kinase B (TrkB) in a rat model of focal cerebral ischemic injury. Xue Sai Tong (XST), comprising Panax Notoginseng, served as the positive control. Mechanisms of neuroprotection were analyzed following TLJN injection. Following establishment of the middle cerebral artery occlusion models, TLJN and XST were intraperitoneally injected, and 2, 3 5-triphenyltetrazolium chloride staining results revealed that TLJN injection reduced infarct volume, suggesting that TLJN exerted a neuroprotective effect. Enzyme-linked immunosorbent assay results showed that TLJN elevated BDNF and growth associated protein-43 expression in ischemic brain tissues, as well as serum BDNF levels. Reverse-transcription polymerase chain reaction and western blot results showed that TLJN injection did not affect TrkB expression in the ischemic brain tissues of rats. These results suggested that TLJN injection reduced damage to ischemic brain tissues and increased BDNF expression. In addition, TLJN injection resulted in better promoting effects on neurotrophic factor expression compared with XST. 展开更多

Objective:Syndrome differentiation is a unique part of traditional Chinese medicine(TCM).Syndrome factors play an important role in the diagnosis and treatment of TCM syndromes.Thromboelastography(TEG)intuitively reflects the blood status of patients with acute ischemic stroke(AIS)and is important in the treatment and prognosis of AIS.To identify the relationship between TCM syndrome factors and TEG in AIS patients and standardize TCM syndrome differentiation and treatment objectives,we designed a prospective cohort study of 103 AIS patients.Methods:We used the diagnostic criteria for AIS in the Chinese Guideline for Diagnosis and Management of Acute Ischemic Stroke 2010.Diagnosis of phlegm-heat and fu-organ excess syndrome was based on the TCM Scale for the Syndrome of Phlegm-heat and fu-organ Excess.The ischemic Stroke TCM Syndrome Factor Diagnostic Scale was used to identify and diagnose syndrome factors.General information,scores of syndrome factors and values of TEG parameters of all enrolled patients were recorded.Results:There were significant differences in internal fire and phlegm-dampness scores between patients with and without phlegm-heat and fu-organ excess syndrome(P<.05).In patients with phlegm-heat and fu-organ excess syndrome,internal fire was negatively correlated with TEG parameters R and K(P<.05)and positively correlated with alpha Angle and coagulation index(P<.01).There were no significant correlations between the two syndrome factors and MA(P Z.058)and LY30(P>.05)or between both syndrome factors and TEG parameters in patients without phlegm-heat and fu-organ excess syndrome.Conclusion:The syndrome factor internal fire is a potential predictor of increased platelet function and fibrinogen activity in AIS patients with phlegm-heat and fu-organ excess,and a potentially important predictor of blood hypercoagulability in TCM. 展开更多

As a chronic disease that seriously endangers public health, the number of coronary heart disease (CHD) patients in China has increased in recent years. There is great potentiality to integrate traditional Chinese and Western medicine therapies to prevent and treat CHD in clinical practice. However, most of the current integrated therapies still lack sufficient high-quality evidence, and the key links in how to apply are unclear. It is urgent to optimize them through evidence-based research to further improve the effectiveness. Therefore, we propose strategies to conduct evidence-based optimization of integrated traditional Chinese and Western medicine therapies in prevention and treatment of progressive cardiovascular diseases. These are integrated clinical trial design, attention to the key links of taking effect, combination of clinical and basic research. Based on the strategies, we started the national key research and development project "Evidence-based optimization research of TCM therapies in prevention and treatment of CHD (angina pectoris - myocardial infarction - heart failure)", which is expected to be a new paradigm in the field. 展开更多

Objective: This meta- analysis evaluated the efficacy and safety of Chinese herbal medicine (CHM) combined withangiotensin-converting enzyme inhibitors (ACEIs) and/or angiotensin-receptor blockers (ARBs)for treatment ofincipient diabetic nephropathy(IDN). Methods: Nine data bases were searched for randomized controlled trials ofChinese herbal medicine(CHM) and ACEI/ARB for treatment of IDN. Included articles were published betweenJanuary2006 and December 2016. All studies were divided into prescriptions containing both Astragali Radix andRehmanniae Radix (i subgroup), Astragali Radix(Huangqi) or Rehmanniae Radix(Dihuang) (ii subgroup), neitherAstragali Radix nor Rehmanniae Radix (iii subgroup). Review Manager 5.3 was used for subgroup analysis.Results: In total, 28 RCTs with 2017 patients were included. The results showed 1)the urinary albumin excretionrate (UAER) can be reduced significantly using CHM with ACEI or ARB for treatment of IDN compared to ACEIor ARB alone, and reduction of the UAER of the i subgroup was superior to that of the other two subgroups;2)serum creatinine (Scr) levels can be reduced significantly using CHM combined with ACEI or ARB, andreduction of Scr in the ii subgroup was superior to that in the iii subgroup; 3)reduction of BUN in group A was notbetter than that in group B. Conclusion: In summary, CHMs combined with ACEI/ARB can decrease UAER andScr significantly compared to the use of ACEI/ARB during IDN treatment. The effect was more significant in theCHM group containing Astragali Radix or Rehmanniae Radix. The application of Astragali Radix and RehmanniaeRadix should be emphasized in third stage diabetic nephropathy. 展开更多

