Gastric cancer(GC),the third leading cause of cancer-related death globally,is complex and heterogeneous.This review explores multidisciplinary investigations of traditional Chinese medicine(TCM)combined with Western medical practices,emphasizing the development of nutraceuticals for cancer prevention.Using advanced analytical chemistry and food chemistry techniques,this study investigated how TCM components may be optimized for nutraceutical development.Focusing on molecular interactions with GC pathways,particularly the NF-κB,PI3K/Akt,and Wnt/β-catenin pathways,we examined the effects of TCM polyherbal formulas,extracts,and isolated compounds.These agents modulate apoptosis and cellular proliferation,underscoring their potential in preventive strategies.The convergence of nutraceutical and medicine food homology studies highlights a significant shift towards integrating TCM-derived compounds in a preventive health framework.This approach aims not only to enhance efficacy and reduce side effects but also to champion a preventive paradigm using personalized medicine to advance proactive health maintenance and disease prevention.The combination of TCM and western medical practices offers promising avenues for future research and practical applications in GC prevention. 展开更多

Objective:Reporting research is as important a part of a study as its design or analysis.Reporting guidelines(RGs)provide structured advice on how to report research studies clearly and adequately.This study aimed to review the development of RGs of Chinese medicine(CM)and to provide recommen-dations for improvement.Methods:Through a systematic search of the Enhancing the QUAlity and Transparency Of health Research(EQUATOR)Network and electronic databases up to January 1,2022,we identified a total of 15 RGs of CM,and further summarized their characteristics and applications.In addition,we reviewed the development of international RGs and analyzed its impact for CM.Results:Compared with the generic RGs,the reporting standards of CM have been rapidly developed over the last 10 years,of which 57%were issued in recent 3 years(2019-2021).Currently,the system of RGs of CM has been established,especially for clinical trials,including various CM interventions and covering the guidelines from trial registration,protocol,results publication to the evidence synthesis and clinical practice guideline.However,the application of RGs of CM is suboptimal.Conclusion:It is necessary to take further measures to promote practical application,improve journals’endorsement,and establish quality monitoring procedures for RGs of CM in the future. 展开更多

With the successive release of the CONSORT extensions for acupuncture,moxibustion,cupping,and Tuina/massage,this review aims to assess the reporting characteristics and quality of randomized controlled trials(RCTs)based on these specific guidelines.A comprehensive review was conducted by searching multiple databases,including Embase,Ovid MEDLINE(R),All EBM Reviews,AMED,CNKI,VIP Chinese Medical Journal Database,and Wanfang Data,for publications from January 1 to December 31,2022.Two reviewers independently evaluated the eligibility of the records,extracted predetermined information,and assessed the reporting based on the STRICTA,STRICTOM,STRICTOC,and STRICTOTM checklists.Among the included 387 studies(acupuncture,213;Tuina/massage,85;moxibustion,73;cupping,16),the overall reporting compliance averaged 56.0%,with acupuncture leading at 62.6%,followed by cupping(60.2%),moxibustion(53.1%),and Tuina/massage(47.9%).About half of the evaluated items showed poor reporting(compliance rate<65%).Notably,international journals demonstrated significantly higher reporting quality than Chinese journals(P<0.05).Although acupuncture trials had relatively higher compliance rates,deficiencies persist in reporting non-pharmacological therapies of Chinese medicine,particularly in areas like treatment environment details and provider background information. 展开更多

