Aquaporin-4-IgG-seropositive neuromyelitis optica spectrum disorders:progress of experimental models based on disease pathogenesis

Neuromyelitis optica spectrum disorders are neuroinflammatory demyelinating disorders that lead to permanent visual loss and motor dysfunction.To date,no effective treatment exists as the exact causative mechanism remains unknown.Therefore,experimental models of neuromyelitis optica spectrum disorders are essential for exploring its pathogenesis and in screening for therapeutic targets.Since most patients with neuromyelitis optica spectrum disorders are seropositive for IgG autoantibodies against aquaporin-4,which is highly expressed on the membrane of astrocyte endfeet,most current experimental models are based on aquaporin-4-IgG that initially targets astrocytes.These experimental models have successfully simulated many pathological features of neuromyelitis optica spectrum disorders,such as aquaporin-4 loss,astrocytopathy,granulocyte and macrophage infiltration,complement activation,demyelination,and neuronal loss;however,they do not fully capture the pathological process of human neuromyelitis optica spectrum disorders.In this review,we summarize the currently known pathogenic mechanisms and the development of associated experimental models in vitro,ex vivo,and in vivo for neuromyelitis optica spectrum disorders,suggest potential pathogenic mechanisms for further investigation,and provide guidance on experimental model choices.In addition,this review summarizes the latest information on pathologies and therapies for neuromyelitis optica spectrum disorders based on experimental models of aquaporin-4-IgG-seropositive neuromyelitis optica spectrum disorders,offering further therapeutic targets and a theoretical basis for clinical trials. 展开更多

BACKGROUND Atypical optic neuritis,consisting of neuromyelitis optica spectrum disorders(NMOSD)or myelin oligodendrocyte glycoprotein antibody disease(MOGAD),has a very similar presentation but different prognostic implications and longterm management strategies.Vascular and metabolic factors are being thought to play a role in such autoimmune neuro-inflammatory disorders,apart from the obvious immune mediated damage.With the advent of optical coherence tomography angiography(OCTA),it is easy to pick up on these subclinical macular microvascular and structural changes.AIM To study the macular microvascular and structural changes on OCTA in atypical optic neuritis.METHODS This observational cross-sectional study involved 8 NMOSD and 17 MOGAD patients,diagnosed serologically,as well as 10 healthy controls.Macular vascular density(MVD)and ganglion cell+inner plexiform layer thickness(GCIPL)were studied using OCTA.RESULTS There was a significant reduction in MVD in NMOSD and MOGAD affected as well as unaffected eyes when compared with healthy controls.NMOSD and MOGAD affected eyes had significant GCIPL thinning compared with healthy controls.NMOSD unaffected eyes did not show significant GCIPL thinning compared to healthy controls in contrast to MOGAD unaffected eyes.On comparing NMOSD with MOGAD,there was no significant difference in terms of MVD or GCIPL in the affected or unaffected eyes.CONCLUSION Although significant microvascular and structural changes are present on OCTA between atypical optic neuritis and normal patients,they could not help in differentiating between NMOSD and MOGAD cases. 展开更多

H型栅NMOS器件因其强抗辐照和低功耗等优势已逐渐成为PDSOI电路设计中的核心器件。但H型栅NMOS器件的Ids-Vds曲线会在漏极电压较高时发生明显的翘曲现象,称为Kink效应。该效应严重影响一定工作条件下的电路性能和稳定性。为此,依据实测和TCAD仿真数据,分析了H型栅NMOS器件发生Kink效应的机理,并且基于0.15μm SOI工艺,进一步量化分析了顶层硅膜厚度、阱浓度、栅尺寸、温度以及总剂量辐照等方面对Kink效应的影响。最终结果表明,高漏极电压下NMOS器件体区积累大量空穴导致寄生NPN三极管开启,从而引发了Kink效应。本工作完善了H型栅NMOS器件Kink效应的研究,为PDSOI电路设计中抑制Kink效应提供了有益的参考。 展开更多