Objective:To analyze the registration information of Corona Virus Disease 2019(COVID-19)related clinical trials from all the clinical trial registry accepted by the International Committee of Medical Journal Editors(ICMJE).Methods:All the database of ICMJE-accepted clinical trial registry platform were searched for COVID-19 related clinical trials that registered from December 8,2019 to February 19,2020.Results:All the database totally contained data of 209 COVID-19 related clinical trials,including 66(31.6%)traditional Chinese medicine(TCM)related studies and 143(68.4%)non-TCM related studies.176(84.2%)clinical trials were registered in ChiCTR,as 33(15.8%)were registered in ClinicalTrials.gov.There were 152(72.7%)intervention trials,including 9(4.3%in total)preventive trials,136(65.1%in total)therapeutic trials and 7(3.3%in total)convalescent trials.Conclusion:At the present stage,clinical studies are still aiming at exploring the effective treatment of COVID-19,and the existing treatments need more high-quality clinical evidence supporting.Traditional Chinese medicine may have played an important role in all stages of the epidemic preventing and control,and the enthusiasm of carrying out scientific research has also been greatly improved.The relevant policies of clinical trial registration should be popularized among researchers,including traditional Chinese medicine researchers.And the methodological quality of clinical trials’design urges improving. 展开更多

Objective:To study the relationship between the recovery of knee joint injury and traditional Chinese medicine constitution of young male amateur cyclist in Hainan.Methods:720 young male amateur cyclists were selected from the tour nanli lake national wetland park cycling race and Hainan international tourism island cycling league(ding'an station)in 2019.According to the classification criteria of TCM constitutions in classification and determination of TCM constitutions revised by the Chinese association of traditional Chinese medicine in April 2009,the TCM constitutions were divided into mild constitution group and Yin deficiency constitution group.Standard Kujala knee pain questionnaire was used to evaluate knee joint function in pinyin and Yin deficiency groups.To observe the relationship between TCM constitution and functional injury and recovery degree of knee joint.Results:Body mass index(BMI)of Yin deficiency quality was lower than that of pinyin deficiency quality(P<0.01).Score of knee flexion and extension degree was higher in Yin deficiency quality 30 minutes before the race(P<0.05).The score of Yin deficiency quality was lower after 30 minutes of squatting(P<0.01)and jumping(P<0.05).The total score of knee joint was higher 5 days after the race(P<0.01).The knee flexion and extension(P<0.05)and jump(P<0.05)were higher in the pre-race and post-race scores.Conclusion:There is a high correlation between the constitution of traditional Chinese medicine and the degree of functional injury and recovery of knee joint.The degree of post-game sports injury is higher and the recovery of post-game sports injury is faster.Therefore,different sports protection schemes can be formulated for different constitutions in bicycle RACES to reduce the occurrence of sports injuries and accelerate the recovery time of sports injuries. 展开更多

Obesity represents a social health problem worldwide, associated with serious health risks and increased mortality. The prevalence of obesity is reported to be increasing in both developed and developing countries. Obesity is associated with a significant range of comorbidities and is linked with increases in mortality, thus the treatment of obesity is very important. Chinese herbal medicine (CHM) has been used for weight management both in China and in western countries for many years, the effectiveness and safety of CHMs in obesity have been proved. Yet the principles of treating obesity with CHMs are hard to manage due to the complexity of TCM theory. In this study, a novel text mining method was developed based on a comprehensive collection of literatures in order to explore the treatment principles more intuitively. Networks of TCM patterns and CHMs which are most frequently used in obesity treatment are built-up and analyzed, two major principles are explored in treating obesity: one is resolving phlegm and dampness, the other is clearing heat and reinforcing deficiency. These findings might guide the clinicians in treatment of obesity. 展开更多