Constipation aff ects more than 14%of the global population with complex pathogenesis.Compound Traditional Chinese medicines(TCM),such as CDD-2101,a manufactured version of a time-tested TCM,hold great promise for constipation treatment due to the multiple-component-multiple-target strategy.Here,a novel multiple network analysis based on metabolomics and network pharmacology was developed to decipher the complex interplays between CDD-2101 and constipation.First,CDD-2101 alleviated constipation in a loperamide-induced acute constipation mice model and no adverse eff ect was observed.Then,fecal metabolomics and phytochemistry profi les were conducted by liquid chromatography-mass spectrometry analysis.The untargeted metabolomics built a metabolomic network related to signifi cantly modulated pathways during the constipation alleviation,including bile acid secretion,steroid hormone biosynthesis,glycerophospholipid metabolism,and linoleic acid metabolism.Based on the phytochemistry,network pharmacology predicted similar modulated pathways and built a hub-target network including retinoid X receptor alpha(RXRA),cytochrome P4501A1(CYP1A1),cytochrome P4501A2(CYP1A2),and phospholipase A2-IVA(PLA2G4).Third,integrated networks were built to select the key pathways and targets,as well as the key ingredients,including rhein,hesperetin,albifl orin,and magnolol from the TCM,which were corroborated by molecular docking,in vitro enzyme assays,and targeted metabolomics results.Finally,an integrated network was built to elucidate the multi-ingredient-multi-symptom linkage during the treatment.In conclusion,the novel multiple-network analysis revealed the complex“multi-component-multi-target-multi-pathway”mechanisms of CDD-2101 against constipation.Our work provides insight into the understanding of active compounds and pharmacological mechanisms,contributing to the development of natural medicines. 展开更多

Irinotecan(CPT11)chemotherapy-induced diarrhea affects a substantial cancer population due to b-glucuronidase(Gus)converting 10-O-glucuronyl-7-ethyl-10-hydroxycamptothecin(SN38G)to toxic 7-ethyl-10-hydroxycamptothecin(SN38).Existing interventions primarily address inflammation and Gus enzyme inhibition,neglecting epithelial repair and Gus-expressing bacteria.Herein,we discov-ered that dehydrodiisoeugenol(DDIE),isolated from nutmeg,alleviates CPT11-induced intestinal muco-sitis alongside a synergistic antitumor effect with CPT11 by improving weight loss,colon shortening,epithelial barrier dysfunction,goblet cells and intestinal stem cells(ISCs)loss,and wound-healing.The anti-mucositis effect of DDIE is gut microbiota-dependent.Analysis of microbiome profiling data from clinical patients and CPT11-induced mucositis mice reveals a strong correlation between CPT11 chemotoxicity and Gus-expressing bacteria,particularly Enterococcus faecalis(E.faecalis).DDIE coun-ters CPT11-induced augmentation of E.faecalis,leading to decreased intestinal Gus and SN38 levels.The Partial Least Squares Path Model(PLS-PM)algorithm initially links E.faecalis to dysregulated epithelial renovation.This is further validated in a 3D intestinal organoid model,in which both SN38 and E.faecalis hinder the formation and differentiation of organoids.Interestingly,colonization of E.fae-calis exacerbates CPT11-induced mucositis and disturbs epithelial differentiation.Our study unveils a microbiota-driven,epithelial reconstruction-mediated action of DDIE against mucositis,proposing the‘Gus bacteriaehosteirinotecan axis’as a promising target for mitigating CPT11 chemotoxicity. 展开更多

Tumor microenvironment contributes to poor prognosis of pancreatic adenocarcinoma(PAAD)patients.Proper regulation could improve survival.Melatonin is an endogenous hormone that delivers multiple bioactivities.Here we showed that pancreatic melatonin level is associated with patients'survival.In PAAD mice models,melatonin supplementation suppressed tumor growth,while blockade of melatonin pathway exacerbated tumor progression.This anti-tumor effect was independent of cytotoxicity but associated with tumor-associated neutrophils(TANs),and TANs depletion reversed effects of melatonin.Melatonin induced TANs infiltration and activation,therefore induced cell apoptosis of PAAD cells.Cytokine arrays revealed that melatonin had minimal impact on neutrophils but induced secretion of Cxcl2 from tumor cells.Knockdown of Cxcl2 in tumor cells abolished neutrophil migration and activation.Melatonin-induced neutrophils presented an N1-like anti-tumor phenotype,with increased neutrophil extracellular traps(NETs)causing tumor cell apoptosis through cell-to-cell contact.Proteomics analysis revealed that this reactive oxygen species(ROS)-mediated inhibition was fueled by fatty acid oxidation(FAO)in neutrophils,while FAO inhibitor abolished the anti-tumor effect.Analysis of PAAD patient specimens revealed that CXCL2 expression was associated with neutrophil infiltration.CXCL2,or TANs,combined with NET marker,can better predict patients'prognosis.Collectively,we discovered an anti-tumor mechanism of melatonin through recruiting N1-neutrophils and beneficial NET formation. 展开更多