Objective:Using neuromyelitis optica immunoglobulin G(NMO-IgG)to induced ex vivo mice spinal cord slice model.Methods:Vibratome-cut transverse spinal cord slices from 7-day-old C57BL/6Jmouse pups were cultured on transwell porous supports for 7days,then randomly divided into the control group and NMO model group.Slices of the control group were further cultured with human serum complement,while slices from NMO model group were exposed to complement and NMO-IgG.After 24-hour incubation,slices of both groups were measured for aquaporin-4(AQP4),glial fibrillary acidic protein(GFAP),myelin basic protein(MBP)and neurofilament light chain(NFL)by immunofluorescence.Results:Slices exposed to NMO-IgG showed astrocyte swelling,and a significant loss of AQP4and GFAP staining.Ratios of the loss of AQP4and GFAP staining were 77.74%±6.75%and 75.62%±5.76%respectively in the model group,and NMO-like injury score was 3.11±0.60.But there were no obvious losses of AQP4and GFAP staining in the control group,and NMO-like lesion score was 0.00.There were significant differences between the two groups with regards to the above indexes(P<0.01).Ratios of the loss of MBP and NFL staining in the model group were 37.60%±4.88%and46.29%±4.98%respectively,while the corresponding figures in the control group were 9.10%±1.63%and 5.80%±0.81%,and the differences between the two groups were statistically significant(P<0.01).Conclusion:These results suggested that NMO-IgG-induced ex vivo spinal cord slice model possesses typical features of NMO,and this model might be useful for relevant fundamental studies. 展开更多

Neuromyelitis optica is an inflammatory demyelinating disease of the central nervous system that differs from multiple sclerosis.Over the past 20 years,the search for biomarke rs for neuromyelitis optica has been ongoing.Here,we used a bibliometric approach to analyze the main research focus in the field of biomarkers for neuromyelitis optica.Research in this area is consistently increasing,with China and the United States leading the way on the number of studies conducted.The Mayo Clinic is a highly reputable institution in the United States,and was identified as the most authoritative institution in this field.Furthermore,Professor Wingerchuk from the Mayo Clinic was the most authoritative expe rt in this field.Keyword analysis revealed that the terms "neuro myelitis optica"(261 times), "multiple sclerosis"(220 times), "neuromyelitis optica spectrum disorder"(132 times), "aquaporin4"(99 times),and "optical neuritis"(87 times) were the most frequently used keywords in literature related to this field.Comprehensive analysis of the classical literature showed that the majority of publications provide conclusive research evidence supporting the use of aquaporin-4-IgG and neuromyelitis optica-IgG to effectively diagnose and differentiate neuromyelitis optica from multiple sclerosis.Furthermore,aquaporin-4-IgG has emerged as a highly specific diagnostic biomarker for neuromyelitis optica spectrum disorder.Myelin oligodendrocyte glycoprotein-IgG is a diagnostic biomarke r for myelin oligodendrocyte glycoprotein antibody-associated disease.Recent biomarkers for neuromyelitis optica in clude cerebrospinal fluid immunological biomarkers such as glial fibrillary acidic protein,serum astrocyte damage biomarkers like FAM19A5,serum albumin,and gammaaminobutyric acid.The latest prospective clinical trials are exploring the potential of these biomarkers.Preliminary results indicate that glial fibrillary acidic protein is emerging as a promising candidate biomarker for neuromyelitis optica spectrum disorder.The ultimate goal of future research is to identify non-invasive biomarkers with high sensitivity,specificity,and safety for the accurate diagnosis of neuro myelitis optica. 展开更多