Traditional Chinese medicine(TCM)clinical efficacy evaluation runs through the development of TCM and has been progressing so far.With the introduction of evidence-based medicine(EBM)in recent 20 years,TCM clinical efficacy evaluation has developed rapidly,accumulated research and experience,improved the quality and standardization of research,but also gradually found deep-seated deficiencies.In 2019,the China Center for Evidence-based Traditional Chinese Medicine(CCEBTCM)was established,and TCM was included in the 11th revision of the International Statistical Classification of Diseases(ICD-11).In above opportunities for TCM clinical efficacy evaluation,it is necessary to face up to the previous deficiencies,so that the TCM clinical efficacy evaluation can truly reflect the actual clinical practice characteristics,including TCM clinical problems,TCM thinking and TCM diagnosis&treatment mode.Based on the summary and analysis of past research,development concepts are proposed in this article.On the one hand,original concepts of TCM should be confidently paid more attention to the design and implementation of TCM clinical efficacy evaluation to be close to TCM clinical practice.On the other hand,it is necessary to consolidate the foundation of TCM clinical practice before evaluating efficacy.Evaluated object built on TCM ontology knowledge should be demonstrated based on clinical practice,and unknown evaluated object combined with new methods,technologies and concepts should be maturely integrated in clinical practice firstly. 展开更多

AIM: To prepare the specific magnetic resonance(MR) probes for detection of hepatocellular carcinoma(HCC) using one-pot method.METHODS: The carboxylated dextran-coated nanoparticles were conjugated with anti-α-fetoprotein(anti-AFP) or anti-glypican 3(anti-GPC3) antibodies through 1-(3-dimethylaminopropyl)-3-ethylcarbodiimide hydrochloride/N-hydroxysuccinimide(EDC/NHS)-mediated reaction to synthesize the probes.The physical and chemical properties of the probes were determined by transmission electron microscopy(TEM) and dynamic light scattering, and the relaxivity was compared to uncombined ultrasmall superparamagnetic iron oxide nanoparticles(USPIONs) using a 1.5T clinical MR scanner.The binding efficiency of the antibodies to nanoparticles was measured with an ultravioletvisible spectrophotometer.In addition, the probes were incubated with targetable cells in vitro.RESULTS: The superparamagnetic MR probes(antiGPC3-USPION probe and anti-AFP-USPION probe) were synthesized using one-pot method.Their mean hydrodynamic diameter was 47 nm with a broader slight size distribution.The coupling efficiency of carboxylated dextran-coated ultrasmall superparamagnetic iron oxide(USPIO) with anti-GPC3 or anti-AFP antibody was 15.9% and 88.8%, respectively.Each of the USPIO nanoparticles may bind 3 GPC3 antibodies or 12 AFP antibodies.The statistical analysis showed no significance(P > 0.05) in shortening the T1 and T2 values when comparing the USPIO-AFP or USPIOGPC3 to USPIO.Analysis of TEM images revealed that anti-GPC3-USPION probes and anti-AFP-USPION probes could specifically enter into the Hep G2 cell by combining with the GPC3 receptors or AFP receptors, whereas the Hep G2 cell sample incubated with USPIONs showed no or few nanoparticles in the cytoplasm.CONCLUSION: The synthesized probes using one-pot method can be used for in vitro experimental study and have potential clinical application in MR imaging for detection of hepatocellular carcinomas. 展开更多

This study investigated the effects of Yishendaluo decoction on the loss of blood-brain barrier integrity in mice exhibiting experimental autoimmune encephalomyelitis.To this end,we used real-time fluorescent quantitative PCR to measure the levels of mRNAs specific to the T cell markers CD4 and CD8,and the monocyte marker CD11b.In addition,we used Evans blue dye extravasation in the spinal cord and brain tissues to assess blood-brain barrier permeability.The results indicated that an increase in blood-brain barrier permeability was associated with an increase in CD4,CD8 and CD11b mRNA expression in experimental autoimmune encephalomyelitis mice.Yishendaluo decoction administration significantly reversed inflammatory cell accumulation in cerebral tissues of experimental autoimmune encephalomyelitis mice. 展开更多