目的应用光学相干断层扫描(optical coherence tomography,OCT)及血管成像(optical coherence tomography angiography,OCTA)评价视神经脊髓炎谱系疾病(neuromyelitis optica spectrum disorders,NMOSD)视网膜结构和血管密度改变及其与... 目的应用光学相干断层扫描(optical coherence tomography,OCT)及血管成像(optical coherence tomography angiography,OCTA)评价视神经脊髓炎谱系疾病(neuromyelitis optica spectrum disorders,NMOSD)视网膜结构和血管密度改变及其与临床疾病特征的相关性。方法共纳入水通道蛋白-4抗体阳性的NMOSD患者42例(84只眼),健康对照(对照组)45名(90只眼)。根据视神经炎(optic neuritis,ON)病史,NMOSD组进一步分为两组:有ON病史的NMOSD+ON组和无ON病史的NMOSD-ON组。通过OCT和OCTA测量神经节细胞复合体(ganglion cell complex,GCC)厚度、视网膜神经纤维层(retinal nerve fiber layer,RNFL)厚度、黄斑区浅层毛细血管丛(superficial capillary plexus,SCP)、深层毛细血管丛(deep capillary plexus,DCP)和视盘区血管的血管密度。记录患者NMOSD病程和发作次数、最佳矫正视力(best corrected visual acuity,BCVA)和扩展残疾状态量表(expanded disability status scale,EDSS)评分,分析上述指标与OCT和OCTA观察指标之间的相关性。结果黄斑和视盘的结构改变:与对照组比较,NMOSD+ON组总体及颞区(TEM)、上区(SUP)、鼻区(NAS)、下区(INF)各区的RNFL厚度均明显降低(P<0.05);NMOSD-ON组总体和SUP区的RNFL厚度明显降低(P<0.05)。与NMOSD-ON组比较,NMOSD+ON组总体及TEM、SUP、NAS、INF各区的RNFL厚度均明显降低(P<0.05)。与对照组比较,NMOSD+ON组总体、SUP及INF区的GCC厚度明显降低(P<0.05);NMOSD-ON组总体及SUP区的GCC厚度明显降低(P<0.05)。与NMOSD-ON组比较,NMOSD+ON组总体GCC厚度及SUP、INF区的GCC厚度明显降低(P<0.05)。黄斑区血管密度:与对照组比较,NMOSD+ON组SCP总体、中心凹、中心凹旁及中心凹周围TEM、SUP、NAS、INF各区血管密度均明显降低(P<0.05)。与对照组比较,NMOSD-ON组DCP总体血管密度、中心凹旁及中心凹周围TEM、SUP、NAS、INF各区血管密度均明显降低(P<0.05)。与NMOSD-ON组比较,NMOSD+ON组SCP总体血管密度以及中心凹旁TEM区、SUP区、INF区和中心凹周围TEM、SUP、NAS、INF各区血管密度均明显降低(P<0.05),DCP中心凹、中心凹旁NAS区、中心凹周围TEM区血管密度明显升高(P<0.05)。与对照组比较,NMOSD+ON组视盘区总体血管密度,视盘内血管密度以及和视盘旁鼻上区(NS)、鼻下区(NI)、下鼻区(IN)、下颞区(IT)、颞下区(TI)、颞上区(TS)、上颞区(ST)、上鼻区(SN)各区血管密度均明显降低(P<0.05)。与NMOSD-ON组比较,NMOSD+ON组除视盘内血管密度差异无统计意义(P=0.268)外,视盘区总体血管密度和视盘旁各区血管密度均明显降低(P<0.05)。NMOSD组病程与GCC厚度、RNFL厚度、SCP血管密度、放射状周围毛细血管(RPC)血管密度均呈负相关(分别r=-0.436,P<0.001;r=-0.337,P=0.002;r=-0.325,P=0.003;r=-0.239,P=0.028),发病次数与RNFL厚度、RPC血管密度呈负相关(分别r=-0.360,P=0.001;r=-0.315,P=0.004),BCVA与GCC厚度呈负相关(r=-0.303,P=0.005)。结论NMOSD患者发生和不发生ON的眼,均存在视网膜结构和视网膜血管的损伤,且NMOSD病程越长、发病次数越多,损伤越明显。与没有ON的眼相比,ON患眼SCP血管密度减低。 展开更多

Comparison of inebilizumab or rituximab in addition to glucocorticoid therapy for neuromyelitis optica spectrum disorders

AIM:To investigate the short-term efficacy and safety of inebilizumab for neuromyelitis optica spectrum disorders(NMOSD).METHODS:A total of 33 patients with NMOSD treated with inebilizumab(Group INB,n=15)or rituximab(Group RTX,n=18)in addition to high-dose glucocorticoids were included.Both groups underwent hormone shock therapy during the acute phase.Subsequently,Group INB received inebilizumab injections during the remission phase,while Group RTX received rituximab injections.A comparison of aquaporins 4(AQP4)titer values,peripheral blood B lymphocyte counts,and visual function recovery was conducted before and 8wk after treatment.Additionally,adverse reactions and patient tolerability were analyzed after using inebilizumab treatment regimes.RESULTS:Following inebilizumab treatment,there was a significantly improvement in the visual acuity of NMOSD patients(P<0.05),accompanied by a notable decrease in AQP4 titer values and B lymphocyte ratio(P<0.05).Moreover,inebilizumab treatment showed a partial effect in preventing optic nerve atrophy(P<0.05).However,there were no significant differences in other therapeutic effects compared to rituximab,which has previously demonstrated substantial therapeutic efficacy(P>0.05).Furthermore,inebilizumab exhibited higher safety levels than that of rituximab injections.CONCLUSION:The combination of inebilizumab and high-dose glucocorticoids proves to be effective.In comparison to rituximab injections,inebilizumab displays better tolerance and safety.Moreover,it demonstrates a partial effect in preventing optic nerve atrophy.Thus,it stands as an effective method to reduce the disability rates and improve the daily living ability of patients with NMOSD. 展开更多

Objective:To analyze the different clinical features of patients with early-onset(EO-NMOSDs)and late-onset neuromyelitis optica spectrum diseases(LO-NMOSDs).Methods:A total of 51patients with neuromyelitis optica spectrum disease who were diagnosed in our hospital for the first time from January 2015 to December 2022 were included in the First Affiliated Hospital of Hainan Medical College and divided into 22 cases in the EO-NMOSDs group and 29 cases in the LO-NMOSDs group according to whether the age of onset was 50 years old.The basic data,Extended Disability Status Scale(EDSS)score,blood and cerebrospinal fluid test indicators of the two groups were statistically analyzed.Results:There were no significant differences in demographic characteristics,clinical features and serum AQP-4 antibody positivity rate between the two groups(all P>0.05),and there were significant differences in triglycerides(TG),low-density lipoprotein(LDL),apolipoprotein A(APOA),apolipoprotein B(APOB)and lipoprotein a(P=0.010,P=0.048,P=0.014,P=0.061,P=0.001,respectively),and cerebrospinal fluid LDH,There were significant differences between microprotein quantification and EDSS score(P=0.018,P=0.034,P=0.025,respectively),and the level of microprotein quantification in cerebrospinal fluid of LO-NMOSDs had a certain correlation with the degree of disability(r=0.52,P<0.03).Conclusion:LO-NMOSDs and EO-NMOSDs group patients have similar demographic characteristics,serum AQP-4 antibody positive rate and clinical features,but compared with EO-NMOSDs,patients in LO-NMOSDs group are prone to abnormal lipid metabolism,higher trace proteins in cerebrospinal fluid and more likely to be disabled,and among LO-NMOSDs,the higher the trace protein in the cerebrospinal fluid,the more severe the disability status of patients. 展开更多

目的对比利妥昔单抗(RTX)与吗替麦考酚酯(MMF)治疗视神经脊髓炎(NMO)患者的效果。方法回顾性选择2020年1月至2022年10月于南阳市中心医院接受治疗的69例NMO患者为研究对象,并按治疗方法分成RTX组(37例)和MMF组(32例),治疗和随访时长为6... 目的对比利妥昔单抗(RTX)与吗替麦考酚酯(MMF)治疗视神经脊髓炎(NMO)患者的效果。方法回顾性选择2020年1月至2022年10月于南阳市中心医院接受治疗的69例NMO患者为研究对象,并按治疗方法分成RTX组(37例)和MMF组(32例),治疗和随访时长为6个月。比较两组视力情况[平均最小分辨角对数(logMAR)矫正视力、视野平均缺损度(MD)、平均敏感度(MS)]、扩展残疾状态量表(EDSS)评分、疾病复发情况和生存质量(MSIS-29)评分的差异。结果两组治疗后的平均IogMAR矫正视力、MS均高于同组治疗前,MD、EDSS各维度评分均低于同组治疗前(P<0.05),但组间差异无统计学意义(P>0.05);两组治疗后平均复发次数低于同组治疗前,且RTX组低于MMF组(P<0.05);两组治疗后的MSIS-29评分均低于同组治疗前,且RTX组的生存质量改善程度优于MMF组(P<0.05)。结论RTX和MMF均在NMO患者的治疗中发挥了显著的效果,其在视力水平、疾病残疾改善程度方面的作用均无明显差异,但RTX更有利于降低患者的复发率,提高生活质量。 展开更多