The Chinese herbal formula Tongluo fiunao, containing the active components Panax notogin- seng and Gardenia jasminoides, has recently been patented and is in use clinically. It is known to be neuroprotective in cerebral ischemia, but the underlying pathway remains poorly understood. In the present study, we established a rat model of cerebral ischemia by occlusion of the middle cerebral artery, and administered Tongluo Jiunao, a positive control (Xuesai Tong, containing Panax notoginseng) or saline intraperitoneally to investigate the pathway involved in the action of TongIuo fiunao injection. 2,3,5-Triphenyltetrazolium chloride (TTC) staining showed that the cerebral infarct area was significantly smaller in model rats that received Tongluo fiunao than in those that received saline. Enzyme-linked immunosorbent assay revealed significantly greater expression of neurotrophin 3 and growth-associated protein 43 in ischemic cerebral tissue, and serum levels of neurotrophin 3, in the Tongluo Jiunao group than in the saline group. The reverse transcription polymerase chain reaction and immunohistochemical staining showed that after treatment with Tongluo fiunao or Xuesai Tong, tropomyosin-related kinase C gene expression and immunoreactivity were significantly elevated compared with saline, with the greatest expression observed after Tongluo Jiunao treatment. These findings suggest that Tongluo Jiunao injection exerts a neuroprotective effect in rats with cerebral ischemia by activating the neurotrophin 3/ tropomyosin-related kinase C pathway. 展开更多

Objective:To systematically review the efficacy and safety of Shenshuaining granule combined with Western medicine treatment for Diabetic kidney disease(DKD)by meta-analysis.Methods:Databases including CNKI,WanFang Data,VIP,CBM,TheCochrane,Embase and PubMed were electronically searched from inception to May 2020 to collect randomized controlled trials of Shenshuaining granule with western medicine treatment for Diabetic nephropathy.The trial was screenedbased on inclusion and exclusion criteria,and the methodological quality of the included relevant trial was assessed by Cochrane Handboo-k.Meta-analysis was then performed by Revman 5.3 software.Results:A total of 9 studies involving 892 patients were included.Both groupinclude 446 patients.The result of meta-analysis showed that Shenshuaining granule combined with Western medicine treatment significantly improved the total effective rate of diabetic nephropathy[RR=1.26,95%CI(1.17,1.37),P<0.00001],further reduced ser umurea nitrogen[MD=-1.65,95%CI(-2.53,-0.77),P=0.0002],24h urine albumin excretion rat[SMD=-4.21,95%CI(-6.23,-2.19),P<0.0001],serum creatinine[MD=-21.50,95%CI(-28.48,-14.50),P<0.00001].However,the difference of fasting blood glucose[MD=-0.46,95%CI(-1.18,0.27),P=0.22]was not statistically significant.Conclusions:Shenshuaining granule can improve the rate o-f renal pellet filtration in patients with diabetic nephropathy,reduce kidney injury and protect kidney function.Although there are sideeffects,it is relatively safe. 展开更多

Objective:To evaluate the efficacy and safety of Shen Zhi Ling oral liquid(SZL)alone or combined with basic treatment in treating Alzheimer's Disease(AD).Methods:Databases including PubMed,the Cochrane Library,Embase,Web of science,China National Knowledge Infrastructure(CNKI),Wanfang Database and the Chinese Science and Technology Journal Database(VIP)were searched from inception to September 20,2020 to collect relevant randomized controlled trials(RCTs)comparing SZL with other nootropic drugs for AD.The primary clinical outcomes were scores of the Mini Mental State Examination(MMSE)and the Activities of Daily Living(ADL)during treatment.The secondary outcomes of our study included other neuropsychological scale scores including Rapid Verbal Retrieve(RVR),Digit Span(DS),Fuld Object-Memory Evaluation(FOM)and adverse events or reactions.Two authors searched and screened the literature independently,then extracted the data and evaluated the methodological quality by using the handbook of Cochrane Collaboration,and analyzed the data via the Review Manager 5.3 software.Results:In total,we included four studies,covering 217 participants.All studies were in a low methodological quality.The result of MMSE score,ADL score,RVR score,DS score and FOM score in the meta-analysis showed that no statistically significant difference(P>0.05)between the treatment groups and control groups,and the statistical heterogeneity of all meta-analyses was small(I2<50%).SZL was indicated no better than other nootropic drugs.Among the included studies,no study described adverse events.Conclusions:Considering the low quality of the included trials and the different levels of AD patients,we are unable to come to any conclusion about the efficacy and safety of SZL compared with other nootropic drugs.Moreover,we still need rigorously designed,multi-center,large-scale trials to further confirm the efficacy and safety of SZL. 展开更多