【目的】通过小鼠侧脑室注射卡介苗(BCG)致中枢神经系统结核分枝杆菌(MTB)感染,检测小鼠血清中是否有视神经脊髓炎(NMO)-IgG/水通道蛋白4(AQP4)-Ab的表达。【方法】雌性C57BL/6J小鼠40只,6~8周,随机分为4组:5×106CFU BCG组、1... 【目的】通过小鼠侧脑室注射卡介苗(BCG)致中枢神经系统结核分枝杆菌(MTB)感染,检测小鼠血清中是否有视神经脊髓炎(NMO)-IgG/水通道蛋白4(AQP4)-Ab的表达。【方法】雌性C57BL/6J小鼠40只,6~8周,随机分为4组:5×106CFU BCG组、1×106CFU BCG组、0.01 mol/L PBS组、正常组。右侧侧脑室注射,注射后1周脑室注射追加1次。小鼠BCG注射后第7、14、21、28天,分别取血并检测血清中NMO-IgG/AQP4-Ab。注射后第28天处死小鼠行抗酸杆菌培养,并对脑和脊髓进行HE、抗酸、LFB髓鞘染色,免疫组化检测病灶处AQP4、GFAP的表达及C5b9沉积情况。【结果】脑室注射后,BCG组小鼠行为活动降低,出现肢体瘫痪,甚至死亡,其中5×106CFU BCG组表现严重,死亡率高。注射卡介苗21 d时小鼠血清检测发现5×106CFU BCG组中2只小鼠血清NMO-IgG/AQP4-Ab阳性,1×106CFU BCG组中4只小鼠血清中抗体阳性。第28天时5×106CFU BCG组的两只血清阳性的小鼠死亡,1×106CFU BCG组中原抗体阳性的小鼠血清中抗体仍为阳性。病理检测显示脑和脊髓出现炎性细胞浸润,病灶处抗酸染色阳性,出现脱髓鞘改变。组织培养3周后可见MTB生长,免疫组化显示病灶区域AQP4、GFAP表达降低,部分炎性细胞及血管周围出现补体C5b9沉积。【结论】中枢神经系统MTB直接感染可诱导产生NMO-IgG/AQP4-Ab。 展开更多

【目的】建立NMO-IgG/anti-AQP4抗体检测的间接免疫荧光法(IIF),细胞分析法(CBA)和荧光免疫沉淀法(FIPA)法;研究并比较不同的检测方法对中国人群NMO疾病谱的检测效率。【方法】建立3种检测NMO-IgG/anti-AQP4抗体的方法,包括IIF、CBA和F... 【目的】建立NMO-IgG/anti-AQP4抗体检测的间接免疫荧光法(IIF),细胞分析法(CBA)和荧光免疫沉淀法(FIPA)法;研究并比较不同的检测方法对中国人群NMO疾病谱的检测效率。【方法】建立3种检测NMO-IgG/anti-AQP4抗体的方法,包括IIF、CBA和FIPA。应用盲法检测病人血清106例,其中视神经脊髓炎(NMO)组24例,纵形扩展性脊髓炎(LETM)组22例,多发性硬化(MS)组30例,健康对照(HC)组30例;对其检测的阳性率进行分析。【结果】通过3种方法检测,发现NMO组NMO-IgG/anti-AQP4抗体阳性率分别为IIF(62.5%,15/24),CBA(91.7%,22/24)及FIPA(75.0%,18/22),LETM组为IIF(59.1%,13/22),CBA(86.4%,19/22)及FIPA(72.3%,16/22),MS组IIF、CBA及FIPA均为(3.3%,1/30)。正常对照组未见该抗体阳性表达。NMO组及LETM组血清NMO-IgG/anti-AQP4抗体阳性率明显高于MS和健康对照组。【结论】在中国人群NMO疾病谱患者中,IIF、CBA和FIPA的检测效率基本和美欧报道一致。IIF是该抗体检测的基础方法;CBA法看起来最为敏感;FIPA检测可以量化该抗体,可用于临床高通量大样品筛查。 展开更多

目的系统评价中国NMO与HR-NMO患者AQP4抗体检测对其诊断及鉴别诊断的意义,明确进一步的治疗和预后评估。方法检索中国学术期刊全文数据库(CNKI)、万方数据库、中国生物医学文献数据库(CBM disc)、Pubmed、e BASE等。严格按照纳入和排除... 目的系统评价中国NMO与HR-NMO患者AQP4抗体检测对其诊断及鉴别诊断的意义,明确进一步的治疗和预后评估。方法检索中国学术期刊全文数据库(CNKI)、万方数据库、中国生物医学文献数据库(CBM disc)、Pubmed、e BASE等。严格按照纳入和排除标准进行筛选文献。采用Stata.12进行数据处理。结果纳入11篇13组研究资料其中包含AQP4抗体阳性NMO患者350例,HR-NMO患者168例。Meta分析结果:NMO组AQP4抗体阳性率高于HR-NMO组,差异具有统计学意义,合并效应量为RR=0.66,95%Cl(0.53,0.83)。分层后CBA(细胞分析法)RR=0.636,95%Cl(0.461,0.878)显示NMO组AQP4抗体阳性率高于RH-NMO组;IIFA(间接荧光免疫法)RR=0.681,95%Cl(0.460,1.009)显示两组阳性率差异无统计学意义。结论 AQP4抗体检测对NMO疾病谱疾病诊断、治疗及预后评估具有重要的临床意义。 展开更多

【目的】旨在比较鼠脑法和细胞法检测中国人视神经脊髓炎(NMO)疾病谱(NMOSD)的滴度是否具有一致性。【方法】每个样本分别采用鼠脑法和细胞法检测,阳性血清采取倍比稀释以确定最终稀释倍数。应用盲法检测血清NMO-IgG/抗水通道蛋白4(AQP4... 【目的】旨在比较鼠脑法和细胞法检测中国人视神经脊髓炎(NMO)疾病谱(NMOSD)的滴度是否具有一致性。【方法】每个样本分别采用鼠脑法和细胞法检测,阳性血清采取倍比稀释以确定最终稀释倍数。应用盲法检测血清NMO-IgG/抗水通道蛋白4(AQP4)110例,其中视神经脊髓炎19例,复发性视神经炎(RON)17例,纵形扩展性脊髓炎(LETM)13例,多发性硬化(MS)31例,健康对照(HC)30例;对其检测的阳性率和滴度进行分析,并比较滴度与脊髓病变长度的相关性。【结果】大部分样本用两种方法检测的滴度之间都缺乏一致性。NMO组患者NMO-IgG/抗AQP4抗体鼠脑法(63.2%,12/19),细胞法(89.5%,17/19),RON组鼠脑法(47.1%,8/17),细胞法(70.6%,12/17),LETM组鼠脑法(46.2%,6/13),细胞法(69.2%,9/13),MS组鼠脑法(6.5%,2/31),细胞法(9.7%,3/31)。鼠脑法和细胞法检测所得到的NMOSD患者NMO-IgG/AQP4抗体滴度均和脊髓病变的长度无线性相关性,两种检测方法之间也缺乏滴度线性相关性。【结论】鼠脑法和细胞法检测抗体滴度的不一致性提示有必要进行多因素相关性研究以探讨影响两种检测方法抗体滴度的因素。 展开更多

目的:探讨血清视神经脊髓炎-免疫球蛋白G(NMO-IgG)阳性的视神经脊髓炎谱系疾病(NMOSDs)患者的临床和影像特点。方法:纳入34例NMO-IgG阳性NMOSDs患者,回顾性分析其临床表现、实验室指标和影像学结果。结果:34例NMOSDs患者中,男女比例为1... 目的:探讨血清视神经脊髓炎-免疫球蛋白G(NMO-IgG)阳性的视神经脊髓炎谱系疾病(NMOSDs)患者的临床和影像特点。方法:纳入34例NMO-IgG阳性NMOSDs患者,回顾性分析其临床表现、实验室指标和影像学结果。结果:34例NMOSDs患者中,男女比例为1∶16,首发年龄为(44.65±5.27)岁。首发症状为感觉异常(52.94%),肢体无力(44.12%),视力下降(29.41%),低热、嗜睡(8.82%),顽固性呃逆呕吐(8.82%),复视(2.94%),行走不稳(2.94%)。临床综合征为脊髓炎(82.35%),视神经炎(50.00%),最后区综合征(14.70%),间脑综合征(8.82%),大脑综合征伴有NMOSD特征性大脑病变(17.65%),其他脑干综合征(14.70%)。临床发作次数1~8次,平均(2.41±0.63)次,24例(70.59%)为复发-缓解病程,10例(29.41%)为单相病程。MRI表现:脑内病灶的发生率依次为脑干35.29%、大脑半球17.65%、间脑8.82%、视交叉8.82%。脊髓病灶以颈胸髓(50.00%)同时累及最为常见,其次为胸髓(17.65%)、颈髓(14.70%)。长节段脊髓受累(≥3个节段)24例(70.59%),其中11例(32.35%)≥10个椎体节段。免疫指标:血清NMO-IgG阳性率100%,脑脊液NMO-IgG阳性率29.41%;抗核抗体阳性率38.24%;甲状腺过氧化物酶抗体增高率32.35%;甲状腺球蛋白增高率17.65%。结论:NMO-IgG阳性NMOSDs患者以中年女性多见,首发症状以感觉异常、肢体无力及视力下降为多,脊髓炎和神经炎是最常见的临床综合征,多表现为复发-缓解病程;MRI病灶以颈胸髓同时受累最为常见,且多≥3个脊柱节段;常合并多种免疫指标异常。 展开更多