Objective:To explore the association of Chinese medicine constitution susceptibility to diabetic nephropathy(DN) and transforming growth factor(TGF)-β1(T869C) gene polymorphism.Methods:TGF-β1 gene polymorphism detected with polymerase chain reaction-restriction fragment length polymorphism(PCRRFLP) was screened for 180 DN cases and 180 type 2 diabetic mellitus(T2DM) cases without combined DN. Patients with DN were surveyed epidemiologically with constitution in the Chinese medicine questionnaire (CCMQ).Binary logistic regression analysis was utilized to study the correlation between nine types of Chinese medicine constitution and TGF-β1(T869C) gene polymorphisms.Results:The DN group has a higher frequency of TGF-β1(T869C) gene polymorphism than the T2DM group,and CC/CT genotypes than the T2DM group[CC,CT,TT(DN group):88,87,5(cases) versus(T2DM group) 71,73,36(cases),P0.05].The phlegm-dampness constitution,damp-heat constitution,and blood stasis constitution have correlations with TGF-β1 (T869C) gene polymorphism.Conclusion:Chinese medicine constitutions were associated with TGF-β1(T869C) gene polymorphism,a potential predictor of susceptibility to DN in T2DM patients. 展开更多

Kidney-tonifying recipe can reduce the accumulation of advanced glycation end products, prevent neuronal degeneration and improve cognitive functions in ovariectomized rats. Radix Achyranthis Bidentatae alcohol extracts may dose-dependently inhibit non-enzymatic saccharification in vitro. This study aimed to examine the effect of Radix Achyranthis Bidentatae on advanced glycation end products and on learning and memory capabilities in ovariectomized rats. Ovariectomized rats were treated with Radix Achyranthis Bidentatae alcohol extracts (containing 1.5 g/kg crude drug) or 0.1% aminoguanidine for 12 weeks and behavioral testing was performed with the Y-electrical maze. This test revealed that Radix Achyranthis Bidentatae and aminoguanidine could improve the learning and memory capabilities of ovariectomized rats. Results of competitive enzyme-linked immunosorbent assay showed that treatment with Radix Achyranthis Bidentatae or aminoguanidine reduced the accumulation of advanced glycation end products in the frontal cortex of ovariectomized rats, while increasing content in the blood and urine. Biochemical tests showed that treatment with Radix Achyranthis Bidentatae or aminoguanidine decreased superoxide dismutase activity in the serum and frontal cortex, and increased serum levels of glutathione peroxidase in ovariectomized rats. In addition there was no apparent effect on malondialdehyde levels. These experimental findings indicate that Radix Achyranthis Bidentatae inhibits production of advanced glycation end products and its accumulation in brain tissue, and improves learning and memory capabilities in ovariectomized rats. These effects may be associated with an anti-oxidative action of the extract. 展开更多

OBJECTIVE:To evaluate the effectiveness of Xiaoyin Jiedu(XYJD)granules in the treatment of psoriasis vulgaris(PSV)in patients with a blood-heat pattern(BHP)in terms of Traditional Chinese Medicine(TCM).We also aimed to identify the possible underlying immunological mechanism.METHODS:Twenty-five PSV patients with BHP and ten normal controls were enrolled from January 1,2015 to December 31,2016.Patients were randomly assigned to either the XYJD group(15 cases)or the placebo group(10 cases),in which patients were treated with XYJD granules or a placebo,respectively.Additionally,albolene was used to relieve skin dryness in these two groups.The psoriasis area and severity indexes,dermatology life quality indexes and itching scores were assessed at the end of the 2 nd,4 th and 8 th week of treatment.The number of peripheral blood T helper(Th)9,Th17 and regulatory T cells(Tregs)and the m RNA and protein expression levels of PU.1,RAR-related orphan receptor(ROR)-γt,forkhead box protein 3(Foxp3),interleukin(IL)-9,IL-17,IL-23 and IL-10 in the control and experimental groups were compared before and after treatment.RESULTS:Psoriasis area and severity indexes and itching scores of patients in the XYJD group were significantly lower than those in the placebo group,whereas dermatology life quality indexes were significantly higher.In comparison with the placebo group,XYJD granules significantly reduced the number of Th17 cells and the m RNA and protein expression levels of Th17-related ROR-γt,IL-17,IL-22 and IL-23 in the peripheral blood and reduced the number of Th9 cells and the m RNA and protein expression levels of Th9-related PU.1 and IL-9.CONCLUSION:XYJD granules were effective against PSV in patients with BHP by reducing the number of Th9 and Th17 cells and the levels of their related cytokines. 展开更多
